A bone island is a hamartomatous malformation in the bone, unusually having a huge expansion. We report a rare case of giant bone island of femur concurrent with ipsilateral femoral head necrosis. Although roentgenographic features indicated the possible diagnosis of bone island, there were unusual findings, such as a huge expansion of the lesion, increased uptake of radionuclide on bone scan, and presence of symptoms. INTRODUCTION: To evaluate the diagnostic value of single photon emission computed tomography/computed tomography (SPECT/CT) in patients with avascular necrosis (AVN) of the femoral head.
METHODS: A search of the databases PubMed, ScienceDirect, and Google Scholar was undertaken using targeted index word. Studies satisfying inclusions and exclusions criteria were identified and additional related literature sources were considered. Analysis included pooled sensitivity and specificity, odds ratio (OR), and receiver operating characteristic (ROC) analysis.

Authors describe the results of 10 patients operated for avascular necrosis of the femoral head. The first operation was performed in October 1987. The follow-up was in average 41.9 (24-54) months, minimally 2 years. 7 patients improved, 2 deteriorated, 1 was unchanged. The analysis of the results showed that this operation is indicated only in strictly stated conditions.

In recent years diagnosis, and in particular early diagnosis, of osteonecrosis of the hip has been much improved by modern imaging modalities. Magnetic resonance imaging is widely accepted as the primary imaging modality for the early detection of this disorder and can help to facilitate the therapeutic management in later stages. However, plain radiographs, bone scans and computed tomography are still important techniques, which cannot be avoided in most cases. This paper describes the relative values of the different modalities. The staging system used is the international ARCO system.

A number of HbSS patients presenting with aseptic necrosis of the femoral head have been seen at Mulago Hospital in Kampala, Uganda. A record review of the hospital's Sickle Cell Anaemia Clinic for the period 1985-1989 revealed that aseptic necrosis of the femoral head has been diagnosed in 2.9% (47/1611) of patients attending regularly.
