Recurrent unipolar mania in a psychiatric hospital setting in the Fiji Islands.

This study aimed to determine the rate of unipolar mania and compare its characteristics with those of other bipolar affective disorders in a psychiatric hospital in the Fiji Islands. Fifty-one patients with unipolar mania seen between January 1999 and October 2000, had their diagnosis confirmed using the Schedules for Clinical Assessment in Neuropsychiatry and the International Classification of Diseases, 10th edition. Their demographic and clinical characteristics were compared with those of 31 manic-depressive patients seen during the period under review. Unipolar mania constituted 47.2% of

The prevalence and nature of anemia among apparently normal subjects in Fiji.

A total of 4939 apparently healthy Fijian and Indian subjects living in Fiji were tested for anaemia by determination of the microhaematocrit of a sample of capillary blood. The prevalence of anaemia during childhood varied with age but was similar for Fijians and Indians of either sex. The overall prevalences were: 0-4 years, 20.3%; 5-9 years, 3.7%; 10-14 years, 23.5%. The prevalence of anaemia among Indian women (33.3%) was substantially higher than that for Indian men (6.9%), Fijian women (8.1%) or Fijian men (6.8%). Iron deficiency was the most common cause of anaemia and was established b

Hospital morbidity in the Fiji islands with special reference to the saccharine disease.

The concept of the excessive consumption of carbohydrates as a cause of many diseases of civilisation has previously been proposed under the name of the 'saccharine disease'. A review of the hospital morbidity figures for these diseases in a divisional hospital in the Fiji Islands is presented. The hospital serves a population comprised of Indians and Fijians, suggesting comparison with the province of Natal, South Africa. Indians have a higher incidence of diabetes melitus, myocardial infarction, duodenal ulcer, acute appendicitis, gallstones, renal stones and eclampsia. Their diets differ ma

National control programs for scabies: Experiences from Fiji and Solomon Islands.

INTRODUCTION: The most recent World Health Organization roadmap for neglected tropical diseases sets a target for countries to control scabies through several interventions, including mass drug administration in endemic areas where prevalence is 10% or greater using oral ivermectin and topical scabicides. This report documents the experiences and lessons learned from the first two countries in the world, Fiji and Solomon Islands, to implement national ivermectin-based mass drug administration for scabies. By identifying key challenges, this article aims to inform the global community as effort

Dietary patterns associated with anaemia among pregnant women in Suva, Fiji.

BACKGROUND: Anaemia is prevalent during pregnancy in Fiji. In many countries, inadequate nutrient intake is associated with anaemia during pregnancy. We aimed to describe the relationship between dietary patterns and anaemia amongst pregnant women in Fiji. METHODOLOGY: A hospital-based, age-matched case-control study was conducted in the Antenatal Clinic at the main hospital in Suva, Fiji. Anaemia was defined as a haemoglobin concentration of less than 11.0 g/dL in trimesters 1 and 3, and less than 10.5 g/dL in trimester 2. Dietary variables were assessed using a semi-quantitative food freque

Interactions between timing and transmissibility explain diverse flavivirus dynamics in Fiji.

Zika virus (ZIKV) has caused large, brief outbreaks in isolated populations, however ZIKV can also persist at low levels over multiple years. The reasons for these diverse transmission dynamics remain poorly understood. In Fiji, which has experienced multiple large single-season dengue epidemics, there was evidence of multi-year transmission of ZIKV between 2013 and 2017. To identify factors that could explain these differences in dynamics between closely related mosquito-borne flaviviruses, we jointly fit a transmission dynamic model to surveillance, serological and molecular data. We estimat

Mosquito-borne infections in Fiji. V. The 1971-73 dengue epidemic.

A dengue epidemic due to type 2 virus involving some 3,400 cases began in Fiji early in 1971, had a peak during May, June and July, and cases have continued to occur with a low incidence during 1972 and 1973. Many of the notified cases showed classical dengue fever symptoms and there were no confirmed cases of haemorrhagic fever. A serological survey indicated that there had been at least 20,000 subclinical infections. It is probable that the virus was introduced to Fiji either through the port of Lautoka or Nadi international airport in February 1971. The normal travel patterns of residents m

SAMJ: fast image annotation on ImageJ/Fiji via segment anything model.

Accurate image annotation is essential for training artificial intelligence (AI) systems in biomedical image analysis, enabling tasks such as cell detection, tissue quantification, and disease characterization. However, creating pixel-level annotations is a time-consuming and labor-intensive process that requires expert input, limiting the development and adoption of AI methods. Recent advances in foundation models, such as the Segment Anything Model (SAM), enable interactive object segmentation from simple user prompts, but their integration into widely used bioimage analysis platforms remain

Meningococcal C Disease Outbreak Caused by Multidrug-Resistant Neisseria meningitidis, Fiji.

We describe an outbreak of invasive meningococcal disease (IMD) caused by Neisseria meningitidis serogroup C in Fiji. We created surveillance case definitions and collected data by using standard investigation forms. Bacterial identification, antimicrobial susceptibility testing, and PCR were performed in Fiji. Molecular testing was conducted at the Microbiological Diagnostic Unit in Melbourne, Victoria, Australia. During January 2016-December 2018, a total of 96 confirmed or probable IMD cases were reported. Of case-patients, 61.5% (59/96) were male and 38.5% (37) female, 84.4% (81) were indi

A Large Leptospirosis Outbreak following Successive Severe Floods in Fiji, 2012.

Severe flooding has been linked to outbreaks of leptospirosis. Two sequential extreme flood events in Western Fiji caused the largest outbreak of leptospirosis recorded in the South Pacific, with 1,217 total suspected cases, of which 314 were probable and confirmed. Most (83%) cases occurred within 6 weeks of the flood events, displaying a biphasic epidemic curve associated with the floods. Given the temporal proximity of cases to flooding events, most of the transmission appeared to occur during or immediately after the floods; therefore, prevention of exposure to contaminated environments is

Dengue type 1 epidemic with haemorrhagic manifestations in Fiji, 1989-90.

A dengue type 1 epidemic occurred in Fiji between July 1989 and July 1990. Virus isolation in C6/36 cell cultures and Toxorhynchites mosquitos yielded 36 strains. Of the 3686 cases recorded by the Ministry of Health, 60% involved indigenous Fijians and 37%, Indians. A house-to-house survey revealed that a large majority of patients had classical dengue symptoms and 8% reported haemorrhagic manifestations. Among the children and adults hospitalized for dengue, 43% had haemorrhagic manifestations, including epistaxis, gingival bleeding, haematemesis, melaena and haematuria. A total of 15 patient

A seroepidemiological study of hepatitis B amongst Fiji health care workers.

Hepatitis B immunization for health care workers is common policy in many countries where they constitute a particular at risk group. A seroepidemiological study of hepatitis B virus (HBV) in Fiji health care workers was conducted to determine whether this occupational group (or subgroups thereof) were at higher risk of infection than the general Fiji population. The purpose of this study was to ascertain whether health staff should be immunized, or whether it would be more productive to focus resources on neonatal immunization. Blood samples were obtained from 2,639 health workers and the ser

Oral melanosis: a case report of Peutz-Jegher's Syndrome in Suva, Fiji.

This paper reports on a case of oral melanosis resultng from Peutz-Jeghers Syndrome, a hereditary disease characterized by harmatomatous polyposis and by mucocutaneous melanic pigmentation, in a 14-year-old Fijian girl with a family history of the disease. The patient underwent surgery for treatment of an intestinal obstruction due to a small intestine intussusception. Recognition of the characteristic pigmentation by dentists may lead to early diagnosis of associated harmatomatous polyps or neoplastic disease that may be life threatening.

Prevalence-incidence bias in longitudinal studies of rheumatic heart disease in Fiji.

BACKGROUND: Rheumatic heart disease (RHD), a consequence of a dysregulated immune response to Streptococcus pyogenes infection, remains the most commonly acquired cardiovascular condition in under 25-year-olds. Predominantly occurring in low- and middle-income countries (LMICs), RHD has an estimated prevalence of 40.5 million cases globally, although RHD complication rates in endemic settings remain uncertain. Accordingly, we undertook a prospective cohort study of RHD mortality and morbidity in Fiji with the aim of comparing complication rates to those estimated in our previous retrospective

Genomic epidemiology of Salmonella Typhi in Central Division, Fiji, 2012 to 2016.

BACKGROUND: Typhoid fever is endemic in some Pacific Island Countries including Fiji and Samoa yet genomic surveillance is not routine in such settings. Previous studies suggested imports of the global H58 clade of Salmonella enterica var Typhi (Salmonella Typhi) contribute to disease in these countries which, given the MDR potential of H58, does not auger well for treatment. The objective of the study was to define the genomic epidemiology of Salmonella Typhi in Fiji. METHODS: Genomic sequencing approaches were implemented to study the distribution of 255 Salmonella Typhi isolates from the C

A novel genetic variant of Streptococcus pneumoniae serotype 11A discovered in Fiji.

OBJECTIVES: As part of annual cross-sectional Streptococcus pneumoniae carriage surveys in Fiji (2012-2015), we detected pneumococci in over 100 nasopharyngeal swabs that serotyped as '11F-like' by microarray. We examined the genetic basis of this divergence in the 11F-like capsular polysaccharide (cps) locus compared to the reference 11F cps sequence. The impact of this diversity on capsule phenotype, and serotype results using genetic and serologic methods were determined. METHODS: Genomic DNA from representative 11F-like S. pneumoniae isolates obtained from the nasopharynx of Fijian childr

High prevalence of rheumatic heart disease in Fiji detected by echocardiography screening.

AIM: Rheumatic heart disease poses a huge burden for developing countries, with Pacific Island nations having among the highest prevalence reported in the world. It is recognised that echocardiography is much more sensitive than clinical examination for detection of rheumatic heart disease, but resource and cost limitations are delaying implementation of screening programmes in developing nations. Rapid echocardiography using low-cost portable machines and a non-expert operator may be a useful compromise, allowing widespread screening and control of rheumatic heart disease in developing countr
