Maternal understanding of fetal movement in third trimester: a means for fetal monitoring and reducing stillbirth.

BACKGROUND: Fetal movement, a sign of life, is widely considered as an indicator of fetal health status. Therefore, perceived alteration in regular fetal movement after the age of viability may signify impending adverse perinatal outcome. AIMS: This study aimed to determine maternal knowledge, behavior, and concerns about abnormal fetal movement in the third trimester of pregnancy. MATERIALS AND METHODS: A total of 225 women were surveyed using a self-administered questionnaire at the out-patient prenatal clinics of two tertiary health facilities in Nigeria between December 1, 2012 through J

Smartphone Detection of Fetal Movements Using Artificial Intelligence.

OBJECTIVE: To evaluate whether machine learning could be used with audio recordings from a smartphone to detect fetal movements that create disruptions of the amniotic fluid environment. METHODS: We conducted a prospective study to simultaneously record fetal movements seen on ultrasound and audio recordings using a smartphone placed on the maternal abdomen and to compare these with maternal perception of fetal movements. Smartphone audio segments were preprocessed to reduce noise, window the signal, and divide them into typed 1-second audio snippets. These were subsequently converted into vi

Developmental trajectories of general and breathing movements in fetal twins.

This study examined the developmental trajectories of general and breathing movements in fetal twins. Fetal movement patterns were assessed from real-time ultrasound recordings performed at 12-15, 20-23, and 28-32 weeks of gestation in 42 twin pairs. Results indicated that both general movements and breathing movements followed a curvilinear, inverted U-shaped curve. Developmental trajectories were unrelated within pairs of twins and were not associated with gestational age at birth and birth weight. However, sex differences were found for general movements with males displaying more time maki

Management and Outcome of Reduced Fetal Movements-is Ultrasound Necessary?

OBJECTIVE: To review the management and outcome of pregnancies of women presenting to obstetrical triage with decreased fetal movements (DFM). STUDY DESIGN: A retrospective review of women presenting with DFMs to two large Canadian obstetrical centres with a combined 9490 deliveries per year. The charts were reviewed for compliance with the Canadian guidelines for demographics (age, parity, GA, comorbidities, etc.), pregnancy management (admission vs. discharge, need to deliver), and pregnancy outcomes (mortality, morbidity, GA at delivery, Apgar scores, etc.). Patients who did not comply wit

Effect of beta-endorphin on fetal breathing movements in sheep.

It has been suggested that endogenous opioids, such as beta-endorphin (beta-EP), act to depress respiration in the fetus and newborn. We have investigated the effect of infusing beta-EP either intravenously or into a lateral cerebral ventricle on breathing movements and electrocortical activity in eight fetal lambs between 116 and 133 days gestation. Intravenous infusion of beta-EP (200 or 500 micrograms over 1 h) increased plasma beta-EP concentrations 2- to 230-fold and was associated with a small decrease in the percent time spent breathing, from 57.8 +/- 9.1 to 51.3 +/- 8.2%/h (n = 6 exp).

Fetal respiratory movements: only part of the biophysical profile.

In a period of 1.5 years, approximately 1000 women with high-risk pregnancy received sonographic examinations in the authors' laboratory. Of these patients, five women with postterm pregnancy were found to have oligohydramnios and no other reassuring body or limb movements, despite the finding in each case of regular, sustained fetal respiratory movements. Four of the five neonates had evidence of fetal compromise at birth and the fifth had intrauterine growth retardation but good Apgar scores. The recurrence of the problem emphasizes the need to consider fetal respiration as only one part of

An evaluation of the importance of formal, maternal fetal movement counting as a measure of fetal well-being.

Maternal perception of fetal movements has, over the years, become recognised as a valuable tool for early detection of fetal compromise. Several studies published in the mid-1970s have demonstrated that a reduction or cessation in maternally perceived fetal movements may precede antepartum late fetal death by a day or longer. From these findings formal fetal movement counting emerged as a valuable, non-invasive method of assessing fetal well-being; the belief being that clinical actions taken on the basis of reduced fetal movement counting may prevent antepartum death or morbidity. Conversely

Temporary brittle bone disease: association with decreased fetal movement and osteopenia.

Infants who present with multiple unexplained fractures pose a difficult diagnostic dilemma of child abuse versus intrinsic bone disease. Temporary brittle bone disease is a recently described disease characterized by a transient bone weakness in the first year of life which presents with multiple, unexplained fractures that can be confused with child abuse. The purpose of this study was to determine if there are common, historical features in infants with unexplained fractures that might suggest a basis for the fractures, and to determine if bone density measurements might indicate that such

Placental PHLDA2 expression is increased in cases of fetal growth restriction following reduced fetal movements.

BACKGROUND: Maternal perception of reduced fetal movements (RFM) is associated with increased risk of fetal growth restriction (FGR) and stillbirth, mediated by placental insufficiency. The maternally expressed imprinted gene PHLDA2 controls fetal growth, placental development and placental lactogen production in a mouse model. A number of studies have also demonstrated abnormally elevated placental PHLDA2 expression in human growth restricted pregnancies. This study examined whether PHLDA2 was aberrantly expressed in placentas of RFM pregnancies resulting in delivery of an FGR infant and expl

Ductus arteriosus flow velocity modulation by fetal breathing movements as a measure of fetal lung development.

A test is needed that would accurately predict favorable neonatal lung performance in the presence of prolonged severe oligohydramnios caused by ruptured membranes so that optimal obstetric care can be provided. We propose such a test that is based on the degree of modulation of fetal ductal blood flow velocity by fetal breathing movements after maternal glucose loading. In a prospective cross-sectional study of 49 normal pregnancies (50 fetuses) between 25 and 38 weeks an exponential increase in breathing-related ductal blood flow velocity modulation was observed with advancing gestational ag

Is buprenorphine use in pregnancy linked to decreased fetal breathing movement.

OBJECTIVE: Biophysical profile (BPP) testing of opioid-exposed fetuses in the third trimester is a commonly used assessment. Opioid use in pregnancy is associated with modest increased odds of stillbirth (OR 1.3). Our hypothesis was that BPP score results in buprenorphine patients might be different from a control group of high-risk patients. MATERIALS AND METHODS: This retrospective chart review analyzed the BPP assessments of patients receiving buprenorphine compared to patients followed for chronic hypertension. Patients seen between July 1, 2016, to June 30, 2020, were evaluated. Two hund

Fetal Movement Counting in Prolonged Pregnancies: The COMPTAMAF Prospective Randomized Trial.

In prolonged pregnancies, the risks of neonatal morbidity and mortality are increased. The aim of this trial was to assess the benefits of maternal information about fetal movement (FM) counting on neonatal outcomes in prolonged pregnancy. It was a prospective, single center, randomized, open-label study conducted from October 2019 to March 2022. Intention-to-treat analyses were performed on 278 patients randomized into two 1:1 groups (control group and FM counting group). The primary outcome was a composite score of neonatal morbidity (presence of two of the following items: fetal heart rate

Perinatal development of the rat hip joint with restrained fetal movement.

We compared the structures of the femoral head (FH) of neonates between normal and operated legs with restrained fetal movement using an exo utero technique. At embryonic day (E) 16.5, one hind limb was sutured onto the embryonic membrane and the fetuses were allowed to develop exo utero until the term (E22.5). There was no significant difference in the largest diameter of the FH between the non-operated and operated side FH in the operated neonates and the FH of the non-operated neonates. By scanning electron microscopy, roughness and collagen fiber bundles, which were detected on the surface

Maternal response to daily fetal movement counting in primary care settings.

Maternal counting of fetal movement (FM) to assess fetal well-being was studied in 394 pregnancies followed by family physicians. Counting was well accepted, with 85% of women finding it reassuring and 91% wanting to include it in subsequent pregnancies. Eighty-eight percent of women reported counting five or more days per week. Thirteen women reported decreased activity a total of 20 times. A nonstress test (NST) was performed 20 times and an oxytocin challenge test (OCT) was performed four times as a result of these reports. One of the 20 reports of decreased activity was followed by a nonre

Fetal movements are related to maternal characteristics: a prospective ultrasound study.

BACKGROUND: Fetal movements are important indicators of well-being that are associated with stillbirth. Yet neither a universal definition nor gold standard exists for what constitutes normal or abnormal. Current monitoring methods such as maternal perception lack diagnostic accuracy, while ultrasound provides detailed assessment in real time that is not practical outside the clinic setting. Maternal perception remains the primary method of fetal movements detection in clinical practice. OBJECTIVES: To assess normal fetal movements patterns using real-time ultrasound and maternal perception a

From abnormal fetal movements to neonatal seizures: A literature review.

Fetal seizures (FS) are underestimated and underdiagnosed events with a poor prognosis that could precede neonatal seizures. FS diagnosis could be clinical or by means of ultrasound. FS causes are heterogenic. After a PubMed, Google Scholar and SCOPUS research, aimed to find publications concerning FS, we selected 23 studies, mainly case reports with literature reviews, for a total of 29 patients. We evaluated the method of diagnosis, gestational age at diagnosis of FS, FS etiology, time of etiological diagnosis, fetal/neonatal outcome and possible therapeutic options. Fetal seizures were main

Predictive factors in pregnancies with reduced fetal movements: a pilot study.

BACKGROUND: Pregnancies with reduced fetal movements (RFM) are at risk for poor neonatal outcomes and stillbirth. AIM: To investigate whether Doppler measurements or angiogenic factors are good predictors of adverse neonatal outcomes in pregnancies with RFM. METHODS: This is a prospective pilot cohort study of 3243 women seeking care for RFM. Standard care was carried out in all cases. An extra Doppler examination was performed in 128 women to assess the flow in the middle cerebral artery, the umbilical artery, and the uterine artery. The outcomes of pregnancies with reduced fetal movements: A retrospective cohort study.

INTRODUCTION: The objective of this study was to examine the outcomes and interventions in pregnant women presenting with a perception of reduced fetal movements (RFM), and to determine if repeated episodes of RFM increase the risk of adverse outcomes. MATERIAL AND METHODS: This was a retrospective cohort study conducted in six NHS hospitals within the Thames Valley network region, UK and one neighboring hospital, an area with approximately 31 000 births annually. All women with a primary presentation of perceived RFM after 24 completed weeks of gestation during the month of October 2016 were
