The assessment of the internal rotation gait in cerebral palsy: an electromyographic gait analysis.

A study of 12 cerebral palsied children with internal rotation revealed three patterns of electromyographic activity: (1) Diagnostic pattern--where a simple muscle group stood out as the responsible agent--notably the medial hamstrings; (2) Nondiagnostic pattern--nonrecurring pattern; (3) Nondiagnostic pattern--recurring "mass limb reflex" pattern. In all cases, electromyography was useful for: (1) confirmation of clinical impressions. Electromyographic confirmation of phasic hamstring overactivity gives a firm basis for tendon surgery with expectancy of good results. (2) Detection of the resp

Preoperative gait pattern as predictor of gait changes following selective dorsal rhizotomy.

BACKGROUND: Selective dorsal rhizotomy (SDR) can improve gait in children with spastic paresis. However, outcomes vary, and selecting individuals who will benefit from the procedure remains challenging. How pre-SDR gait pattern predicts post-SDR gait has not previously been investigated. RESEARCH QUESTION: This study aims to compare how different gait patterns change after SDR to gain insight into the potential relevance of gait patterns as a selection criterion for SDR. METHODS: Kinematic and kinetic data of nineteen children with bilateral spastic paresis due to cerebral palsy or hereditar

Gait event detection accuracy: Effects of amputee gait pattern, terrain and algorithm.

Several kinematic-based algorithms have shown accuracy for gait event detection in unimpaired and pathological gait. However, their validation in subjects with lower limb amputation while walking on different terrains is still limited. The aim of this study was to evaluate the accuracy of three kinematic-based algorithms: Coordinate-Based Algorithm (CBA), Velocity-Based Algorithm (VBA) and High-Pass Filtered Algorithms (HPA) for detection of gait events in subjects with unilateral transtibial amputation walking on different terrains. Twelve subjects with unilateral transtibial amputation, usin

Pathological gait in Rett syndrome: Quantitative evaluation using three-dimensional gait analysis.

OBJECTIVES: Ataxic-rigid gait is a characteristic gait pathology in patients with Rett syndrome (RTT). In the present study, we aimed to quantitatively evaluate gait pathology in patients with RTT using three-dimensional gait analysis (3DGA). METHODS: We performed 3DGA in 11 patients with RTT ranging from 5 to 18 years (median age, 9 years) and in 33 age-matched healthy female controls. We compared the results of 3DGA, including spatiotemporal gait parameters and comprehensive indices of gait kinematics, such as the Gait Deviation Index (GDI) and Gait Profile Score (GPS), between the two grou

Classification of Ataxic Gait.

Gait disorders accompany a number of neurological and musculoskeletal disorders that significantly reduce the quality of life. Motion sensors enable high-quality modelling of gait stereotypes. However, they produce large volumes of data, the evaluation of which is a challenge. In this publication, we compare different data reduction methods and classification of reduced data for use in clinical practice. The best accuracy achieved between a group of healthy individuals and patients with ataxic gait extracted from the records of 43 participants (23 ataxic, 20 healthy), forming 418 segments of s

Brainstem lesions and gait.

The brainstem contains virtually all of the important structures involved in experimental models of locomotion, encompassing control of upright posture, balance, and stepping. The physiologic basis for these functions is intricately related. Studies of the effects of lesions and disease on these functions in humans are limited to clinical observation and hampered by the anatomic complexity of closely spaced structures and lack of selectivity of lesions. Accordingly, any description of the clinical effects of brainstem lesions on gait and posture is imprecise because weakness and ataxia either

Contactless abnormal gait detection.

We present a new method to detect abnormal gait based on the symmetry verification of the two-leg movement. Unlike other methods requiring special motion captors, the proposed method uses image processing techniques to correctly track leg movement. Our method first divides each leg into upper and lower parts using anatomical knowledge. Then each part is characterised by two straight lines approximating its two borders. Finally, leg movement is represented by the angle evolution of these lines. In this process, we propose a new line approximation algorithm which is robust to the outliers caused

Progressive ataxic gait disorder.

Paraneoplastic cerebellar degeneration (PCD) is a well-described paraneoplastic syndrome. In patients with anti-Yo associated PCD, neurological symptoms precede the diagnosis of the underlying cancer in approximately 60% of cases. Ovarian, breast and other gynaecological malignancies are most frequently found as causative malignancies. Antitumour treatment should be commenced at an early stage of the disease. Identification of the tumour is a diagnostic challenge in many of these patients. In the case reported herein, a secondary tumour with unknown primary was suggested after detection of a p

Primary progressive freezing gait.

Freezing gait is an incapacitating symptom often observed in patients with Parkinson's disease. It has been less frequently described in association with multi-infarct state, multisystem atrophies, and normotensive hydrocephalus. In our movement disorder clinic, we have diagnosed (and followed up to 3 years; median, 16 months), 18 patients in whom progressive freezing gait was the sole neurological dysfunction. These 15 men and 3 women (aged 60-82 years; 74 +/- 6) were subjected to an extensive neurological workup that included clinical evaluation, videotaping for grading of gait disability, c

Clinimetrics of freezing of gait.

The clinical assessment of freezing of gait (FOG) provides great challenges. Patients often do not realize what FOG really is. Assessing FOG is further complicated by the episodic, unpredictable, and variable presentation, as well as the complex relationship with medication. Here, we provide some practical recommendations for a standardized clinical approach. During history taking, presence of FOG is best ascertained by asking about the characteristic feeling of "being glued to the floor." Detection of FOG is greatly facilitated by demonstrating what FOG actually looks like, not only to the pa

Characteristics of gait in hemiplegia.

The following review examines the walking patterns of patients who have hemiplegia, primarily as a result of a stroke. Attention is given to the changes in the distance and temporal factors of walking, phasic patterns and joint ranges of motion throughout the walking cycle, the ground reaction forces, joint moments of force, joint powers, energy expenditure, and muscle activation patterns. The effect of orthotic intervention on these walking parameters is also addressed. A frequently cited issue regarding the gait patterns of these patients was that their walking patterns exhibit significant d

The syndrome of senile gait.

Infrared computed stroboscopic photometry was used to quantify the kinematic profiles of walking in 10 elderly patients with symmetrical neurological disturbances of gait and in 19 age-matched neurologically healthy people. Clinical examination of the patients revealed similar profiles of walking even though their diagnoses were vascular dementia (2), normal pressure hydrocephalus (2), Alzheimer dementia with possible normal pressure hydrocephalus (2), mixed Alzheimer and vascular dementia (1), peripheral neuropathy (1), Alzheimer dementia with parkinsonian features (1), and undetermined (1).

In-toeing gait in children.

Torsional deformities in the lower extremities, resulting from constrained intrauterine position, tend to spontaneously correct toward the adult configuration during growth. Internal tibial torsion, metatarsus adductus and femoral anteversion are clinically evident conditions that occur because of arrested normal postnatal development. An understanding of the dynamics of spontaneous correction permits assessment of the child with in-toeing gait as well as the selection of proper treatment.

Gait and balance disorders.

This chapter focuses on one of the most common types of neurologic disorders: altered walking. Walking impairment often reflects disease of the neurologic structures mediating gait, balance or, most often, both. These structures are distributed along the neuraxis. For this reason, this chapter is introduced by a brief description of the neurobiologic underpinning of walking, stressing information that is critical for imaging, namely, the anatomic representation of gait and balance mechanisms. This background is essential not only in order to direct the relevant imaging tools to the regions mor

[Gait disturbances in neurology].

Gait disorders lead to loss of mobility and therefore of an individual's autonomy. The greatest risk factor for gait disorders is old age. Other important factors include drugs and their side effects, cognitive impairment, and alcohol abuse. In successful aging there is only a slight decline in gait velocity. The classification into lower- (peripheral), middle- (spinal, basal ganglia), and higher-level gait disorders (e.g. frontal or psychogenic) is still common but contradictory. Gait disorders in neurology are frequent. Detailed descriptions of gait patterns in hypokinetic gait disorders, dy

[Gait disorders in the elderly].

Falls are frequent in the elderly and are responsible for an increased morbi-mortality. The elderly who has a tendency to fall is at higher risk of repeated hospitalisation and of institutionalisation. A link between cognitive and gait disorders is put forward by several recent studies. This paper is concerned with the various gait abnormalities encountered in the elderly, especially seriale gait, and the means to detect them.
