Reconstruction of soft tissue defect of lower extremity with free SCIA/SIEA flap.

Free tissue transfer is the most important means of soft tissue reconstruction for the lower extremity. However, cosmetic results and donor site morbidity are only of secondary concern of lower extremity reconstruction. In our efforts to obtain the best possible outcome for patients, as our first priority we chose free superficial circumflex inferior artery (SCIA)/superficial inferior epigastric artery (SIEA) flap over other free flap options in the soft tissue reconstruction of lower extremity because of some advantages. Over the past 3 year, 25 patients underwent lower extremity reconstructi

Free flap choice for soft tissue reconstruction of the severely damaged upper extremity.

Reconstruction of complex wounds of the hand associated with severe bone, tendon, nerve and soft-tissue injuries has been a major problem in hand surgery. Early definitive soft-tissue coverage of this kind of extensive wound with well-vascularized tissue is one of the most important stages of reconstruction for salvage of the extremity and restoration of function. Although multiple free flap donor sites have been described for complex upper extremity wounds, the authors think that anterolateral thigh (ALT) and lateral arm (LA) flaps are good choices for soft-tissue reconstruction in the upper

The free thoracoumbilical flap for resurfacing large soft-tissue defects of the lower extremity.

Both cadavers and living patients were studied regarding a method to resolve large skin defects with bone exposure in the leg, with long-distance thrombosis of the anterior tibial vessels or posterior tibial vessels resulting from traumatic lesions. Forty-six casting mold specimens of cadaveric legs were investigated. There were rich communication branches among the anterior tibial artery, posterior tibial artery, and fibular artery in the foot and ankle, which complemented each other well. Twenty-six patients with large skin defects with bone exposure in the proximal or middle segment of the

Free tissue transfer of the rectus abdominis myoperitoneal flap for oral reconstruction in a dog.

A five-month-old intact/male Boxer dog was presented 5-days following bite wound trauma to the maxillary region resulting in an oronasal fistula extending from the maxillary canine teeth to the soft palate. Multiple surgical procedures using local, buccal mucosal flaps failed to repair the oronasal fistula. Free tissue transfer of the rectus abdominis myoperitoneal flap using microvascular surgical techniques was successful in providing soft tissue reconstruction of the hard palate area. Complications of these surgical techniques included muscle contraction and subsequent muzzle distortion. Sm

Reconstruction of soft-tissue defects using serratus anterior adipofascial free flap.

The serratus anterior muscle has been suggested as a versatile and reliable flap for reconstruction of head and neck and extremity injuries. The adipofascial layer overlying the serratus anterior muscle is the anatomic layer, which is supplied by the same branch of thoracodorsal artery. Even though great progress has occurred in the prevention of postoperative adhesion of extremity injuries, the problem has not been completely solved and is still of special importance in complex injuries. Between March of 1995 and February of 1996, seven patients underwent reconstructive operation as a result

[Free flap-plasty in a larger soft tissue defect of the hip and gluteal region].

The purpose of this paper is to show a case in which we felt at the end, that the chosen procedure did not fulfill our expectations and in which an alternative procedure would have been preferable. In a 47 year old woman a deep, healed soft tissue defect between the left trochanter and the iliac crest was built up with a free latissimus dorsi musculocutaneous flap. The original plan was to suture the vessels to the gluteal vessels and the nerve to the gluteal nerve. The muscle would have been placed in such a way that the muscle fibres would run from the iliac crest to the trochanter to replac

Comparison of Bony and Soft Tissue Free Flaps in the Management of Osteoradionecrosis of the Mandible and Maxilla.

BACKGROUND/OBJECTIVE: Advanced (Notani III) osteoradionecrosis (ORN) often requires microvascular free-flap (MVFF) reconstruction, yet comparative outcomes by flap category and the effect of peri-oncologic factors on postoperative events remain uncertain. We compared outcomes after bony versus soft-tissue MVFF and evaluated potential risk factors for reconstructive failure (RF) and acute and late complications. METHODS: This single-center retrospective cohort study included adults with Notani III ORN who underwent MVFF reconstruction at a tertiary hospital between 2008 and 2024. Twenty-three

Reconstruction of Finger Soft Tissue Defects With a Thinned Free Anterolateral Thigh Flap.

BACKGROUND: Reconstructing complex soft tissue defects of the finger requires both functional and aesthetic aspects. There are many free tissue transfers as suitable options for digital resurfacing. The anterolateral thigh (ALT) flap is not the first choice for reconstruction of this area because it has the disadvantage that the subcutaneous tissue is too thick; however, its use as a thinned form has yet to be described much. This study presents the flap thinning technique in the small-sized ALT flap for the finger soft tissue defect. METHOD: From May 2009 to March 2018, 10 patients with comp

Intraoperative hypotension and flap loss in free tissue transfer surgery of the head and neck.

BACKGROUND: In free flap head and neck reconstructions, hemodynamic management is complicated by the deleterious effects of excessive crystalloid administration. Patients may undergo periods of hypotension or excess fluid administration. The purpose of this study was to present our examination of the hypotheses that intraoperative hypotension and blood pressure lability are associated with increased fluid administration and flap failure. METHODS: We reviewed the records of 445 patients undergoing head and neck surgery involving free tissue transfer. We used multivariate logistic regression to

Free-flap resurfacing of tissue defects in the foot due to large gouty tophi.

BACKGROUND: Large or extensive gouty tophi on the feet can cause functional impairment, drainage sinus, and infected necrosis, finally resulting in complex soft-tissue defects with tendon, joint, bone, nerve, and vessel exposure. Reconstruction of complex soft-tissue defects of the foot is still challenging. The purpose of this report was to review the outcomes of free-flap reconstructive surgery for treating the metatarsal joint defects of the feet caused by chronic tophaceous gout. METHODS: Ten patients who had large tophus masses (>5 cm) and ulceration on the feet were admitted to our hosp

Free-flap salvage of soft tissue complications following the lateral approach to the calcaneus.

Soft tissue complications including wound slough and/or infection after lateral foot incisions used to approach the calcaneus are notoriously frequent. Simpler wound management may be possible when early, aggressive treatment is deployed. If definitive coverage has been delayed, local options typically become unreliable. Free tissue transfers may then become the mainstay for limb salvage. Over the past 2 decades in our joint experience in managing such complications referred to us, 8 free flaps were necessary in 7 patients who all had had some form of traumatic injury to the calcaneus. The mea

Versatility of rectus abdominis free flap for reconstruction of soft-tissue defects in extremities.

Rectus abdominis flaps, whose blood supply is mainly provided by superior and deep inferior epigastric vessels, are suitable not only for local transfer but also as free flaps. Based on abundant anastomoses of deep inferior epigastric vessels with other vessels such as superior epigastric vessels, lower intercostal vessels, subcostal vessels, lumbar vessels, superficial epigastric vessels, and superficial and deep iliac circumflex vessels, the rectus abdominis flap may be designed as a vertical flap, transverse flap, or oblique flap. From September 1995-October 2002, 42 free rectus abdominis f

Free tissue transfer versus pedicled flap cost in head and neck cancer.

OBJECTIVE: We sought to compare the overall 1-year management costs for patients receiving a free tissue transfer with those of patients receiving a pedicled flap reconstruction as a component of their primary head and neck cancer treatment. STUDY DESIGN AND SETTING: Case-control, cost identification analysis of 21 matched pairs of patients and multivariate analysis of variables associated with treatment costs was conducted in a tertiary referral academic institution. RESULTS: No significant difference in total 1-year charges between the pedicled and free tissue transfer groups was found. A

Lower limb revascularization and free flap transfer for major ischemic tissue loss.

The objective of this study was to assess the value of lower limb revascularization and free flap transfer (LLR-FFT) in the management of critical leg ischemia and major tissue loss. A total of 29 consecutive patients with critically ischemic leg and major tissue loss underwent 24 simultaneous and 6 staged LLR-FFT procedures. The main outcome measures were bypass graft patency, free flap viability, leg salvage, patients alive with salvaged leg, and survival. At the 2-year follow-up, the bypass graft patency rate was 85%, secondary free flap viability rate was 82%, and 82% of patients achieved
