[Cyanide poisoning: forensic toxicology observations in the study of 54 cases of fatal poisoning].

The present study describes various observations made during the examination of 54 cases of lethal cyanide intoxication at the Institute of Forensic Medicine of the University of Zürich during a period of more than 40 years. Data pertain to the scene of death, the medicolegal inspection, the autopsy, the histological examinations, the chemical analyses, the various types of poisoning observed and the diagnostic criteria used. The intoxicated victims were mostly adults who had professional access to various cyanogenic compounds and had ingested them with the intention of committing suicide. Cas

Machine learning in forensic toxicology: Concepts, applications and challenges in bioanalysis, ADME, and toxicodynamics.

Forensic toxicology focuses on the detection, quantification, and interpretation of medicinal and recreational drugs, other chemicals or poisons, and their metabolites in biological matrices. Chromatography, combined with mass spectrometry (MS), is the most widely used analytical technique. However, forensic toxicology faces increasing analytical challenges due to a continuously changing drug landscape. In particular, the emergence of new psychoactive substances (NPS) has driven the development of more complex analytical methods (e.g., high-resolution mass spectrometry), novel markers (e.g., m

An enduring threat: NPS benzodiazepines in forensic and clinical toxicology from 2021 to 2025.

The proliferation and sustained detection of novel psychoactive benzodiazepines in forensic and clinical toxicology, combined with their potential for adverse events, poses an enduring threat to public health and safety with complex characteristics and challenges unique from other subclasses of novel psychoactive substances (NPS). This study aimed to systematically review the published effects, observations, and toxicological data from postmortem and human performance studies [including clinical, driving under the influence of drugs (DUID), and drug-facilitated crimes (DFC)] involving NPS benz

Two fatal cases of lacosamide poisoning with contrasting postmortem findings: implications for forensic toxicology.

Lacosamide is a third-generation antiepileptic drug with expanding clinical use worldwide. Although generally considered safe, it has occasionally resulted in fatal intoxications; however, detailed postmortem data remain limited. Here, we describe two fatal cases of acute lacosamide poisoning with contrasting postmortem findings. In case 1, the cardiac blood concentration was extremely high (200 µg/mL). The gastric contents contained no visible solid residue at autopsy, although computed tomography images revealed hyperdense material in the stomach, extending toward the pyloric region. The car

Recent challenges and trends in forensic analysis: Δ9-THC isomers pharmacology, toxicology and analysis.

Δ9-tetrahydrocannabinol (Δ9-THC) isomers, especially Δ8-tetrahydrocannabinol (Δ8-THC), are increasing in foods, beverages, and e-cigarettes liquids. A major factor is passage of the Agriculture Improvement Act (AIA) that removed hemp containing less than 0.3 % Δ9-THC from the definition of "marijuana" or cannabis. CBD-rich hemp flooded the market resulting in excess product that could be subjected to CBD cyclization to produce Δ8-THC. This process utilizes strong acid and yields toxic byproducts that frequently are not removed prior to sale and are currently inadequately studied. Pharmacologic

Skeletal tissue, a viable option in forensic toxicology? A view into post mortem cases.

Blood analysis is the golden standard in the field of forensic toxicology. However, when extended decomposition of the remains has occurred, alternative matrices are required. Skeletal tissue may provide an appropriate sample of choice since it is very resistant to putrefaction. However, today, the absence of reference data of drug concentrations in skeletal tissue poses a problem to meaningfully and reliably conduct toxicological testing on human skeletal material. The present study investigates the viability of skeletal tissue as an alternative matrix to evaluate xenobiotic consumption in le

Food toxicology--from forensic to policy tool: an evolution from reactive to proactive action.

Since the nineteenth century, food toxicology has evolved in several ways. The analytical power of the food toxicologist has been increased. The concept of adulterated food was modified significantly. Most commodities are now processed and addition of numerous non-nutrient additives is legalised. Other "alien" compounds get introduced in the food chain from background contamination or during processing and distribution. This makes the task of the food toxicologist more complex. From forensic science food toxicology developed to a policy aid.

Toxicological risk profiles in suicide: The forensic value of hair-based substance detection.

INTRODUCTION: Suicide is a major global cause of premature mortality and a critical public health concern. Chronic alcohol misuse and illicit drug use are well-established risk factors, but conventional post-mortem toxicology frequently fails to detect long-term substance exposure. Hair analysis represents a valuable forensic tool, enabling the reconstruction of pre-mortem behavioral patterns and offering insight into chronic substance use and its association with suicidal behavior. MATERIALS AND METHODS: This retrospective study examined 74 suicide cases (59 males, 15 females) from 2023 to 2

A Pilot Study on the Deposition of Drugs in Dental Tissues as Alternative Matrices for Forensic Toxicology.

BACKGROUND: Teeth may serve as alternative matrices for post-mortem toxicological analysis. Although the mechanism of drug incorporation into dental tissues remains unclear, it is hypothesized that orally ingested substances can permeate dental enamel and reach dentin and the pulp. OBJECTIVES: This study investigated enamel permeability to commonly ingested drugs, considering molecular characteristics (methadone - MET and dextromethorphan hydrobromide - DXM), pH, sugar content, and tooth conditions (intact enamel, cement-enamel junction-CEJ exposure, enamel wear). MATERIALS AND METHODS: Sixt

Stability of synthetic cathinones in clinical and forensic toxicological analysis-Where are we now?

Understanding the stability of analyzed drugs in biological samples is a crucial part for an appropriate interpretation of the analytical findings. Synthetic cathinones, as psychoactive stimulants, belong to a major class of new psychoactive substances. As they are subject to several degradation pathways, they are known to clinical and forensic toxicologists as unstable analytes in biological samples. When interpreting analytical data of synthetic cathinones in biological samples, analysts must be aware that the concentration of analytes may not accurately reflect the levels at the time they w

Efficacy of drug screening in forensic autopsy: retrospective investigation of routine toxicological findings.

Toxicological analysis is indispensable in forensic autopsy laboratories, but often depends on the limitations of individual institutions. The present study reviewed routine drug screening data of forensic autopsy cases (n=2996) during an 18.5-year period (January 1996-June 2014) at our institute to examine the efficacy of the procedures and findings in autopsy diagnosis and interpretation. Drug screening was performed using on-site immunoassay screening devices and gas chromatography/mass spectrometry (GC/MS) in all cases, followed by re-examination using GC/MS and liquid chromatography/tande

"Detection of sedative and antipsychotic in exhumed institutionalized patients: a forensic toxicological case series".

Post-mortem toxicological analysis in exhumed bodies presents considerable methodological and interpretative difficulties. These limitations become particularly relevant in judicial contexts involving elderly individuals in long-term care, where pharmacological regimens may be deliberately omitted from medical records. This study investigates a series of 15 exhumed cases in which the detection of sedative and antipsychotic drugs, particularly Midazolam and Promazine, raised concerns of potentially unauthorized or inappropriate administration. All cases underwent full forensic autopsy, includin

Forensic and toxicological insights into aconite poisoning: A retrospective analysis of clinical and postmortem findings.

Aconitine, a highly toxic alkaloid derived from the aconite plant, presents significant clinical and forensic challenges due to its cardiotoxic and neurotoxic effects. It may induce fatal ventricular arrhythmias and severe central nervous system disturbances when improperly used. The diagnosis of aconitine poisoning remains difficult, particularly in postmortem contexts, owing to its rapid metabolism and the non-specific nature of autopsy findings. In this retrospective study, we analyzed 35 reported cases of aconitine poisoning published between 1958 and 2024. Each case was evaluated for demo

Drug-Induced Epigenetic Alterations: A Set of Forensic Toxicological Fingerprints?-A Systematic Review.

BACKGROUND/OBJECTIVES: Epigenetics refers to heritable modifications in gene expression that do not involve changes to the DNA sequence. Among these, DNA methylation, histone modifications, and non-coding RNAs play a key role in regulating gene activity and are influenced by environmental factors, including exposure to psychoactive substances. In recent years, it has been hypothesized that such alterations may serve as molecular markers with forensic relevance. This systematic review aims to evaluate whether current evidence supports the use of drug-induced epigenetic changes as potential toxi

A unique case of death by MDPHP with no other co-ingestion: a forensic toxicology case.

Synthetic cathinones are a class of psychoactive drugs that have become, in recent years, of quite common observation in medical and toxicological forensic field. MDPHP (3,4-methylenedioxy-α-pyrrolidinohexanophenone) belongs to this class of substances but lethal acute intoxication caused by this specific substance without other co-ingestions has never been described in literature. We are presenting the unique case of a fatal acute intoxication caused by MDPHP in a 48-year-old man carried to the A&E department of a big Milanese hospital where he suddenly deceased after brief time. Clinical rec

Application of Paper-Based Microfluidic Analytical Devices (µPAD) in Forensic and Clinical Toxicology: A Review.

The need for providing rapid and, possibly, on-the-spot analytical results in the case of intoxication has prompted researchers to develop rapid, sensitive, and cost-effective methods and analytical devices suitable for use in nonspecialized laboratories and at the point of need (PON). In recent years, the technology of paper-based microfluidic analytical devices (μPADs) has undergone rapid development and now provides a feasible, low-cost alternative to traditional rapid tests for detecting harmful compounds. In fact, µPADs have been developed to detect toxic molecules (arsenic, cyanide, etha
