Modeling a hydropic recipient twin in twin-twin transfusion syndrome.

We developed a mathematical model of twin-twin transfusion syndrome (TTTS) that includes a hydropic recipient twin, adding interstitial and intracellular fluid compartments, fetal congestive cardiac failure, and the dynamics of renin-angiotensin system (RAS) mediators to our previous TTTS model. Ten differential equations for each twin, coupled by the net fetofetal transfusion of blood and blood components, i.e., colloids, osmoles, and RAS mediators, describe the development of fetal arterial and venous blood volumes, blood osmolality and colloid osmotic pressure (COP), interstitial fluid volu

Therapy by laser equatorial placental dichorionization for early-onset spontaneous twin anemia-polycythemia sequence.

We report a case of twin anemia-polycythemia sequence (TAPS) treated by fetoscopic laser equatorial placental dichorionization, also known as the 'Solomon technique', at 24 weeks of gestation. TAPS was present despite the absence of fetoscopically visualized chorionic anastomoses from the donor to the recipient twin. The goal of this procedure was to prevent post-laser TAPS in cases of twin-twin transfusion syndrome. The surgery and subsequent intrauterine blood transfusion to the donor twin could result in the survival of both twins without hematologic or neurological complications. Following

Placental characteristics in twin transfusion syndrome.

The authors describe the histological features of the placenta in a typical case of twin-to-twin transfusion, with particular attention focused on the stereologic aspects. Villi from the anemic placental portion were slightly edematous, with small and inconspicuous vessels. The stereologic characteristics of this portion were nearer values of normal placentas. The plethoric region appears as a postmature organ, with a very thin trophoblast layer and numerous vasculo-syncytial membranes. The most dramatic alterations of this region were mainly related to the foetal capillary. According to the s

[In utero treatment of twin-to-twin transfusion syndrome].

Twin-to-twin transfusion syndrome (TTTS) is a serious complication of monochorionic twin pregnancies in which an imbalance in blood flow between the twins is caused by vascular anastomoses in the placenta. TTTS affects 10 to 15% of these monochorionic pregnancies and can lead to high perinatal mortality if left untreated. The Quintero classification, introduced in 1999, helps stratify the severity of TTTS into five stages, thereby facilitating clinical decision-making. Laser photocoagulation of placental anastomoses, a technique developped in the 2000s, has become the standard of care for seve

[Twin Reversed Arterial Perfusion Sequence (TRAP) : a circulatory paradox causing complications in monochorial twin pregnancies].

The TRAP sequence («Twin Reversed Arterial Perfusion Sequence») is a rare condition found in approximately 1 to 3 % of monochorionic twin pregnancies. It results from the existence of placental vascular anastomoses (arterio-arterial and veno-venous) between a normal twin and an acardiac twin (or with a dysfunctional heart). The normal twin then serves as a «pump» for the abnormal twin who is retrogradely perfused. Throughout pregnancy, the well-being and survival of the pump twin is compromised by different mechanisms (risk of high-output heart failure, premature delivery, premature rupture of

Placental vessel segmentation and registration in fetoscopy: Literature review and MICCAI FetReg2021 challenge findings.

Fetoscopy laser photocoagulation is a widely adopted procedure for treating Twin-to-Twin Transfusion Syndrome (TTTS). The procedure involves photocoagulation pathological anastomoses to restore a physiological blood exchange among twins. The procedure is particularly challenging, from the surgeon's side, due to the limited field of view, poor manoeuvrability of the fetoscope, poor visibility due to amniotic fluid turbidity, and variability in illumination. These challenges may lead to increased surgery time and incomplete ablation of pathological anastomoses, resulting in persistent TTTS. Comp

Intrauterine Interventions for the Treatment of Twin Anemia-Polycythemia Sequence: A Systematic Review.

BACKGROUND: Twin anemia-polycythemia sequence (TAPS) is a complication of monochorionic, multiple gestation pregnancies in which blood shunting through placental anastomoses results in chronic anemia in one fetus and chronic polycythemia in another. The outcomes of different treatment modalities for TAPS are not well known. OBJECTIVE: To determine the outcomes of the intrauterine interventions used to treat TAPS. STUDY DESIGN: A systematic literature search of MEDLINE, EMBASE, and CENTRAL was performed in June 2016. Primary outcomes were mortality, morbidity, and adverse perinatal outcomes.

Lessons from monochorionic twin delivery.

The presence of acute peripartum anaemia in a monochorionic twin pregnancy represents a clinical challenge requiring prompt recognition and management. Twin-to-twin transfusion syndrome (TTTS) is a major complication of these pregnancies and a medical emergency in its acute form. Acute intrapartum fetoplacental transfusion (AIFT) has been reported infrequently. The authors present a case of a probable acute TTTS in an uneventful monochorionic monoamnionic twin pregnancy, where typical ultrasound criteria for long-standing TTTS were absent. The first twin was born pale, hypotonic and developed

[Discordant nuchal translucency in monochorionic twin pregnancy. A case report].

The twin-to-twin transfusion syndrome is a serious anomaly occurring from vessels anastomosis between the fetal-planetary circulations of monochorionic twin pregnancies with a high perinatal morbidity and mortality. In the first trimester, the presence of discordant increase of the nuchal translucency associated a reverse blood flow in the ductus venosus are marks of twin-to-twin transfusion syndrome. In this report, we related a case of monochorionic twin pregnancy in the first trimester in that the first diagnostic signal this syndrome was an increase of the measurement of nuchal translucenc

Large-volume therapeutic amniocentesis in the treatment of hydramnios.

OBJECTIVE: To evaluate the safety and outcome of large-volume therapeutic amniocentesis in the treatment of hydramnios. METHODS: Therapeutic amniocentesis was defined as an attempt to remove enough amniotic fluid (AF) in pregnancies complicated by symptomatic hydramnios to leave a normal volume of AF (AF index less than 25 cm). This report includes all therapeutic amniocenteses that were performed at Good Samaritan Regional Medical Center in Phoenix, Arizona, from 1988-1993. RESULTS: Ninety-four patients had 200 therapeutic amniocenteses. The most common condition treated with therapeutic am

Clinical characteristics of monochorionic twins with large hemoglobin level discordance at birth.

OBJECTIVES: To evaluate neonatal outcomes and clinical characteristics of monochorionic diamniotic (MCDA) twins with a large intertwin hemoglobin (Hb) difference at birth. METHODS: This was a retrospective cohort study of MCDA twin gestations delivered at Osaka Medical Center and Research Institute for Maternal and Child Health between 2003 and 2012. Cases of pregnancy termination, acardiac twins or intrauterine death were excluded. A large intertwin Hb difference at birth was defined as > 8.0 g/dL according to the postnatal criteria for twin anemia-polycythemia sequence (TAPS). The intertwin

Randomized controlled trial of twin-twin transfusion syndrome laser surgery: the sequential trial.

BACKGROUND: Intraoperative blood transfer between twins during laser surgery for twin-twin transfusion syndrome can vary by surgical technique and has been proposed to explain differences in donor twin survival. OBJECTIVE: This trial compared donor twin survival with 2 laser techniques: the sequential technique, in which the arteriovenous communications from the volume-depleted donor to the volume-overloaded recipient are laser-occluded before those from recipient to donor, and the selective technique, in which the occlusion of the vascular communications is performed in no particular order.

Learning-based keypoint registration for fetoscopic mosaicking.

PURPOSE: In twin-to-twin transfusion syndrome (TTTS), abnormal vascular anastomoses in the monochorionic placenta can produce uneven blood flow between the two fetuses. In the current practice, TTTS is treated surgically by closing abnormal anastomoses using laser ablation. This surgery is minimally invasive and relies on fetoscopy. Limited field of view makes anastomosis identification a challenging task for the surgeon. METHODS: To tackle this challenge, we propose a learning-based framework for in vivo fetoscopy frame registration for field-of-view expansion. The novelties of this framewor

Skeletal Growth Arrest Lines in Fetal Remains: Histopathology and Correlative Placental Pathology.

INTRODUCTION: Skeletal growth arrest lines (GAL) are transverse lines of metaphyseal radiodensity accompanying episodic severe physiological stress. They are poorly described in fetal remains. MATERIALS AND METHODS: We searched our autopsy practice for instances of fetal GAL in post mortem radiology, and correlated them with long bone histology and placental pathology. We describe the appearance, distribution, and pathology of GAL in a cohort of fetal autopsies, and compare the placental pathology accompanying GAL to the placental pathology of asymmetrical growth restriction (AGR) in the same
