Fournier gangrene - would you KISS it?

Fournier gangrene is a disease characterized by necrotizing fasciitis of the perineal and genital region, resulting from synergistic polymicrobiotic infection. Most infections can be localized to a cutaneous, urethral, or rectal source and can culminate in a fulminant sepsis. Current state of the art is systemic broad-spectrum antibiotics and serial aggressive debridement which result in superficial perineal defect of wide dimensions. We compiled all the cases of Fournier gangrene that required reconstruction after debridement in Centro Hospitalar Universitário Lisboa Central from 2018 to 2022

Fournier gangrene due to Rhizobium Radiobacter.

Fournier's gangrene (FG) is a life-threatening, rapidly progressing acute necrotizing fasciitis of perianal, genitourinary and perineal areas. The most common isolated pathogens are E.coli, Streptococcus and Enterococcus in tissue and abscess cultures. However we present the first Rhizobium radiobacter FG in this case. A 47-year-old man presented with black necrotizing area with malodorous pus drainage in the bottom of the right hemiscrotum. Intravenous imipenem and Clindamycin were started prophylactically; all necrotizing tissues were debrided and right hemiscrotectomy was performed. Rhizobi

Presentation and Diagnosis of Fournier Gangrene.

Necrotizing fasciitis is a severe type of necrotizing soft tissue infection involving the superficial fascia and subcutaneous tissues. Fournier gangrene, a type of necrotizing fasciitis, affects the genitalia or perineum. Although a rare health condition, Fournier gangrene can result in significant morbidity and unnecessary mortality following delay in diagnosis and management. We provide a review of relevant presenting features to aid diagnosis and allow timely surgical management of this serious infectious condition.

[Extensive Fournier gangrene. A dermatologic emergency].

Fournier gangrene is a necrotizing fasciitis of the perineal and genital region, which almost exclusively affects men. The cause is a polymicrobial infection associated with superficial trauma, urological diseases and operations, as well as colorectal diseases. Diabetes mellitus, alcoholism, immunosuppression and other severe illnesses are frequent co-factors. Immediate administration of systemic broad-spectrum antibiotic therapy with coverage of both gram-positive and gram-negative bacteria combined with surgical debridement and intensive medical care can lower the high mortality rate of this

Fournier gangrene associated with Crohn disease.

A 17-year-old boy presented with Fournier gangrene associated with previously undiagnosed Crohn ileocolitis. Fournier gangrene was managed by débridement, broad-spectrum antibiotics, and hyperbaric oxygen. A diverting ileostomy was performed before skin grafting and scrotal reconstruction. Microscopy of a full-layer surgical sample from the terminal ileum revealed granulomas with multinucleated histiocytes, consistent with Crohn disease. Crohn disease was treated with mesalamine, metronidazole, 6-mercaptopurine, and infliximab. The patient was discharged on hospital day 32. At 6-month follow-u

[Treatment of Fournier's gangrene].

Fournier's gangrene is a necrotizing fasciitis and soft-tissue infection of the perineal region. The clinical course of the disease, in many cases, is fulminant. This case presents a patient with symptoms of Fournier's gangrene that developed during hospitalization following a vehicular accident. The disease was diagnosed six days into hospitalization. The patient underwent numerous surgical procedures. Escherichia coli and Enterococcus faecalis were found in a culture of the surgical wound. Because of the progressing septic shock, the patient was admitted to the Intensive Care Unit. Treatment

Fournier's gangrene in Ibadan.

Twenty six patients with Fourniers gangrene seen over an 8 year period at the University College Hospital Ibadan are presented. Their ages ranged from 34 years to 70 years with a mean of 52.5 years. Possible predisposing conditions were present in 17 (65%) patients. The anterior scrotal skin alone was most commonly involved. They were treated with systemic antibiotics, wound debridement and sitz baths. While the wound healed spontaneously on this regimen in 6 (23%) patients, 19 (73%) patients required suture closure of their clean granulating wounds. None required rotational skin flap scrotopl

[A case of Fournier's gangrene].

A 59-year-old man was admitted to our hospital complaining of perineal pain and scrotal redness. He was diagnosed with diabetes mellitus 12-years ago but had not received treatment. The scrotum soon showed necrosis and pus discharge. A culture study revealed Clostridium, Bacteroides and many other bacteria. Antibiotics chemotherapy (CMZ, AMK, MINO) was done but because necrotizing fasciitis progressed, we performed surgical treatment of necrosis. The skin defect was closed naturally with wound irrigation. The patient is doing well and his diabetes is under control. We found 13 other cases of F

Fournier's gangrene of the penis.

Fournier's gangrene is a rare, fulminant, though generally localizaed disease of the scrotum and penis. Three cases are presented in which the sexual practices of the patients may have played a role in the etiology. Hospitalization, parenteral antibiotics, and appropriate surgical treatment are required.

Is Fournier's gangrene severity index useful for predicting outcome of Fournier's gangrene?

OBJECTIVES: Fournier's gangrene (FG) is a rare but life-threatening disease. Although antibiotics and aggressive debridement have been broadly accepted as the standard treatment, the mortality rate remains high. We conducted a retrospective study to analyze the outcome and identify the risk factors and prognostic indicators. METHODS: We retrospectively reviewed the medical records of 25 patients diagnosed with FG between July 1993 and August 2003. Data collected included age, predisposing factors, treatment modalities, length of hospital stay, surgical debridement times, and outcome. The FG s

Caring for a patient with Fournier gangrene.

Fournier gangrene (FG) is a form of necrotizing fasciitis that affects the genital and perineal tissues. This life-threatening condition is most often caused by a polymicrobial bacterial infection of the tissues. Predisposing factors include obesity, advanced age, chronic alcohol abuse, and immunosuppression. Although rare, FG can be life-threatening and can significantly impact the patient's quality of life. Adept nursing care can improve patient outcomes. This article discusses the pathophysiology and common complications of FG and outlines expected nursing considerations for patients with t

Fournier Gangrene Management: Is a Stoma Mandatory?

Fournier gangrene is a rare life-threatening surgical emergency mainly due to a polymicrobial infection of the perineal, genital, or perianal region. It is characterized by rapid tissue destruction and systemic signs of toxicity. It is more frequent in males and immunocompromised patients, such as patients with poorly controlled diabetes, alcoholism, or human immunodeficiency virus (HIV) infection. Treatment often involves surgical intervention, broad-spectrum antibiotic therapy, fecal diversion surgery, and negative pressure wound therapy (NPWT). Delays in diagnosis are associated with high m

[Advances in the research of Fournier gangrene].

Fournier gangrene is a relatively rare clinical critical disease, and its clinical symptoms are not specific and easily unrecognized by some clinicians. It has the features of acute onset, quick development, severe illness, and often accompanied by infection shock which is seriously life-threatening. It is difficult in treatment with high medical costs and long length of hospitalization, which increases pain for patients and relatives and brings heavy economic and psychological burden on patients, society, and medical workers. By reviewing the literature home and abroad and combined with clini

Sudden unexpected death due to Fournier gangrene.

Fournier gangrene is a rare fulminant necrotizing infection of the skin and soft tissue of the perineum that may result in rapid death. We describe the autopsy case of a 70-year-old female with uncontrolled diabetes mellitus who was found unexpectedly dead in her house where she lived with her dog. Examination of the body revelaed a deep necrotic ulceration of the perineum with exposure of the underlying deep muscles that was initially misinterpreted by police officers as post mortem animal predation. Medicolegal examination and histological and microbiological analyses eliminated this hypothe
