Hypercalcemia as a Biomarker of Poor Prognosis in Frail Elderly Patients with COVID-19.

The objective of this cohort study was to determine whether hypercalcemia in early COVID-19 was associated with 3-month mortality in frail elderly patients. Circulating calcium and albumin concentrations at hospital admission and 3-month mortality were assessed in geriatric patients hospitalized for COVID-19 with normal-to-high calcium concentrations. Hypercalcemia was defined as corrected calcium >2.5mmol/L. Covariables were age, sex, functional abilities, malignancies, hypertension, cardiomyopathy, number of acute health issues, use antibiotics and respiratory treatments. In total, 94 partic

Musculoskeletal adaptations to strength training in frail elderly: a matter of quantity or quality?

BACKGROUND: The improvement in muscle strength generally exceeds the increase in muscle size following strength training in frail elderly, highlighting the complex aetiology of strength deficit in aging. The aim of this study was to investigate the effect of heavy-load strength training on a broad number of factors related to specific strength in frail elderly. METHODS: Thirty-four frail elderly men (n = 18) and women (n = 16) aged 67 to 98 (86 ± 7 years) were randomized to either a group performing strength training twice a week for 10 weeks (ST) or a non-exercising control group (CON). Evaluation of chewing and swallowing disorders among frail community-dwelling elderly individuals.

The purpose of the present study was to develop a new assessment scale to evaluate the risk of chewing and swallowing disorders among frail community-dwelling elderly individuals. Subjects were 85 frail elderly individuals (29 men and 56 women) living in southern Japan. First, we pooled 18 subjectively evaluated variables identified by previous studies as being related to chewing and swallowing disorders. Secondly, using factor analysis, we extracted 12 variables as the 'dysphagia risk assessment for the community-dwelling elderly' (DRACE) and examined its reliability using Cronbach's alpha co

MitraClip-related infective endocarditis in a frail, elderly patient: a case report.

BACKGROUND: The incidence of infective endocarditis (IE) following a MitraClip is rare with 17 reported cases in the literature. The reported mortality rate is high, at 41%, despite both medical and surgical therapies. To date, this is the first documented case of IE following a MitraClip procedure in Australia. CASE SUMMARY: An 88-year-old male presented with a 1-week history of confusion and dyspnoea. Clinical examination was significant for a temperature of 37.7°C, a pansystolic murmur and bilateral pitting oedema to mid-shin, but no peripheral stigmata of IE. His history included a MitraC

[Utilization of external hip protectors in frail elderly residents of nursing homes].

Falls are common among the elderly and stem from multiple causes. Much attention has been given to the development of falls-prevention programs, mainly because of the substantial risk for hip fracture sustained after a fall. The population of frail elderly nursing home residents is a high-risk group for falls. The utilization of an external hip protector that covers the greater trochanter of the femur has been shown to reduce the incidence of hip fractures. However, low compliance remains a major obstacle in the effective use of hip protector systems.

Herpes zoster in frail elderly patients: prevalence, impact, management, and preventive strategies.

Population aging is a worldwide phenomenon with significant and manifold impacts on society. Advanced age correlates with the onset of frailty. In this vulnerable state, the immune response is weakened and a higher susceptibility to infectious diseases is observed. The present narrative review aims to cover the topic of herpes zoster (HZ) and its complications in frail populations. The lifetime risk of developing HZ is estimated at about 20-30%, and the risk increases with age. In older people, HZ can lead to the inability to recover the lifestyle, the interests, and the level of activity that

Inferring frail life expectancies in Chicago from daily fluctuations in elderly mortality.

Susceptible sub-populations with existing disease have exhibited stronger relationships between air quality and mortality in time-series studies, but their associated life expectancies have largely been overlooked. Murray and Nelson developed a new time-series model that estimated a small unobserved (frail) sub-population and their resulting life expectancies in Philadelphia, including environment relationships. As a further example in a different geographic area, we used this model with 1987-2000 daily mortality data in Chicago and found a stable frail population at risk of ∼900 persons with

Inverted cup prosthesis to treat recurrent hip dislocation in frail elderly patients.

Recurrent hip dislocation has become an increasing problem in patients who had multiple component revision in total hip arthroplasty. These are often frail elderly patients with loss of constraining soft tissue envelope around the hip and an excessive weakness of the abductor mechanism. We present a simple technique for open reduction of a dislocated hip using a custom-made fully constrained construct with potential for preserving the well fixed femoral implant and parts of the acetabular component. The custom-made constrained construct consists of the application of an inverted cup over the f

Exercise rehabilitation in frail elderly patients on maintenance hemodialysis: a best evidence summary.

PURPOSE: Exercise rehabilitation is the preferred strategy for treating frailty in elderly patients on maintenance hemodialysis (MHD). However, the relevant contents are scattered. This study summarizes the best evidence for exercise rehabilitation in frail elderly MHD patients, to provide high-quality evidence-based references for clinical practice. METHODS: By following the "6S" pyramid model, top-down searches were conducted in computerized decision support systems, international guideline websites, websites of chronic kidney disease professional associations, and various databases, with a

Orthostatic Hypotension and Mortality in Elderly Frail Patients: A Retrospective Cross-Sectional Study.

Orthostatic hypotension (OH) is a common problem in the elderly age group, and some studies have reported an association between OH and increased mortality. We evaluated possible associations between OH and mortality in a retrospective study of frail elderly patients who came for a comprehensive geriatric assessment. The study included all patients ≥ 65 years who were assessed in the outpatient Comprehensive Geriatric Assessment Unit. Data were collected from the computerized medical record, including blood pressure, sociodemographic data, lifestyle, falls, pulse rate, body mass index, functio

Colorectal cancer surgery in elderly and frail patients: Should we leave someone behind?

BACKGROUND: Frailty is a stronger determinant of surgical outcomes in colorectal cancer (CRC) than chronological age. Yet frail patients are often excluded from studies, and their long-term outcomes remain poorly defined. Most research dichotomizes patients as frail or non-frail, leaving the gradation of frailty (mild, moderate, severe) underexplored. METHODS: Prospective, single-center study (2017-2023) including 1028 consecutive CRC patients evaluated for curative treatment. Frailty was screened with PRISMA-7 and stratified using the IF-VIG index. Allocation to surgery or non-operative care

Therapeutic interventions for frail elderly patients: part II. Ongoing and unpublished randomized trials.

There is increasing momentum to measure frailty in clinical practice given its proven value as a predictor of outcomes, particularly in elderly patients with cardiovascular disease. The number of randomized clinical trials targeting frail older adults has been modest to date. Therefore, we systematically searched the ClinicalTrials.gov registry in order to review the frailty intervention trials that had been actively initiated or completed but not yet published. The interventions studied were exercise training in 2 trials, nutritional supplementation in 3 trials, combined exercise plus nutriti

A Case of Pyometra Caused by Achromobacter xylosoxidans and γ-Streptococcus in an Elderly Frail Woman.

Pyometra is an easily overlooked disease with nonspecific symptoms; however, a delayed diagnosis can lead to severe complications. An 80-year-old frail woman presented to our hospital with a chief complaint of persistent fever for 10 days. Her blood tests showed an elevated inflammatory response, and computed tomography showed a 10-cm cystic lesion in the pelvic floor compressing the bladder. A catheter was inserted from the vagina into the uterine cavity, resulting in pus drainage and pyometra diagnosis. A pus culture was subsequently performed, which detected Achromobacter xylosoxidans, a co
