We describe a series of 20 patients with ununited fractures of the femoral neck following neglected trauma or failed primary internal fixation who were seen at a mean of 7.5 months (2 to 18) following injury. Open reduction and internal fixation of the fracture was performed in all patients, together with a myoperiosteal flap on the quadratus femoris muscle pedicle. Union occurred at a mean of 4.9 months (2 to 10) in all patients. The mean follow-up was for 70 months (14 to 144). A 61-year-old man, with a history of right clavicular fracture 35 years prior, visited our hospital due to the sudden onset of vertigo and tinnitus following weakness and numbness in his left arm and leg. He also had a 6-month history of right arm pain with overuse. Brain MRI showed acute brain infarcts in the right posterior cerebral artery territory. We treated 16 cases of delayed union and nonunion of proximal one-third scaphoid fractures with cancellous bone grafting and retrograde insertion of a Herbert screw through a dorsal approach. Definite radiographic union was obtained in 13 of 16 patients after a median of 2 (0.5-8) years of follow-up. Using Cooney's clinical scoring system, 5 cases were excellent, 5 good, 5 fair and 1 poor. The treatment of ununited proximal pole scaphoid fractures with retrograde insertion of the Herbert screw offers the advantages of a short period of immobilization and a good function.

A 63-year-old man presented with a complaint of left ulnar wrist pain after playing golf. Computed tomography was used to detect a fracture at the base of the hook of hamate. CASE: An 18-year-old autistic boy with seizure disorder had a 4-month-old hip injury with a similar right hip injury 6 months earlier. X-rays revealed an ununited fracture neck femur on the left and a malunited fracture neck femur on the right hip. Magnetic resonance imaging indicated preserved head vascularity. An ununited scaphoid fracture whose proximal fragment is small, sclerotic, fragmented, or badly deformed is usually not suitable for bone-grafting. This is the anatomico-radiological study of a 47-year-old ununited intracapsular fracture of the hip. The femoral head was osteoporotic but its center contained a zone of osteomedullary necrosis in the form of a bone infarct. Its contour was reshaped and remnants of articular cartilage were covered by osteophyte-like bone formations; furthermore, an acetabular osteophyte was observed.
