We describe here a case of 36-year-old man with a hepatic cavernous hemangioma that was misdiagnosed as a gastric submucosal tumor (SMT) with endoscopic ultrasound (EUS) and CT scan. On gastroscopy, a submucosal tumor was found on the cardia of the stomach. Based on EUS and abdominal CT scan, the lesion was diagnosed as a gastric duplication cyst. The patient underwent gastroscopy after receiving laparotomy at the median point of the abdomen. A spherical mass arising from the left triangular ligament of the liver region was found. There was no abnormal finding at the cardia of the stomach.

Gastroscopy is the gold standard for investigating upper gastrointestinal complaints. Open access gastroscopy has advantages, but may not always be feasible in a small unit. Our 2-consultant general surgical unit provides gastroscopy on demand for medical and surgical patients. We audited the 499 gastroscopies carried out in our unit in 1997 to assess yield and diagnostic trends. A total of 124 subjects who were undergoing routine endoscopy were randomly assigned to one of three groups. All three groups received lignocaine throat spray. Two hundred and eight patients underwent oesophago-gastro-duodenoscopy within the first year of its introduction at a peripheral general hospital in Malaysia. There were 7 cases of gastric cancer, proved histologically, as compared to 38 cases of benign gastric ulcer. The frequencies of the common symptoms and signs in the two conditions are compared. Of the 3 main ethnic groups, Malays, Chinese and Indians, the incidence of gastric ulcer was highest among the Chinese, next the Indians; however, the number of gastric cancers were too few for analysis of its racial incidence.

In the first 3 yr of an uncensored open access gastroscopy service in a County Hospital, 891 patients attended for first gastroscopy.
