Multidisciplinary Routine Outcome Monitoring in Forensic Psychiatry: Concordance Between Clinician and Patient Assessment.

Patient involvement in routine outcome monitoring (ROM) is vital for collaborative treatment and understanding differences between patient and clinician perspectives. This study assessed the agreement between patient and clinician ratings using the Instrument for Forensic Treatment Evaluation (IFTE) and its self-report version (IFTE-S) in high-security forensic psychiatric centers in Flanders, Belgium. Results showed concordance in domains like protective behavior and resocialization skills, but patients rated their problematic behavior as less severe than clinicians did. Item-level analyses r

[Disastrous conditions: Forensic Psychiatry in Eastern Germany - The 'Haus 213' in Berlin-Buch].

AIM AND METHODOLOGY: This article analyzes the little-examined forensic psychiatry in the GDR after the enactment of the Criminal Code of 1968. Using files from the Berlin State Archive, the Federal Archive and the Stasi Records Archive, it traces the historical development of the forensic psychiatric clinic in Berlin-Buch, "Haus 213". RESULTS: Like other closed psychiatric clinics, House 213 suffered from overcrowding, inadequate security measures and increasingly catastrophic structural conditions until the end of the GDR. After the 1968 reform and the abolition of the GDR's "Maßregelvollzu

Toward an Integrated Sexual Policy in Forensic Psychiatry: A Mixed Method Pilot Study.

Forensic inpatients reside for long periods in restricted environments, which do not support the presence of sexual experiences or the expression of existing needs. However, sexuality and sexual health are important aspects in the overall recovery from mental illness. Given the lack of national policies, management decisions are bestowed upon individual institutions and staff members. This research aims to describe the current sexual policies in 32 forensic psychiatric wards in Flanders (the Dutch-speaking part of Belgium), varying from low to high security, and explore the perspective of fore

Research Progress and Application Prospect of Facial Micro-Expression Analysis in Forensic Psychiatry.

Research on facial micro-expression analysis has been going on for decades. Micro-expression can reflect the true emotions of individuals, and it has important application value in assisting auxiliary diagnosis and disease monitoring of mental disorders. In recent years, the development of artificial intelligence and big data technology has made the automatic recognition of micro-expressions possible, which will make micro-expression analysis more convenient and more widely used. This paper reviews the development of facial micro-expression analysis and its application in forensic psychiatry,

Homicide-suicide: current evidence, risk assessment, and medico-legal implications in forensic psychiatry.

Homicide-suicide is a rare but highly impactful form of lethal violence characterized by the convergence of suicidality, interpersonal aggression, and psychopathology. This narrative review summarizes current evidence regarding epidemiology, major subtypes, interpretative models, psychiatric and relational risk factors, risk assessment strategies, prevention approaches, and medico-legal implications within forensic psychiatry. Available data suggest that homicide-suicide represents a distinct phenomenon rather than the mere succession of homicide and suicide, most frequently occurring in intra

Conflicting minds: Immanuel Kant, Johann Daniel Metzger, and the debate about forensic psychiatry.

This article explores the dispute between the philosopher Immanuel Kant and the physician Johann Daniel Metzger over the moral autonomy of individuals with mental illness. Situating the debate within the broader context of the evolving philosophical and medical professions in eighteenth-century Germany, the article examines how a professional conflict emerged over who - the physician or the philosopher - should serve as the legal authority in cases where moral responsibility was in question. The analysis shows that this was not merely a theoretical issue for Kant, but a practical one, brought

Court-ordered forensic psychiatry treatment in prison: determinants of outcome and risk mitigation.

BACKGROUND: Court-ordered forensic psychiatry treatments (COT) are specifically designed to reduce the risk of violence in mentally disordered offenders. Given their high costs and ethical issues, mental health professionals need admission criteria to be able to select those candidates with optimal benefit. This study analyses offender-related and treatment-related determinants of COT outcome and risk mitigation. METHODS: This two-year longitudinal study assessed the evolution of 117 adult offenders admitted to a specialized medium-security forensic psychiatry clinic. Treatment outcome includ

Forensic psychiatry evaluation in a U. S. immigration detention center in southwest Texas.

Immigration and asylum-seeking are issues that have recently. warranted increased attention and significance. In the aftermath of terrorist attacks, people are more wary of asylum seekers who are usually held in detention centers after fleeing persecution and not for criminal or immigration charges. Asylum seekers may raise suspicions due to inconsistent statements and can be held in detention for the duration of the process and/or denied asylum. Inconsistencies may be influenced by psychiatric symptoms, cognitive abilities, the type of past trauma experienced, the questioning style of the int

Determinants of clinical outcome and length of stay in acute care forensic psychiatry units.

Criminological and sociodemographic variables, such as previous criminal convictions, increased risk of violence, early onset of mental disorder, antisocial personality, psychosis and low social support, have all been related to longer length of stay (LoS) and poorer outcome in long stay forensic services. The factors impacting on LoS and clinical response in acute care specialized units are poorly documented. To address this issue, we examined the psychiatric records of all cases admitted between January 1st and December 31th 2020 in the sole acute ward for detained persons located in the cen

[Forensic-Psychiatric Consultations in General Psychiatry - Supporting Treatment of Patients Prone to Commit Violence].

A subgroup of psychiatric patients are at increased risk of committing interpersonal violence, which may lead to placements in forensic-psychiatric institutions. The majority of patients treated in forensic hospitals have had contact with the general psychiatric care system years before being forensically committed due to an offence. Nevertheless, attempts to establish models related to violence prevention in general psychiatry have remained sparse. In the Canton of Zurich, the forensic psychiatric consultation liaison service provides the general psychiatric clinics with access to forensic ps

When forensic psychiatry clashes with jurisdiction: nemesis and challenges of a patient suffering from glioblastoma.

This case study explores the interaction of brain pathology, criminal behaviour and art in forensic psychiatry through the case of a 68-year-old man exhibiting neuropsychiatric symptoms and delusions. His progressive cognitive and emotional deterioration led to aggressive behaviour, threats towards colleagues and family and allegations of violent and sexual assault. After months of his refusing treatment, magnetic resonance imaging revealed a grade IV glioblastoma. Despite the terminal diagnosis, he was placed in a forensic acute psychiatric unit while under prosecutorial investigation, becaus

[A mistake in forensic psychiatric evaluation or abuse of psychiatry for non-medical purposes - a case report].

In this study we attempted to visualize certain irregularities that took place in the evaluation of a patient with personality disorders performed by psychiatrist expert witness, which resulted in an incorrect diagnosis, leading to wrong ruling of the court and a referral of the patient to clinical therapy lasting six years. The psychiatric and psychological expert opinions submitted to the court and first-hand psychiatric and psychological examination of the patient were analyzed. Efforts were made to show that the failure to comply with the diagnostic criteria in the process of diagnosis and

Long-term seclusion in forensic psychiatry: Bare life, social death, and the role of the nurse.

BackgroundThe use of seclusion and other restraints in forensic mental health settings are intended to be employed as a last resort and for the shortest time possible to achieve therapeutic effect. And yet, in Canadian forensic hospital settings, patients can remain in seclusion for weeks, months, even years. An ambiguous legal framework across Canadian provinces lacks mechanisms to limit or forbid the long-term seclusion of patients, which we define as any continuous confinement lasting 15 days or longer.Research objectivesThe paper undertakes a critical examination of long-term seclusion pra

The prevalence of antipsychotic polypharmacy and its associations with violent behaviors among forensic psychiatry patients.

BACKGROUND: Effective control of psychotic symptoms and behavioral problems is crucial in forensic psychiatric settings to enhance recovery and manage different risk concerns. While antipsychotic polypharmacy is commonly employed to ensure symptom control and risk mitigation, there is limited research on its burden and relationship with violent and aggressive behaviors in forensic psychiatric populations. OBJECTIVES: This study aims to examine the prevalence of antipsychotic polypharmacy among forensic psychiatry patients in Ontario and assess its relationship with recent incidents of aggressi

Offenders with Autism Spectrum Disorder: A Clinical Case Series and Diagnostic Considerations in Forensic Psychiatry.

PURPOSE: Autism spectrum disorder (ASD) is increasingly recognized in adulthood, including in forensic settings, where its diagnostic and medico-legal implications remain insufficiently defined. Current evidence does not support a simple association between ASD and offending; rather, offender presentations in autistic individuals appear heterogeneous and are likely shaped by the interaction between core autistic features, intellectual and adaptive functioning, comorbidity, developmental adversity, and contextual stressors. PARTICIPANTS AND METHODS: This clinical-forensic case series describes

The uptake and utility of a reflective practice group for psychiatry registrars in a state-wide forensic service.

OBJECTIVES: It is essential that mental health clinicians have access to reflective processes where they may understand and make sense of emotional responses to patients, teams, and organisations. The authors share their experience of initiating and successfully running a reflective practice group, framed with Balint principles, for psychiatry registrars working in forensic settings across Victoria. METHOD: We describe the process of setting up a Balint group for this professional population. Qualitative feedback was obtained from group members. The data was analysed thematically, regarding m

Model Building in Forensic Psychiatry: A Machine Learning Approach to Screening Offender Patients with SSD.

Today's extensive availability of medical data enables the development of predictive models, but this requires suitable statistical methods, such as machine learning (ML). Especially in forensic psychiatry, a complex and cost-intensive field with risk assessments and predictions of treatment outcomes as central tasks, there is a need for such predictive tools, for example, to anticipate complex treatment courses and to be able to offer appropriate therapy on an individualized basis. This study aimed to develop a first basic model for the anticipation of adverse treatment courses based on prior

The effect of clozapine on the social behaviour schedule in patients attending a forensic psychiatry day hospital.

This study examined the difference in the functional capacity of patients receiving clozapine, compared with those prescribed other antipsychotic medication, who attend the forensic psychiatry day hospital at Murray Royal Hospital, Perth. (This facility caters for 34 patients. It provides a number of advantages such as access to therapies and supervision of mental state and medication at this critical time in the care pathway.) This study was cross-sectional in nature and examined the patients' demographic details, diagnosis and medication. A functional assessment was carried out using the soc

[Are too Many Temporarily Placed Non-culpable Offenders Committed to Forensic Psychiatric Hospitals in Germany? Thoughts on the Relationship of Forensic and General Psychiatry].

From the perspective of the forensic clinics, the judiciary is increasingly ordering temporary placements according to Sec. 126a of the Code of Criminal Procedure. Three hypotheses are proposed that could (partly) explain this increase: 1) Courts' tendency to hand down this decision even in cases involving minor offences. 2) Courts' tendency to hand down this decision despite positive prognoses. 3) Changes in the reporting practices of psychiatric clinics. Overly simple explanations for the increase in temporary placements, therefore, fall short. This makes it more urgent to strengthen the pri

Weapon use during the index offense: a study among forensic psychiatry patients in Ontario, Canada.

BACKGROUND: Understanding the nature and circumstances around the use of weapons to perpetrate an offense among individuals with mental illness is crucial for evidence-informed policies and actions. However, little is known about the prevalence and factors associated with weapon use during index offenses among patients in the forensic system. Therefore, the present study was designed to address this gap and determine the prevalence and the patient and victim characteristics associated with weapon use during the index offense in a Canadian province. METHODS: This retrospective exploratory stud
