Drugs and gastric damage.

The effects of aspirin, salicylate formulations and substitutes, smoking (nicotine), indomethacin, corticosteroids, phenylbutazone, ethanol, caffeine and reserpine on the gastric mucosa are discussed. The damaging effects of the drugs are considered in terms of the gastric mucosal barrier, gastric erosions, microbleeding and haematemesis and melaena and finally whether they cause peptic ulcer. There is suggestive evidence that unbuffered aspirin is a cause of haematemesis and melaena and of gastric ulcer but the incidence rates for hospital admission are low, being 10 to 15 per 100,000 heavy u

Assessment of the drug inhibitor specificity of the human liver 4-methylumbelliferone UDP-glucuronosyltransferase activity.

The data suggest that the 4MU-UDPGT activity of human liver microsomes probably contributes to the glucuronidation of a limited number of clinically used drugs. However, confirmation of this ultimately requires studies to be performed with purified isozymes, cDNAs expressed in cell culture, or specific inhibitory antibodies.

Peripherally acting analgesics.

The therapeutic effect of aspirin and the other nonsteroidal anti-inflammatory drugs derives from the peripheral inhibition of prostaglandin synthetase. Aspirin produces irreversible inhibition, whereas the inhibition triggered by the other nonsteroidal anti-inflammatory drugs is reversible. Despite proved analgesic efficacy, use of aspirin and the nonsteroidal anti-inflammatory drugs may be accompanied by a wide range of side effects of a potentially serious nature. For relief of pain, there appears to be no clear-cut superiority of one nonsteroidal anti-inflammatory drug over another, and pa

Top-Down Exfoliation of Ultrathin Chiral 2D Nanosheets for High-Resolution Gas Chromatography.

Compared with bulk crystal materials that have limitations such as high mass transfer resistance and insufficient exposure of surface action sites, the development of ultrathin two dimensional nanosheets is more attractive. Developing chiral stationary phases with both structural stability and high enantioselectivity remains a key challenge in this field. This study reported a chiral ionic organic single-crystal (BPDS-PBIM), which was constructed by the ionic self-assembly of 4,4'-biphenyldisulfonic acid (BPDS) and chiral ionic liquid (PBIM). Its distinct supramolecular environment with multip

Dual-engineering metalloimmunotherapy mediates Staphylococcus aureus virulence silencing and biofilm immune microenvironment reprogramming against implant-associated infections.

Implant-associated infections (IAIs) caused by Staphylococcus aureus (S. aureus) are notoriously recalcitrant to treatment due to the self-reinforcing interplay between bacterial virulence and a suppressive biofilm immune microenvironment (BIME). Here, we present a dual-engineering metalloimmunotherapy nanoplatform that synchronously silences bacterial virulence and reprograms host immunity to eradicate IAIs. The nanoplatform, termed HMPF, integrates a fenoprofen-loaded, polydopamine-modified hollow MnO2 core that is further cloaked with a macrophage-erythrocyte hybrid membrane, enabling bacte

Contact and photocontact allergy to ketoprofen: clinical and experimental study.

In the last 3 years, we have studied 10 patients with allergic reactions to topical ketoprofen. We have investigated the sensitization and irritant potential of the drug and the possibility of cross-reactivity with other aryl-propionic non-steroidal anti-inflammatory drugs (ibuproxam, ibuprofen, naproxen, fenoprofen, flubiprofen and tiaprofenic acid). 2 of our patients had contact dermatitis, and the other 8 photocontact dermatitis. One of our patients showed cross-reactivity between ketoprofen and ibuproxam on patch testing. In the photopatch tests, we observed cross-reactivity between ketopr

In vivo phototoxicity of non-steroidal anti-inflammatory drugs evaluated by the mouse tail technique.

Skin photosensitivity is a side-effect reported for several non-steroidal anti-inflammatory drugs (NSAID). In vitro studies have revealed phototoxic properties in a majority of these compounds with a preponderance for derivatives of propionic acid. Fourteen NSAIDs, the majority commercial preparations in clinical use, have been assayed by the mouse tail technique for in vivo phototoxicity evaluation. Intraperitoneal single doses in the range 12.5-200 mg/kg were given in combination with UVA irradiation for 5 h (total dose 54 J/cm2). The phototoxic reaction was measured 24 h later as the wet we

Clinical pharmacokinetics of drugs used in juvenile arthritis.

Juvenile arthritis is defined as the occurrence of objective evidence of arthritis for a minimum of 6 weeks, in a child 16 years of age or younger. With a reported incidence of 9 to 19.6 per 100,000 children, juvenile arthritis is considered to be a rare disease. There is no known cure; however, up to 75% of patients will undergo remission by late adolescence. Drugs used in the treatment of juvenile arthritis are divided into 2 major classes: (a) the nonsteroidal anti-inflammatory drugs (NSAIDs) including salicylates, naproxen, ibuprofen, fenoprofen, ketoprofen, flurbiprofen, indomethacin, sul

Protein Engineering of a Genetically Encodable Biosensor for Wastewater Detection of Profen NSAIDs.

Non-steroidal anti-inflammatory drugs (NSAIDs) are pervasive environmental contaminants due to their frequent and widespread use, multiple paths of release into surface and ground water supply, diversity of the chemical class, and toxicity to aquatic and other non-target species. In particular, the 2-arylpropionic acid ("profen") class of NSAIDs poses significant risks to aquatic ecosystems due to incomplete removal during wastewater treatment. Current monitoring precludes high-frequency testing at point sources. Here, we present the engineering and application of a genetically encodable, prot

Natural Biosorbents for Detecting Emerging Contaminants in Tap and River Water via Pipette Tip Solid-Phase Extraction (PT-SPE).

In this study, a pipette tip solid-phase extraction (PT-SPE) method was developed using cork powder as a natural biosorbent for the extraction of five pharmaceutical contaminants: diclofenac, ibuprofen, ketoprofen, bezafibrate, and fenoprofen. The method was applied to both spiked tap water and real river water samples. The cork powder was characterized using Fourier Transform Infrared Spectroscopy (FTIR), Scanning Electron Microscopy (SEM), and Brunauer-Emmett-Teller (BET) analysis to assess its functional groups, morphology, and surface properties. The analytes were separated and quantified

Oral manifestations of drug therapies in the geriatric patient.

Oral manifestations of drug therapies in the geriatric patient must be considered in a multifactorial approach. A logical pattern of individual "normality" must be perceived, and variation from this pattern, with the aid of both physical examination and adequate drug history, will enable the clinician to interpret protean manifestations of untoward actions of chemotherapeutic agents in and about the oral cavity.

Ibuprofen-arginine generates nitric oxide and has enhanced anti-inflammatory effects.

Ibuprofen, a chiral non-steroidal anti-inflammatory drug chemically related to fenoprofen and naproxen, has moderate but definite anti-inflammatory, analgesic and antipyretic properties, with considerably less gastrointestinal adverse effect than other drugs in the same family. Currently available in the market are preparations in which bioavailability of ibuprofen is increased by salification with various salts. We have investigated the pharmacological properties of one such salt, ibuprofen-arginine, of biological interest because l-arginine acts as substrate of the nitric oxide (NO) synthesi

Benoxaprofen stimulates proteoglycan synthesis in normal canine knee cartilage in vitro.

Several nonsteroidal antiinflammatory drugs which are cyclooxygenase inhibitors (e.g., salicylates, fenoprofen, ibuprofen) have been shown to suppress proteoglycan synthesis by normal joint cartilage in vitro. We examined the effect of benoxaprofen, a long-acting proprionic acid derivative which inhibits lipoxygenase in addition to causing moderate cyclooxygenase inhibition. When added to the culture medium in concentrations comparable with those obtainable in serum of patients treated with the drug (e.g., 10 and 50 micrograms/ml), benoxaprofen increased proteoglycan synthesis in slices of nor
