Adult-acquired flatfoot deformity.

Originally known as posterior tibial tendon dysfunction or insufficiency, adult-acquired flatfoot deformity encompasses a wide range of deformities. These deformities vary in location, severity, and rate of progression. Establishing a diagnosis as early as possible is one of the most important factors in treatment. Prompt early, aggressive nonsurgical management is important. A patient in whom such treatment fails should strongly consider surgical correction to avoid worsening of the deformity. In all four stages of deformity, the goal of surgery is to achieve proper alignment and maintain as

Revision of failed flatfoot surgery.

Revision of failed flatfoot surgery presents a unique and challenging dilemma for the foot and ankle surgeon. Revision surgery is focused on establishing a plantigrade foot with correction of the hindfoot valgus, midfoot abduction, and forefoot varus. Successful reconstruction of failed flatfoot surgery begins with a proper evaluation. No treatment algorithm exists for the management of the malaligned flatfoot. Patient complaints, an understanding of the initial deformity and biomechanical problems, and surgeon experience play a role in correction of failed flatfoot surgery.

Flatfoot deformity pathogenesis. A trilogy.

The significant conclusion from this trilogy is that most developmental flatfeet are present at birth. It is likely that intrauterine forces and fetal foot position initially create the abnormal bony relationships between anatomically normal bones. This bony relationship is present before the child takes his first step. Second, as the result of this abnormal relationship between normal bones, the active muscular force balance between opposing muscle groups is distorted. This allows one muscle to have a mechanical advantage over its antagonist. This actively maintains and progressively worsens

Tarsal coalition and painful flatfoot.

The prevalence of tarsal coalition is probably 1% or less. The two sites most commonly affected are the calcaneonavicular joint and the middle facet of the talocalcaneal joint. Diagnosis should be suspected in the preteen or teenage patient with insidious or sudden onset of pain in the midfoot to hindfoot associated with a lack of motion in the subtalar joint. Initial treatment with immobilization or an orthosis may relieve symptoms, but most patients will have persistent symptoms that warrant surgical correction. Long-term results indicate that excision of the coalition is moderately successf

[Insole management of pediatric flatfoot].

Five components can be differentiated in the development of flexible flatfeet. In children, a slight form of flatfeet is physiological; however, the more pronounced form requires correction. The criteria for the use of insoles are discussed, and the decisive factor in making this decision is to examine the child in normal standing or walking position. The principles of correction and various types of insoles are presented, and the advantages and disadvantages an discussed. Attention is paid in particular to the role of the shoe when the foot is in the standing position or in motion. In additio

[Morphofunctional changes in human flatfoot].

The process of the flatfoot formation is accompanied not only by stretching but also by pressing of the plantar ligaments and muscles. In the plantar aponeurosis and ligaments subjected to pressing, adaptive rearrangements are observed; they are accompanied by transformation of the fibrillar connective tissue into cartilaginous one, that rather prevents the ligaments to affect the heigh of the fornix. In the structure of the plantar muscles rather noticeable changes occur, destructive ones including, that is seen in their microscopical picture.

Symptomatic flatfoot in cerebral palsy.

PURPOSE OF REVIEW: The purpose of this review is to evaluate the current literature and best practices in the evaluation and treatment of symptomatic flatfoot in cerebral palsy. RECENT FINDINGS: While techniques to reconstruct the neuromuscular flatfoot and reestablish bony levers have remained similar over time, the concept of surgical dosing has helped guide appropriate interventions based on the magnitude of disease and functional level of the child. Moreover, the utilization of multisegment foot modeling in motion analysis has allowed quantitative description of such deformities and their

Treatment of Stage 4 Flatfoot.

Stage 4 flatfoot represents only a small proportion of flatfoot cases and is likely to become even rarer. The evidence base to guide treatment is limited to case series and expert opinion. Therefore, a pragmatic approach to treatment must be taken. Low-demand individuals may manage well with conservative treatment. Surgical management is complex, likely to require staging, and has a significant complication profile. Patients should be fully informed and understanding of this. First principles of surgery should be followed, including restoring hindfoot and ankle joint alignment, appropriate sof

Pediatric Flatfoot: Pearls and Pitfalls.

Pediatric flatfeet are common, are usually asymptomatic, and typically improve over time as young children age. It is critical to differentiate flexible from rigid flatfeet and to assess for associated Achilles contracture with a careful history, physical examination, and initial radiographs. Although there are limited data, nonsurgical management of symptomatic flatfeet, both flexible and rigid, should be exhausted before considering surgical intervention. If patients fail conservative treatment, surgical management with joint-preserving, deformity-corrective techniques is typically used for

What's New in Pediatric Flatfoot?

BACKGROUND: Children with flatfeet are frequently referred to pediatric orthopaedic clinics. Most of these patients are asymptomatic and require no treatment. Care must be taken to differentiate patients with flexible flatfeet from those with rigid deformity that may have underlying pathology and have need of treatment. Rigid flatfeet in infants may be attributable to a congenital vertical talus (CVT); whereas those in older children and adolescents may be due to an underlying tarsal coalition. We performed a review of the recent literature regarding evaluation and management of pediatric flat

Radiographic assessment of adult flatfoot.

BACKGROUND: The accurate measurement of flatfoot on standing radiographs allows correct diagnosis of the condition and evaluation of reconstructive procedures. METHOD: The standing radiographic measurements of patients with symptomatic, adult flatfoot were compared to controls using blinded observers. RESULTS: On the lateral radiograph, the talar-to-first metatarsal angle, the calcaneal pitch angle, and the medial cuneiform-fifth metatarsal height differed significantly between the patient group and the controls. The difference in the talar-to-first metatarsal angles on lateral radiographs w

The rigid flatfoot. Tarsal coalitions.

Tarsal coalitions are the major cause of painful rigid flatfoot deformity in children and adolescents. Talocalcaneal and calcaneonavicular coalitions are the most common sites. They are often bilateral and may be asymptomatic. They represent a failure of fetal mesenchymal differentiation, and onset of symptoms in children often corresponds to the time of ossification of the fibrous or cartilaginous coalition. The most common presenting symptom is pain, and diagnosis is aided by plain radiographs and computerized tomography. Some patients respond to conservative measures, but surgical treatment
