BACKGROUND: Enteric nerve cells begin to mature during the last trimester of pregnancy and become mature only after birth. The degree of maturation seems to be related to bowel motility.
CASE REPORT: A girl was born from a pregnancy complicated by hydramnios, and did not pass meconium before the 64th hr of life. Barium enema on day 4 showed a left microcolon with no distension of the transverse colon. Tests for cystic fibrosis were negative. On day 12, the patient presented with septicemia due to Pseudomonas maltophylia. Parenteral alimentation by central catheter was instituted. Surgical rec

In 18 cases of serious and fairly serious toxemic pregnancy, the authors gave 500 micrograms L-Thyroxin "Henning" intraamniotically after the laboratory evaluation demonstrated negative pulmonary maturity from an amniotic fluid sample. Maternal and fetal complications due to the drug could not be observed. Despite a 39% premature incidence, IRDS and hyaline membrane disease were not observed. The administered T4 caused a positive change in the direction of the L/S ratio and the Clements test. The other amniotic fluid parameters did not change, except the T4 level. The extremely high T4 level o

Delaying delivery of a premature abdominal pregnancy may allow for fetal pulmonary maturation. Prolonged extreme oligohydramnios, however, resulting in significant pulmonary hypoplasia would frustrate any effort to wait for pulmonary maturation. Little mention of this complication is found in the literature. Therefore, two cases of abdominal pregnancy are presented, one with fetal pulmonary hypoplasia and one without. Assessing the volume of amniotic fluid is suggested when a decision is being made to deliver this type of pregnancy.

Antenatal steroid (ANS) therapy is the standard care for women at imminent risk of preterm labor. Despite extensive and long-standing use, 40%-50% of babies exposed antenatally to steroids do not derive benefit; remaining undelivered 7 days or more after ANS treatment is associated with a lack of treatment benefit and increased risk of harm. We used a pregnant sheep model to evaluate the impact of continuous versus pulsed ANS treatments on fetal lung maturation at an extended, 8-day treatment to delivery interval. Continuous low-dose ANS treatments for more than 72 h in duration improved fetal

INTRODUCTION: Tracheal occlusion (TO) is an investigational therapy for severe congenital diaphragmatic hernia that decreases pulmonary hypoplasia, but sustained TO also induces deficient surfactant synthesis. Intramuscular maternal administration of retinoic acid (RA) in a surgical rabbit model of congenital diaphragmatic hernia showed a beneficial effect on lung maturation. We evaluated the potential of RA delivery into the trachea and studied the combined effects of TO and RA on normal lung development.
METHODS: Experiments were performed on normal rabbit fetuses. Liposomes and capric trig

OBJECTIVE: To determine if the right and the left testes migrate at the same time during the human fetal period.
SUBJECTS AND METHODS: We studied 164 human fetuses (328 testes) ranging in age from 12 to 35 weeks post-conception. The fetuses were carefully dissected with the aid of a stereoscopic lens at ×16/25. The abdomen and pelvis were opened to identify and expose the urogenital organs. Testicular position was classified as: (a) Abdominal, when the testis was proximal to the internal ring; (b) Inguinal, when it was found between the internal and external inguinal rings); and (c) Scrotal,

Recent evidence indicates that intra-amniotic endotoxin (LPS) and interleukin-1 alpha (IL-1alpha) accelerate foetal lung maturity and protect from respiratory distress syndrome (RDS) more effectively than does antenatal glucocorticoid. Inflammatory cytokines promote development of chronic lung disease in the premature, acute RDS (ARDS) in children and adults. Systemic exposure to LPS or cytokines can result in generalized multiorgan damage. The abnormal host defence in the foetus and the premature newborn need to be considered in therapeutic interventions.

A case is reported of fetal survival after maternal listeremia. Transplacental fetal infection may have been prevented by early delivery. Review of the literature suggests that management be based on cultures of the maternal and fetal compartments. We believe that the considerations for immediate delivery as opposed to medical therapy should rest on the fetal maturity and the sensitivity of the organism to antibiotics which will reach therapeutic levels in the fetal compartment.

Many extremely low birth weight (LBW) neonates now survive with intensive care. Their survival depends on fetal and neonatal adaptations of multiple organ systems, which represents a plasticity of development. Many extremely LBW neonates do not have severe lung immaturity, will breathe, and do not require surfactant treatment. The two clinically relevant modulators of this early lung maturation are antenatal corticosteroid treatments and fetal exposure to inflammation. Those same frequent fetal exposures also can mature the fetal innate immune system to become more proinflammatory. However, re

Dexamethasone has been widely used in clinical practice to promote fetal lung maturity and reduce neonatal respiratory distress syndrome and perinatal mortality. Nevertheless, its administration is a double-edged sword, as a large number of studies have shown that there are obvious disadvantages in pregnant women and fetal development. In this review, we comprehensively retrospect the latest literature on the toxicological effects and mechanisms of dexamethasone on fetal development, in an attempt to provide a valuable basis for further studies and clinical trials in the future. Overall, prena

The substantial morbidity and mortality of congenital diaphragmatic hernia (CDH) is attributed to the pulmonary hypoplasia caused by the presence of abdominal viscera in the chest during intrauterine life. Recent experimental studies suggest that surfactant deficiency may also contribute to CDH pathophysiology. Clinically, the amniocentesis-derived lecithin to sphingomyelin (L/S) ratio and phosphatidylglycerol (PG) data are used to assess fetal lung maturity. We have performed amniotic fluid phospholipid analyses at 33 to 38 weeks' gestation in 18 fetuses with prenatally diagnosed CDH to asses

Lung weight, DNA, RNA, protein, and total body weight were analyzed in fetuses from 14 pregnant Sprague-Dawley rats fed a nutritionally complete liquid diet containing v/v 6% ethanol (ethanol group). Each ethanol rat was matched with an isocalorically pair-fed animal (control group) who received the same liquid diet with carbohydrate substituted for ethanol. The rats were killed on day 20 of gestation. The mean maternal blood ethanol concentration at 0700 h on day 20 was 170 +/- 22 (SE) mg/dl. Compared to controls, ethanol fetuses had reduced body weight (21%), lung dry weight (39%), lung wet

Congenital diaphragmatic hernia (CDH) is a developmental defect of the diaphragm with herniation of abdominal viscera into the thorax. The morbidity and mortality in cases is caused primarily by pulmonary hypoplasia. A prenatal diagnosis of CDH can be established by ultrasound, but ultrasound cannot detect pulmonary hypoplasia with certainty. We evaluated pulmonary hypoplasia by the simple method of lung intensity. The subjects were eight fetuses. In four fetuses, the lungs showed low intensity, and these fetuses all had pulmonary hypoplasia. Two of the four fetuses with high intensity showed

PURPOSE: Prenatal glucocorticoids reverse pulmonary immaturity in rodents with pharmacologically induced congenital diaphragmatic hernia (CDH). The authors applied quantitative stereologic morphometric techniques to test whether these effects could be reproduced in large animals (sheep) with surgically created CDH.
METHODS: Diaphragmatic hernias were created surgically in fetal lambs at gestational day 80. The fetuses were treated with intravenous cortisol (n = 6) or normal saline control (n = 5) from days 133 to 135. Lungs distended at 15 cm pressure from each group were harvested at day 136

Following maternal treatments with 50 mg/kg/day ambroxol for 2 or 3 days before delivery at 28 days gestation, preterm rabbits were ventilated to evaluate lung function. Subsequently, surfactant saturated phosphatidylcholine (SatPC) pool sizes were measured. One half of the ambroxol treated and control rabbits were given surfactant at delivery. Although surfactant improved lung function comparably for control and ambroxol treated rabbits, ambroxol treatments did not change ventilatory pressure requirements, compliances, or the recovery of intravascular labeled albumin in the lungs. The effects of nicotine administration on the synthesis and secretion of surfactant in the rabbit fetal lung were studied by measuring phosphatidylcholine (PC) and phosphatidylglycerol (PG) in the fetal lung tissue and amniotic fluid. The concentrations of both PC and PG in the lung tissue were decreased by administering nicotine to the mother. On the other hand, the concentrations of PC and PG in the amnniotic fluid were not altered by nicotine administration. Moreover, the PG/PC ratio in the amniotic fluid of the nicotine group was similar to that of the control group. The plasma catecholami

We have reviewed 49 pregnancies with ultrasound evidence of fetal urinary tract anomalies and/or oligohydramnios. A surprisingly large number of fetuses with urinary tract obstruction had significant other anomalies. Fetuses with urinary tract obstruction, but a normal amount of amniotic fluid, did well without in utero intervention; however, those with oligohydramnios or polyhydramnios fared poorly. Longer periods of oligohydramnios in fetuses with urinary tract obstruction seemed to be associated with pulmonary hypoplasia and renal dysplasia. Oligohydramnios without urinary tract obstruction
