[Acute-placental dysfunction by villous-maturation defect and late-fetal mortality].

INTRODUCTION: The placental histological examination is essential in the assessment of stillbirths, especially those of third trimester, which are often of placental origin by chronic placental dysfunction and sometimes by acute dysfunction. The physiopathogenesis of the latter remains obscure. MATERIAL AND METHOD: Three stillbirths, one per partum and two after 38 and 39 weeks gestation, caused by acute-placental dysfunction by villous maturation defect were studied. The maternal aetiological investigations and the fetal autopsies were normal. Placentas were pale. Villi were numerous, normal

Prolonged pregnancy: evaluating gestation-specific risks of fetal and infant mortality.

OBJECTIVE: To evaluate gestation-specific risks of stillbirth, neonatal and post-neonatal mortality. DESIGN: Retrospective analysis of 171,527 notified births (1989-1991) and subsequent infant survival at one year, from community child health records. SETTING: Notifications from maternity units in the North East Thames Region, London. MAIN OUTCOME MEASURES: The incidence of births, stillbirths, neonatal and post-neonatal deaths at each gestation after 28 completed weeks. Mortality rates per 1000 total or live births and per 1000 ongoing pregnancies at each gestation were calculated. RESULT

Medical and life-style risk factors affecting fetal mortality, 1989-90.

OBJECTIVES: This report presents fetal mortality data by medical and life-style risk factors of the mother and the fetus. METHODS: Deaths and fetal mortality rates are presented in this descriptive report. Data sources used are vital statistics data for fetal deaths and live births. RESULTS: The data that became available with the revision of the U.S. Standard Report of Fetal Death in 1989 expanded the medical and health data available on mothers and fetuses. Reporting of medical conditions is probably incomplete for fetal deaths as well as for live births. Therefore, caution should be exerc

Relation between parvovirus B19 infection and fetal mortality and spontaneous abortion.

BACKGROUND: Infection with parvovirus B19 may cause fetal losses including spontaneous abortion, intrauterine fetal death and non-immune hydrops fetalis. The aim of this study is to determine the frequency of parvovirus B19 in formalin fixed placental tissues in lost fetuses using real-time PCR method. METHODS: In this cross-sectional study, 100 formalin fixed placental tissues with unknown cause of fetal death were determined using real-time PCR method after DNA extraction. RESULTS: Six out of 100 cases (6%) were positive for parvovirus B19 using real-time PCR. Gestational age of all positi

Severe fetal ventriculomegaly: Fetal morbidity and mortality, caesarean delivery rates and obstetrical challenges in a large prospective cohort.

INTRODUCTION: Severe fetal ventriculomegaly (VM) is defined as an enlargement of the atria of the lateral cerebral ventricles (Vp) of greater than 15 mm. While it is well established that it confers significant risk of morbidity and mortality to the neonate, there is limited information pertaining to the caesarean delivery rates and the obstetric management of these complex cases. The aim of this study was twofold: firstly, to determine survival rates in fetuses with severe VM, and secondly to determine the caesarean delivery rates in continuing pregnancies. We explore the obstetric challenges

Awareness of fetal movements and care package to reduce fetal mortality (AFFIRM): a stepped wedge, cluster-randomised trial.

BACKGROUND: 2·6 million pregnancies were estimated to have ended in stillbirth in 2015. The aim of the AFFIRM study was to test the hypothesis that introduction of a reduced fetal movement (RFM), care package for pregnant women and clinicians that increased women's awareness of the need for prompt reporting of RFM and that standardised management, including timely delivery, would alter the incidence of stillbirth. METHODS: This stepped wedge, cluster-randomised trial was done in the UK and Ireland. Participating maternity hospitals were grouped and randomised, using a computer-generated alloc

Fetal and infant mortality in Norway: A steady rate of decline over time.

BACKGROUND: Fetal and infant mortality rates have declined dramatically world-wide. On a scale of absolute risk, the declines are diminishing and perhaps even plateauing, raising concerns about public health progress. METHODS: We use Norway's national Medical Birth Registry to describe rates of declining stillbirth and infant mortality from 1967 to 2025. RESULTS: For nearly six decades, stillbirth mortality in Norway has declined at an annual rate of about 2.8%, and neonatal mortality has declined by 3.7% a year. Neither rate of decline shows signs of slowing. Postneonatal mortality (28 days

Impact of late embryonic and early fetal mortality on productivity of beef cows.

Although late embryonic/early fetal mortality (LEEFM) has been extensively researched in the context of reproductive efficiency, its long-term impact on cow productivity over a complete production cycle is poorly described in the scientific literature. This study used a prospective cohort design to evaluate the impact of late LEEFM on the productivity of beef cows. Postpartum cows (n = 2204) were exposed to a fixed-timed artificial insemination (FTAI) protocol followed by natural service breeding for the remainder of the breeding season. Pregnancy status was assessed 28 to 35 and 90 to 115 d a

Early-onset fetal growth restriction: A systematic review on mortality and morbidity.

INTRODUCTION: Severe early-onset fetal growth restriction is an obstetric condition with significant risks of perinatal mortality, major and minor neonatal morbidity, and long-term health sequelae. The prognosis of a fetus is influenced by the extent of prematurity and fetal weight. Clinical care is individually adjusted. In literature, survival rates vary and studies often only include live-born neonates with missing rates of antenatal death. This systematic review aims to summarize the literature on mortality and morbidity. MATERIAL AND METHODS: A broad literature search was conducted in OV

Spinal cord transections in fetal rats. Open intrauterine surgery with low mortality.

Open myelotomies were performed in rat fetuses in stages E15-E19 with a microsurgical technique. To prevent oligohydramnios, considered to be one of the main causes of death associated with fetal surgery, amniotic fluid loss was reduced as much as possible. With this technique a reduction of the mortality rate to 38% was reached in 75 consecutive fetuses.

Can promoting awareness of fetal movements and focusing interventions reduce fetal mortality? A stepped-wedge cluster randomised trial (AFFIRM).

BACKGROUND: In 2013, the stillbirth rate in the UK was 4.2 per 1000 live births, ranking 24th out of 49 high-income countries, with an annual rate of reduction of only 1.4% per year. The majority of stillbirths occur in normally formed infants, with (retrospective) evidence of placental insufficiency the most common clinical finding. Maternal perception of reduced fetal movements (RFM) is associated with placental insufficiency and increased risk of subsequent stillbirth. This study will test the hypothesis that the introduction of a package of care to increase women's awareness of the need for

[Fetal and neonatal mortality from 22 weeks of amenorrhea in the Loire area].

BACKGROUND: The Pays de Loire has a low perinatal mortality indicators among French regions but this could be due to under-notification. OBJECTS: To explore this hypothesis we undertook a survey in order to identify all fetal and neonatal deaths occurring at a gestionnal age of 22 weeks or more. We also tried to examine and analyze the causes of death. METHODS: All maternity (26) and neonatal wards (5) in the region took part in the survey in 1995. Clinicians were asked to fill out a questionnaire for all deaths occurring from gestational age (GA) 22 weeks and/or concerning a birthweight of

[Maternal and fetal morbidity and mortality in premature abruptio placentae--a retrospective analysis].

The frequency of abruptio placentae has nearly tripled in our clinic since 1974 and now stands at 1.4% of all deliveries. In a retrospective analysis it was investigated whether improved diagnostic and therapeutic methods have decreased the morbidity and mortality of this severe obstetrical complication. During 3 stages of observation--1974-1979, 1980-1985 and 1986-6/1990--we registered 123 cases. The division into time intervals was based on the introduction of sensitive ultrasonic methods in 1980 and the vaginal scan in 1986. Important risk factors of abruptio placentae were found to be blee
