Transgender women's perspectives on mental health care related to vaginoplasty for gender affirmation.

PURPOSE: This study aimed to describe patient experiences and attitudes about the role of the mental health professional as it relates to pursuing gender affirmation surgery. METHODS: This was a mixed-models study with semi-structured interviews. Participants who presented for gender affirming vaginoplasty and had completed pre-surgical requirements but had not yet had the procedure were invited to participate in the study. Semi-structured phone interviews were conducted from November 2019 and December 2020 until saturation of themes was achieved at a sample size of 14. Interviews were then t

Providing Affirmative Care to Transgender and Gender Diverse Youth: Disparities, Interventions, and Outcomes.

PURPOSE OF REVIEW: This paper reviews the evolving body of research on the mental health of transgender and gender diverse (TGD) youth. Minority stress experiences in families, schools, and the community impact the health and well-being of this population due to experiences of stigma, discrimination, and rejection. Poor healthcare access and outcomes may be compounded in youth with intersectional identities. RECENT FINDINGS: There is increasing evidence that gender-affirming interventions improve mental health outcomes for TGD youth. TGD youth report worse mental health outcomes in invalidati

Characterizing ovarian histopathology in transgender and gender-diverse patients on testosterone: Informing shared decision-making in gender-affirming care.

BACKGROUND: Gender-affirming hysterectomies (GAH) are increasingly performed among transgender and gender-diverse (TGD) individuals, many of whom utilize testosterone. The World Professional Association for Transgender Health (WPATH) guidelines regarding oophorectomy at the time of hysterectomy defer to shared decision-making, as limited data exists on the effects of exogenous testosterone on an individual's ovaries. This study aims to describe the characteristics of the ovarian histopathology exemplified in this cohort and contribute to the growing gender-affirming care database that guides s

Implementation of Patient-Reported Outcome Measures for Gender-Affirming Care Worldwide: A Systematic Review.

IMPORTANCE: Gender-affirming care is a key clinical area that can benefit from implementation of patient-reported outcome measures (PROMs). Identifying barriers to and enablers of PROM implementation is needed to develop an evidence-based implementation strategy. OBJECTIVE: To identify (1) PROMs previously implemented for gender-affirming care and constructs measured, (2) how patients completed PROMs and how results were reported and used, and (3) barriers to and enablers of PROM implementation. EVIDENCE REVIEW: In this systematic review, PubMed, Embase, MEDLINE, PsycINFO, CINAHL, and Web of

Barriers to Evidence-Based Transgender Care: Knowledge Gaps in Gender-Affirming Hysterectomy and Oophorectomy.

Reversal of gender-discriminatory insurance coverage policies has led to a substantial increase in access to gender-affirming surgical care in the United States over the past 20 years. Although the evidence supports the safety, feasibility, and medical necessity of gender-affirming hysterectomy and oophorectomy, there are currently no evidence-based guidelines to define optimal care surrounding many aspects of these surgeries. This commentary reviews the evidence supporting the safety, feasibility, and route of hysterectomy and oophorectomy for transgender men, the benefits and risks of oophor

It Might Take Time: A Study on the Evolution of Quality of Life in Individuals With Gender Incongruence During Gender-Affirming Care.

INTRODUCTION: The distress caused by gender incongruence has been shown to be reduced with gender-affirming care. The current study follows a cohort of patients that requested gender-affirming care at Ghent University Hospital and examines the outcome of the procedures. METHODS: The prospective design with 4 assessment times allows the study to assess the evolution of self-reported quality of life (QoL) and gender incongruence throughout a medical transition process. All the participants (N = 119) received hormone therapy, 52% underwent a separate gonadectomy (N = 62), and 41% vaginoplasty or

Easing the burden: exploring the role of long-acting testosterone formulations in gender-affirming care.

INTRODUCTION: Long-acting testosterone formulations are essential but infrequently used for gender affirmation. Although not FDA approved in the United States for gender dysphoria, testosterone undecanoate is a long-acting testosterone formulation administered every 10 weeks. Similarly, Testopel is an FDA-approved testosterone pellet implanted subcutaneously every three to six months, which slowly releases testosterone for a long-acting androgenic effect. METHODS: A retrospective review was conducted between January 2020 and June 2024. Data was collected through the electronic health record u

Voices from a Multidisciplinary Healthcare Center: Understanding Barriers in Gender-Affirming Care-A Qualitative Exploration.

When seeking gender-affirming care, trans* and gender-diverse individuals often describe experiencing barriers. However, a deeper understanding of what constitutes such barriers is generally lacking. The present research sought to better understand the barriers trans* and gender-diverse individuals experienced, and their effects, when seeking gender-affirming care in the Netherlands. Qualitative interviews were conducted with trans* and gender-diverse individuals who sought care at a Dutch multidisciplinary medical center. Twenty-one participants were included, of which 12 identified as (trans

The Political and Legal Landscape for Gender-Affirming Care and Transgender Health: Implications for Nursing Practice.

Gender-affirming care (GAC) refers to patient care in any setting or for any purpose that affirms a patient's gender identity. Policies impacting transgender, nonbinary, and gender diverse people's access to GAC include those that impose patient age requirements, protect patients from insurance discrimination, determine what care is covered in public and private insurance, force schools to out transgender students, and allow providers to conscientiously object to treating transgender patients. In an era of strengthening policy restrictions on GAC and anti-transgender sentiment, nurses need to

Assessing and Addressing the Risk of Venous Thromboembolism Across the Spectrum of Gender Affirming Care: A Review.

OBJECTIVE: Accumulating evidence demonstrates that gender affirming hormone therapy (GAHT) improves mental health outcomes in transgender persons. Data specific to the risks associated with GAHT for transgender persons continue to emerge, allowing for improvements in understanding, predicting, and mitigating adverse outcomes while informing discussion about desired effects. Of particular concern is the risk of venous thromboembolism (VTE) in the context of both longitudinal GAHT and the perioperative setting. Combining what is known about the risk of VTE in cisgender individuals on hormone the

"These Laws Will Be Devastating": Provider Perspectives on Legislation Banning Gender-Affirming Care for Transgender Adolescents.

PURPOSE: The standards of care for transgender and gender diverse youth (TGDY) experiencing gender dysphoria are well-established and include gender-affirming medical interventions. As of July 2021, 22 states have introduced or passed legislation that bans the provision of gender-affirming medical care to anyone under the age of 18 even with parent or guardian consent. The purpose of this study is to understand what providers who deliver gender-affirming medical care to TGDY think about this legislation. METHODS: In March 2021, we recruited participants via listservs known to be frequented by

Considerations for Gender-Affirming Hormonal and Surgical Care Among Transgender and Gender Diverse Adolescents and Adults With Congenital Heart Disease.

BACKGROUND: Transgender and gender diverse (TGD) individuals and long-term survivors with adult congenital heart disease (ACHD) are both growing populations with specialized needs. No studies assess temporal trends or evaluate the care of TGD individuals with ACHD. METHODS AND RESULTS: Meetings between congenital cardiology and gender-affirming care specialists identified unique considerations in TGD individuals with ACHD. A retrospective chart review was then performed to describe patient factors and outpatient trends in those with an ACHD diagnosis undergoing gender-affirming hormonal or su

The gender affirmative lifespan approach (GALA): A framework for competent clinical care with nonbinary clients.

Background: The limited research on nonbinary individuals suggests that this community experiences significant health disparities. Compared to binary transgender individuals, research suggests that nonbinary individuals are at elevated risk for discrimination and negative mental health outcomes, including depression, anxiety, traumatic stress, and suicidality. Even mental health providers who work with binary transgender individuals often lack knowledge of and training to work competently with nonbinary individuals. Methods: The authors of this conceptual article present the Gender Affirmative

Management of acne in transgender and gender diverse youth part 1: Gender affirming care and risk factors for the development of acne.

Acne vulgaris is among the most common skin disorders afflicting adolescents worldwide, and though well-established guidelines of care exist for acne management, these guidelines do not uniformly consider or address the unique psychosocial and medical needs of transgender and gender diverse (TGD) youth. TGD youth may possess distinct goals of therapy when treating their acne; the use of medicines routinely employed to treat acne may also expose TGD adolescents receiving gender affirming medical therapy to greater risk of adverse events. Part 1 of this two-part review provides dermatologists an

Gender-Affirming Care of Transgender Patients in Primary Care: An Asynchronous, Online Case-Based Curriculum for Internal Medicine Residents.

OBJECTIVES: Transgender and gender-diverse (TGD) individuals represent a growing demographic in the United States. This population, however, faces many health inequities, in part attributable to a lack of education for physicians in training on transgender health care at both the graduate and undergraduate medical education levels. We developed a curriculum on gender-affirming care for Internal Medicine (IM) residents to fill this educational gap. METHODS: We created a virtual, asynchronous, case-based curriculum on gender-affirming care for IM residents at a large urban academic medical cent

Contraceptive use in patients with gender dysphoria who were assigned female at birth receiving care at a specialty gender-affirming clinic.

BACKGROUND: The Centers for Disease Control and Prevention (CDC) recommend that all patients, regardless of gender identity or sexual orientation, should be offered family planning and contraceptive options without assumptions of sexual behaviors and pregnancy risk. Current research on family planning services for lesbian, gay, bisexual, transgender, and queer or questioning patients is limited, but patients who are sexual or gender minorities are at increased risk for unintended pregnancy. OBJECTIVES: The objective of this study was to describe contraceptive use in patients assigned female a

Bathroom bills and gender-affirming care bans: Understanding people's attitudes toward different types of transgender policies.

Policies restricting transgender rights have proliferated across the United States in recent years, yet little research has examined attitudes toward different types of policies affecting transgender people or the psychological mechanisms underlying these attitudes. Across four studies (N = 1,148), we fill this gap. Factor analyses based on attitudes toward a range of hypothetical and real-world transgender-related policies were used to group policies into four categories, which we label "attitude-based policy types." These categories were policies related to (a) discrimination, (b) physical s
