A qualitative study of gestational weight gain counseling and tracking.

Excessive gestational weight gain (GWG) predicts adverse pregnancy outcomes and later obesity risk for both mother and child. Women who receive GWG advice from their obstetric clinicians are more likely to gain the recommended amount, but many clinicians do not counsel their patients on GWG, pointing to the need for new strategies. Electronic medical records (EMRs) are a useful tool for tracking weight and supporting guideline-concordant care, but their use for care related to GWG has not been evaluated. We performed in-depth interviews with 16 obstetric clinicians from a multi-site group prac…

Gestational weight gain and adverse neonatal outcome among term infants.

OBJECTIVE: To examine the relationship between gestational weight gain and adverse neonatal outcomes among infants born at term (37 weeks or more). METHODS: This was a retrospective cohort study of 20,465 nondiabetic, term, singleton births. We performed univariable and multivariable analyses of the associations between gestational weight gain and neonatal outcomes. We categorized gestational weight gain by the Institute of Medicine guidelines as well as extremes of gestational weight gain (less than 7 kg and more than 18 kg). RESULTS: Gestational weight gain above the Institute of Medicine …

Maternal weight gain patterns and birth weight outcome in twin gestation.

OBJECTIVE: To evaluate the association between maternal weight gain patterns, based on pregravid body mass index (BMI) and birth weight outcome in twins, and to make specific recommendations for maternal weight gain during twin gestation. METHODS: One hundred eighty-nine twin pregnancies were reviewed retrospectively. Weekly rates of maternal weight gain before 20 weeks, from 20 weeks to delivery, and for total gestation were calculated. Thresholds of weekly maternal weight gain were determined for underweight and normal-weight women. RESULTS: In underweight women, a higher weekly rate of ga

Placental Gene Expression in Women with Excessive Gestational Weight Gain.

Excessive gestational weight gain (EGWG) promotes adverse physiological and molecular changes in the placenta. In the present study, we examined the whole placental transcriptomic profile in women with EGWG to elucidate the molecular basis of EGWG pathogenesis. Whole transcriptome profile was analyzed in placentas from term patients with EGWG compared to controls using RNA-seq. Eight genes were found to be downregulated, and 318 genes were found to be upregulated in the placentas of the EGWG group in comparison with those of the women with adequate gestational weight gain (AGWG). Analysis of d…

Gestational Weight Gain Following Metabolic Bariatric Surgery: A Scoping Review.

Metabolic bariatric surgery remains the most effective and durable treatment for severe obesity. Women of reproductive age represent the largest demographic group undergoing these procedures. Metabolic bariatric surgery can have both beneficial and adverse effects on pregnancy outcomes. One of the most common adverse effects is fetal growth restriction. To mitigate these adverse effects, it is crucial to explore lifestyle modifications aimed at promoting a healthy pregnancy. Modifiable factors during pregnancy after metabolic bariatric surgery include the amount of gestational weight gain. The…

Socioeconomic and clinical factors associated with excessive gestational weight gain.

PURPOSE: Excessive gestational weight gain (EGWG) is associated with adverse maternal and offspring outcomes but efforts to identify women at high risk for EGWG have been limited. The objective of this study is to identify socioeconomic and clinical factors associated with EGWG. METHODS: This retrospective cohort included pregnant patients who delivered live, term, singleton newborns between January 2018 and February 2020 at seven hospitals within a large health system in New York. Patients were stratified by pre-pregnancy body mass index and then classified based on whether they exceeded the…

Excessive gestational weight gain is associated with severe maternal morbidity.

PURPOSE: We determined the association between gestational weight gain and severe maternal morbidity. METHODS: We used data on 84,241 delivery hospitalizations at Magee-Womens Hospital, Pittsburgh, PA (2003-2012). Total gestational weight gain (kilogram) was converted to gestational age-standardized z-scores. We defined severe maternal morbidity as having ≥1 of the 21 Centers for Disease Control diagnosis or procedure codes for severe maternal morbidity identification, intensive care unit admission, or extended postpartum stay. We used multivariable logistic regression to determine the associ…

Gestational weight gain and newborn anthropometric outcomes in rural Bangladesh.

Low gestational weight gain (GWG) is a known predictor of fetal growth restriction in higher income countries, but there is little information on this association in lower income countries. Our objective is to describe the association between GWG and birth outcomes among pregnant women in rural Bangladesh. Pregnant women were identified in a community-based programme and enrolled into the study at an average of 13 weeks' gestation (n = 4,011). Maternal weight and height were measured at enrolment, maternal weight was measured at 36 weeks' gestation, and newborns were measured after birth. Rate…

Gestational Weight Gain and Peripartum Cardiomyopathy in a Twin Pregnancy.

Preeclamptic twin pregnancy with larger gestational weight gain (GWG) is suggested to have a higher risk of peripartum cardiomyopathy (PPCM). This was true in a 5-year experience at a single center. A primiparous woman with twins and prepregnancy weight of 51.0 kg exhibited hypertension at gestational week (GW) 32(-6/7) and GWG of 18.3 kg (6.0 kg and 2.9 kg during the last four weeks and one week of gestation, resp.) concomitant with generalized edema, gave birth at GW 34(-4/7), developed proteinuria, cough, and dyspnea postpartum, and was diagnosed with preeclampsia and PPCM showing left vent…

Misreport of gestational weight gain (GWG) in birth certificate data.

Birth certificates are potentially a valuable source of information for studying gestational weight gain (GWG) during pregnancy, particularly important given new Institute of Medicine (IOM) guidelines. We examined factors associated with the accuracy of maternal GWG self-report by linking the gold standard obstetric electronic medical record (EMR) of women from a large urban practice to state birth certificates. Primary outcomes included maternal under-reporting of GWG (>10 lbs below the EMR), accurate reporting (within 10 lbs), and over-reporting (>10 lbs above EMR). Data were stratified acro…

Protocol for the development of the WHO gestational weight gain charts.

INTRODUCTION: Gestational weight gain (GWG) is an important indicator of maternal nutrition to be monitored during pregnancy. However, there is no evidence-based tool that can be used to monitor it across all geographic locations and pre-pregnancy body mass index (BMI) categories. The WHO is undertaking a project to develop GWG charts by pre-pregnancy BMI category, and to identify GWG ranges associated with the lowest risks of adverse maternal and infant outcomes. This protocol describes all the steps that will be used to accomplish the development of these GWG charts. METHODS AND ANALYSIS: T…
