Chronic liver disease and hepatology care in transgender and gender diverse populations.

Transgender and gender diverse (trans) people experience unique minority stressors related to their gender identity, along with additional psychosocial and systemic stressors that contribute to health disparities. Many risk factors for chronic liver diseases, such as viral hepatitis, alcohol use disorder, and metabolic dysfunction, disproportionately affect trans people. There have also been historical concerns about the hepatic effects of gender-affirming hormone therapy (GAHT) that are predominantly based on data extrapolated from cisgender populations or hormone formulations that are no lon

The importance of LGBTQ+ identity in the relationship between LGBTQ+ microaffirmations and well-being among LGBTQ+ youth of color.

Much of what is known about the psychological and developmental experiences of LGBTQ+ youth of color comes from a deficit-based perspective, seeking to identify what they lack, rather than what assets they hold. The current study takes a strengths-based perspective and examines positive, developmentally relevant assets among LGBTQ+ youth of color and how these assets interact to shape well-being. More specifically, the study assesses the relationship between LGBTQ+ microaffirmations and well-being and the potential moderating role of LGBTQ+ identity in that relationship. In the study, 156 LGBT

The rain feels different under the same umbrella: Experiences with poverty across LGBTQ subgroups.

Population-based survey data have demonstrated that LGBTQ communities report varying rates of economic insecurity, yet very little research directly assesses how pathways into and experiences with poverty look different among subgroups at the intersections of sexual orientation and gender identity (SOGI). This qualitative study applies constructivist grounded theory analysis to examine the lived experiences of 92 low-income LGBTQ adults. While shared experiences were evident, the analysis highlights issues notably salient to specific SOGI subgroups. Distinct themes underscore the following exp

The peripubertal gender-dysphoric child: puberty suppression and treatment paradigms.

Gender-nonconforming youth are emerging at increasingly younger ages, and those experiencing gender dysphoria are seeking medical care at, or sometimes even before, the onset of puberty. Youth with gender dysphoria are at high risk for depression, anxiety, isolation, self-harm, and suicidality at the onset of a puberty that feels wrong. Medical providers would benefit from understanding interventions that help gender-nonconforming children and youth thrive. The use of gonadotropin-releasing hormone (GnRH) agonists to block the onset of an undesired puberty in youth with gender dysphoria is a r

Advocacy in Practice: The Role of Mental Health Providers in Creating Systems of Support for Transgender and Gender Expansive Youth.

BACKGROUND: Transgender and gender expansive (TGE) youth have seen both an increase in access to mental health and medical services in the previous two decades, and also a sharp increase in bills limiting access to care and supports in the past 3 years. OBJECTIVE: Psychologists have a unique role to play in working with TGE youth, as we have been trained in research methodology, clinical practice and human behavior. As such, psychologists can effectively advocate for the best practices when working with TGE youth. RESULTS: In this article, we provide a brief overview of general concerns of T

Affirmation to fulfillment: Therapy experiences of transgender and nonbinary clients over time.

Transgender and nonbinary (TNB) people experience marginalization due to gender minority stigma. Research has demonstrated that many TNB people seek therapy to manage these stressors as well as more general distress. Given psychology's complex history with TNB care, it is of particular importance to facilitate therapy experiences for TNB clients that are fulfilling and affirming. In this longitudinal study, we investigated TNB peoples' perceptions of how their therapists responded to ongoing contextual factors related to systems of power (e.g., current events, political climate) in TNB peoples

Time Management, Stigma, Jealousy, and More: A Content Analysis of the Challenges of Polyamory.

Despite growing public and scientific interest in consensually non-monogamous (CNM) relationships, clinical and empirical understandings of the challenges faced by people who engage in polyamory remain limited. To address this gap, we conducted a content analysis of 763 open-ended responses from 219 individuals currently in a polyamorous relationship who were asked to describe the challenges of their relationship structure. We identified 15 majors theme and several corresponding minor themes. The four most frequently reported challenges of polyamory included: (1) time management issues (72.6%)

Correlates of body dissatisfaction and eating disorder symptoms in transgender and gender diverse populations: A systematic review.

Transgender and gender diverse (TGD) people face disproportionately high risk for body image concerns and disordered eating/eating disorders. However, strong explanatory models that integrate risk and protective factors specific to this population remain limited. This study addressed this gap through a systematic review of literature on body image concerns and disordered eating among TGD people. A systematic search identified 79 studies, with sample sizes ranging from single case studies to 3388 participants. Factors examined were grouped into sociocultural influences, stigma and discriminatio

The Impact of Birth Year on the Coming Out Process for Transgender and Gender Diverse Individuals from Austria.

This study examined age-related cohort differences in the timing of three key gender identity milestones (first desire to transition, coming out, and initiation of hormone therapy) across gender identities. Data from 212 adult transgender and gender diverse (TGD) individuals (54.2% trans male, 42.5% trans female, 3.3% gender diverse; age 20-76 years) were collected at a single center between 2017 and 2021. Gender diverse participants, defined as those with identities outside the binary, were grouped accordingly. Selected items relating to gender identity milestones and sociodemographic informa

Mental Health, Gender Dysphoria, and Quality of Life in Trans Individuals Following Top Surgery: Protocol for a Prospective Cohort Study.

BACKGROUND: Top surgery refers to various gender-affirming surgical procedures involving breast or chest tissue. Existing research suggests top surgery improves physical, psychological, and social well-being in transgender and gender-diverse individuals (herein "trans") who desire it. However, most studies have been retrospective, have not used validated outcome measures, and have excluded nonbinary individuals, limiting the robustness of the existing research. OBJECTIVE: This study aims to examine changes in mental health, gender dysphoria, risky behaviors, and quality of life of trans peopl

Sexual, dating, and bullying-related forms of violence targeting LGBTQ+ youth among a population-based sample of California high school students.

LGBTQ+ youth experience inequitably high risk for bullying, sexual violence (SV), and dating violence (DV), disproportionately disrupting their social, educational, financial, and health opportunities. LGBTQ+ youth may even be targeted for distinct forms of violence based on their LGBTQ+ identity (e.g., unwanted sexual contact to "prove" one's gender or sexual orientation), yet little is known from population-based studies. This study provides prevalences for a range of ways LGBTQ+ youth are specifically targeted for bullying, SV, and DV using data from a population-based sample of 9th and 11t

From concept to protocol: iterative approach of a target trial emulation on GnRH analog safety in transgender and gender diverse youth.

CONTEXT: The scarcity of robust long-term safety data has contributed to increasing restrictions on the use of gonadotropin-releasing hormone analog (GnRHa) treatment in transgender and gender diverse adolescents (TGDAs) in several countries. Methodological limitations of earlier studies have led to controversy and debate, underscoring the urgent need for high-quality evidence in this field. OBJECTIVE: To present a study design using target trial emulation (TTE) to evaluate long-term safety of GnRHa treatment in TGDAs using observational data from national healthcare registries, and highlight

"Not Ideal Uncles": Unveiling Childhood Sexual Victimization and Hidden Identity Struggles of GBTQI+ Youth in Sex Work in Botswana.

OBJECTIVE: Globally, growing attention has been directed toward the sexual health and developmental experiences of gay, bisexual, transgender, queer, and intersex (GBTQI+) youth. Existing literature links childhood sexual victimization, familial rejection, bullying, and gender nonconformity to adverse sexual health outcomes. Evidence further suggests that sexual and gender minority youth are often targeted for abuse before they self-identify based on perceived gender nonconformity; yet, despite these insights, little is known about these dynamics in Botswana. This study applies an intersection
