Management of Finger Joint Dislocation and Fracture-Dislocations in Athletes.

Although finger joint dislocations are generally thought of as benign by many athletes and assumed to be a sprain, these injuries represent a spectrum that includes disabling fracture-dislocations. Failure to recognize certain dislocations or fracture-dislocations may result in permanent deformity and loss of motion. Simple dislocations are frequently amenable to early return to play with protection; however, more complex injuries may require specialized splinting or surgery. Delay in diagnosis of unstable proximal interphalangeal fracture-dislocations may require reconstruction or fusion. Ear

Interrater reliability in finger joint goniometer measurement in Dupuytren's disease.

We investigated interrater reliability of motion (ROM) measurement in the finger joints of people with Dupuytren's disease. Eight raters measured flexion and extension of the three finger joints in one affected finger of each of 13 people with different levels of severity of Dupuytren's disease, giving 104 measures of joints and motions. Reliability measures, represented by intraclass correlation coefficient (ICC), standard error of the mean (SEM), and differences between raters with the highest and lowest mean scores, were calculated. ICCs ranged from .832 to .973 depending on joint and motio

Vascularized Origami Medial Femoral Condyle Flap for Finger Joint Reconstruction.

Total finger joint reconstruction is challenging. Vascularized toe joint transfer is currently used for reconstruction, but it is difficult to perform, fails to achieve maximal joint flexibility, and is associated with donor-site complications. As an alternative, the authors developed a vascularized medial femoral condyle flap technique, wherein the vascularized corticoperiosteum is shaped via origami, with the donor tissue folded to fit the recipient site. In this article, the authors describe the use of this method for reconstruction of interphalangeal and metacarpophalangeal joints with a r

Design of a Soft Composite Finger with Adjustable Joint Stiffness.

This article presents the design of a soft composite finger with tunable joint stiffness. The composite finger, made of two different types of silicone, uses hybrid actuation by combining tendon and pneumatic actuation schemes. Tendons control the finger shape in a prescribed direction to demonstrate discrete bending behavior due to different material moduli, similar to that of a human's finger. The pneumatic actuation changes the stiffness of joints using air chambers. The feasibility of adjustable stiffness joints is proven using both the parallel spring model and experiments that demonstrat

Enhancement of finger motion range with compliant anthropomorphic joint design.

Robotic researchers have been greatly inspired by the human hand in the search to design and build adaptive robotic hands. Especially, joints have received a lot of attention upon their role in maintaining the passive compliance that gives the fingers flexibility and extendible motion ranges. Passive compliance, which is the tendency to be employed in motion under the influence of an external force, is the result of the stiffness and the geometrical constraints of the joints that define the direction of the motion. Based on its building elements, human finger joints have multi-directional pass

Short-term outcome of finger joint synovectomy in rheumatoid arthritis.

Rheumatoid arthritis (RA) frequently affects finger joints, and persistent synovitis is believed to cause not only bone destruction but also various deformities of the fingers in the long run. Synovectomy of the finger joints is carried out when chronic swelling of the synovium does not respond to any conservative treatment with medication and rehabilitation. In the present study the short-term results of finger joint synovectomy in RA were reviewed in 49 finger joints. The subjects were evaluated at two time points, with average follow-up periods of 14 and 62 months, and the results were comp

Silicone lymphadenopathy: a complication of silicone elastomer finger joint prostheses.

Two cases of lymphadenopathy resulting from a lymph node foreign body reaction to silicone are presented. In each case lymph node biopsy was performed because of the clinical suspicion of neoplastic disease. Histologic study in both cases revealed no evidence of malignant disease, but rather demonstrated multinucleated giant cells in reaction to irregular fragments of refractile foreign material. The foreign substance was consistent in appearance with silicone. In each case a history of long standing rheumatoid arthritis and previous joint replacement with implantation of silicone elastomer pr

Finger joint swelling: correlation with age, gender, and manual labor.

A soft tissue immersion radiography technique was used to study changes in 4,648 finger joints of 166 patients free from signs of inflammatory joint disease. An age-specific correlation was found for joint swelling, joint space narrowing, joint margin spurs, and intraarticular loose bodies. The distal interphalangeal joints were more commonly swollen in women, and the proximal interphalangeal and metacarpophalangeal joints more commonly swollen in manual laborers. Both correlations are highly significant (P less than 0.001). The second and third digits showed a definite predilection for joint

Does distal interphalangeal joint arthrodesis affect proximal interphalangeal joint arthroplasty outcomes in the same finger?

The purpose of this study was to analyse the 1-year outcomes after combining a surface replacing proximal interphalangeal joint arthroplasty and a distal interphalangeal screw arthrodesis and to compare the combined surgery with proximal interphalangeal joint arthroplasty alone. To obtain two groups with similar baseline data from our prospective registry, propensity score matching was used to match 23 fingers with the combined operations with 115 fingers with proximal interphalangeal joint arthroplasty alone. One year after surgery, the mean ranges of motion were 60° (95% CI: 53° to 67°) in t

Motion Analysis of the Wrist and Finger Joints in Sport Climbing.

Climbing is a fast-growing sport, with one of the most common injuries being a rupture of the finger flexor tendon pulley. The strain on pulleys increases as finger joints flex. However, to our knowledge, no study has conducted a kinematic analysis of climbers' fingers. Thus, this study aimed to examine finger kinematics during typical climbing tasks. Eleven elite climbers performed a sequence of four climbing moves, which were recorded by an optical motion capture system. Participants used crimp, half-crimp, and open-hand grips for three trials each, with the fourth condition involving campus

Design modifications and evaluation of the silicone artificial finger joints.

BACKGROUND: There are many reasons that could lead to finger joint arthroplasty, and the most familiar reason is osteoarthritis. Silicone finger joint are the most commonly used implants. However, these implants might fracture with time and cause wear which will lead to chronic inflammation and synovitis for the patient and then implant failure. OBJECTIVE: The aim of this study is to improve the design of the silicone finger joint and simulate the different designs using finite element analysis (FEA) simulation. METHOD: Three different designs were drawn and FEA has been used in this study u

Simultaneous dislocation of both interphalangeal joints in the middle finger.

Simultaneous dorsal dislocation of both interphalangeal joints (IPJs) in one finger is an uncommon injury. This injury usually occurs on the ulnar side of the hand involving ring and little fingers. We report a case of simultaneous dislocation of both IPJs in the middle finger. Closed reduction and splinting with the IPJs in extension provided a good result with full range of motion at the patient's final follow-up.

[Spontaneous fixation of MP-joint of the index finger in flexion].

Spontaneous fixations of MP-joints of fingers and thumbs are rare and are often a challenging task for hand surgeons. We present a 25-year-old female office employee with a fixed painless MP-joint of the left index finger in 90° flexion with normal PIP- und DIP-joint range of motion. Using the palmar approach to the MP-joint the entrapped accessory collateral ligament which was constrained by a prominent radial condylus could be identified. Operative removal of the prominence has released the joint. An uncomplicated wound healing has followed with completely unrestricted range of motion. If a

Movement of finger joints induced by synergistic wrist motion.

BACKGROUND: An experiment has recently been conducted to evaluate and compare the differences in tendon excursions between the flexor digitorum profundus and superficialis using three mobilization techniques. No previous studies deal with the total joint excursions with constant tendon length. The purpose of this study was to investigate the coordinated motion between the finger and wrist joints resulting from passive tension of the muscles while performing synergistic wrist motion. METHODS: The relative joint positions of the hand and wrist were measured using a three-dimensional motion anal

[Ultrasound diagnosis of hand and finger joints. Changes in arthritis].

Arthritis of the hand and finger joints is seen with unspecific and specific signs, as known from the hip and knee joints. The examination has to be done with special applicators (small sized with 7-13 MHz). Especially signs such as erosions and surrounding synovitis ensure the diagnosis of rheumatoid arthritis.

Numerical scoring systems for the progression of osteoarthritis of the finger joints.

We developed methods to assess and to score progression of osteoarthritis (OA) of the distal and proximal interphalangeal (DIP and PIP) and metacarpophalangeal (MCP) finger joints. Thirty-six patients with osteoarthritis (OA) of the finger joints were followed for five years. Anteroposterior radiographs of the hands were obtained at the start of this prospective study and at yearly intervals. The scoring systems used were based on: -1- the increase in incidence of OA during consecutive years in previously normal joints. -2- the radiological progression of the anatomical lesions (changes in ost
