The creation of an international NORSE/FIRES virtual consultation forum.

BACKGROUND: Febrile infection-related epilepsy syndrome (FIRES) and New-Onset Refractory Status Epilepticus (NORSE) are rare, devastating conditions characterized by sudden-onset seizures in previously healthy individuals. Management is hindered by a limited evidence base, a complex pathophysiology involving innate and adaptive immune dysregulation, and the "trial-and-error" nature of treating medication-resistant seizures within isolated clinical settings. OBJECTIVE: To bridge gaps in clinical experience and evidence, we established a recurring virtual international case-based conference tha

Inflammatory epilepsy (FIRES) and haemophagocytic lymphohistiocytosis (HLH): an adult case.

We describe a man in his 30s with haemophagocytic lymphohistiocytosis (HLH), secondary to an upper respiratory tract infection, with subsequent febrile infection-related epilepsy syndrome. He had a prolonged hospital admission, during which he was treated with chemotherapy for HLH and antiepileptic medications for refractory seizures. He was discharged fully dependent to a care facility and died from aspiration pneumonia 11 months later. This case report highlights his management and discusses these conditions' pathophysiology and future management.

Quenching the fires: Pro-resolving mediators, air pollution, and smoking.

Exposure to air pollution and other environmental inhalation hazards, such as occupational exposures to dusts and fumes, aeroallergens, and tobacco smoke, is a significant cause of chronic lung inflammation leading to respiratory disease. It is now recognized that resolution of inflammation is an active process controlled by a novel family of small lipid mediators termed "specialized pro-resolving mediators" or SPMs, derived mainly from dietary omega-3 polyunsaturated fatty acids. Chronic inflammation results from an imbalance between pro-inflammatory and pro-resolution pathways. Research is o

Moyamoya in a Patient with FIRES: A First Case Report.

Febrile infection-related epilepsy syndrome (FIRES) is a form of epileptic encephalopathy with severe refractory epilepsy that presents in previously healthy, school-aged children after significant febrile illness with concomitant rise in body temperature. Suspected causes include genetic or acquired channelopathies, as well as mitochondrial disturbances. In FIRES, the EEG shows diffuse slowing, generalized, and/or multifocal discharges. Seizures are present and resistant to treatment. Moyamoya angiopathy (MMA) is characterized by progressive stenosis of cerebral arteries and subsequent develo

Comparison of tracheal damage from laser-ignited endotracheal tube fires.

Studies were undertaken by the Departments of Otolaryngology-Head and Neck Surgery and Anesthesiology at Northwestern University Medical School and the Medical College of Wisconsin (Milwaukee) to compare the potential for tissue injury to the trachea and lungs of canines. Polyvinylchloride (PVC), Rusch red rubber, and silicone tubes were tested. The effects of an intraluminal tube fire on the larynx and trachea were documented with laryngeal and bronchoscopic photographs taken immediately postburn and at the time of sacrifice six hours later. The most severe burns were associated with the PVC

Release of volatile and semi-volatile toxicants during house fires.

Qualitative results are presented from analysis of volatile and semi-volatile organic compounds (VOCs/SVOCs) obtained through sampling of gaseous effluent and condensed particulates during a series of experimental house fires conducted in a real house. Particular emphasis is given to the 16 polycyclic aromatic hydrocarbons (PAHs) listed by the Environmental Protection Agency due to their potentially carcinogenic effects. The initial fuel packages were either cooking oil or a single sofa; these were burned both alone, and in furnished surroundings. Experiments were performed at different ventil

Comparison of hypothetical LNG and fuel oil fires on water.

Large spills of refined petroleum products have been an occasional occurrence over the past few decades. This has not been true for large spills of liquefied natural gas (LNG). This paper compares the likely similarities and differences between accidental releases from a ship of sizable quantities of these different hydrocarbon fuels, their subsequent spreading, and possible pool-fire behavior. Quantitative estimates are made of the spread rate and maximum slick size, burn rate, and duration; effective thermal radiation; and subsequent soot generation.

Airborne studies of the smoke from the kuwait oil fires.

Airborne studies of smoke from the Kuwait oil fires were carried out in the spring of 1991 when approximately 4.6 million barrels of oil were burning per day. Emissions of sulfur dioxide were approximately 57% of that from electric utilities in the United States; emissions of carbon dioxide were approximately 2% of global emissions; emissions of soot were approximately 3400 metric tons per day. The smoke absorbed approximately 75 to 80% of the sun's radiation in regions of the Persian Gulf. However, the smoke probably had insignificant global effects because (i) particle emissions were less th

Baptism of Fire: Modeling the Effects of Prescribed Fire on Lyme Disease.

Recently, tick-borne illnesses have been trending upward and are an increasing source of risk to people's health in the United States. This is due to range expansion in tick habitats as a result of climate change. Thus, it is imperative to find a practical and cost-efficient way of managing tick populations. Prescribed burns are a common form of land management that can be cost-efficient if properly managed and can be applied across large amounts of land. In this study, we present a compartmental model for ticks carrying Lyme disease and uniquely incorporate the effects of prescribed fire usin

Fire risk assessment in fire hazardous zones of gasoline stations.

OBJECTIVE: This cross-sectional study aimed to assess fire risk in fire hazardous zones (FHZ) at the dispenser areas of gasoline stations. METHODS: There were 47 stations chosen for fire risk assessment and two zones around the dispenser area of each station were assessed. The assessments were carried out by applying the matrix (3 × 4) of fire risk assessment by multipliers of opportunities level of hazard occurrence and the subsequent severity of the incident. RESULTS: Across the 47 gasoline stations studied, there was an average of 23 ± 12 nozzles and none had vapor recovery systems (VRS)

[Aspiration pneumonitis after fire-eating: fire-eater's lung].

On the emergency department we saw two men aged 19 and 26 with symptoms of lipoid pneumonitis (fire-eater's lung) following aspiration of petroleum during fire-eating. They were both admitted to hospital and treated with amoxicillin and clavulanic acid. Both patients were clinically recovered within a few days. Following aspiration of petroleum there is often a period of latency from 8-24 hours before the symptoms occur; it is recommended that patients should be admitted for observation. Known symptoms are coughing, shortness of breath, thoracic pain, fever, tachypnoea and sometimes haemoptysi

Fire performance of CFRP-strengthened piloti-type columns with fire-resistant materials during standard fire exposure.

This paper presents a numerical investigation into the fire endurance of carbon fiber reinforced polymer (CFRP)-strengthened columns, shielded with fire-resistant materials, in piloti-type reinforced concrete buildings. The strengthened column, equipped with a fire protection system, underwent exposure to the ASTM E119 standard time-temperature curve for a duration of 4 h. To comprehensively evaluate the thermal and structural performance of the strengthened column at elevated temperatures and substantiate the effectiveness of the fire protection system, a fully coupled thermal-stress analysis

[Fire in an electric car: the risks of lithium ion battery fire].

Accidents involving electric vehicles containing lithium-ion batteries are becoming increasingly common in the Netherlands. These incidents can result in exposure to fumes from burning Li-ion batteries, potentially resulting in exposure to higher concentrations of HF and LiOH. The effects of these substances should be considered when treating patients exposed to this type of fume. Both can cause respiratory toxicity if inhaled. Pulmonary oedema due to hydrogen fluoride can develop after a latency period of up to 12 hours and in extreme cases hydrogen fluoride can cause hypocalcaemia. Calcium g

Investigation of thermal penetration from the fire scene into the deceased fire victim.

Investigating fatal fires is a complex multi-disciplinary undertaking, with victims considered to be a fundamental part of the evidence. This study investigated thermal penetration into human remains with the aim of extending the results to the assessment of the fatal fire victim. Data was collected during the burning of 15 unembalmed bodies in years 2017-2019 with the San Luis Obispo Strike Team (SLOFIST) on their annual Forensic Fire Death Investigation Course (FFDIC). Cadavers were placed within a scene of individually built compartment rooms to simulate accidental and intentional fires. Th

Fighting fire with fire: using infectious agents to treat persistent infection.

Infectious diseases lead to significant morbidity and mortality. Often, resolution of the acute stage of the disease leads to microbial persistence, resulting in chronic debilitating disease. Management of persistent infections frequently requires lifelong therapy with antimicrobial agents. These infections could be chronic viral infections like HIV, hepatitis B or chronic bacterial persistent infections like prosthetic joint infections caused by multi-drug resistant organisms. Bacteriophages have been designed specifically to target recalcitrant bacterial infections, such as prosthetic joint
