Excessive gingival bleeding in two patients with Glanzmann thrombasthenia.

BACKGROUND: Glanzmann thrombasthenia (GT) is an exceedingly rare but well-defined inherited disorder of platelet function caused by a defect in the glycoprotein IIb/IIIa complex. The association of GT with consanguinity has been noted, especially in geographic regions in which intermarriage is common. In most patients, GT is diagnosed during early infancy or before the age of 5 years. Common manifestations of this disorder are gingival hemorrhage, purpura, epistaxis, petechiae, and menorrhagia. Chronic, prolonged, untreated, or unsuccessfully treated bleeding may be life threatening. METHODS:…

Collagen synthesis in idiopathic and dilantin-induced gingival fibromatosis.

Gingival fibromatosis is an uncommon but benign oral disease which causes progressive and non-hemorrhagic enlargement of the gingiva. There are two types of gingival fibromatosis: idiopathic gingival fibromatosis (GF), which is of unknown cause and may be associated with hereditary factors, and drug-induced gingival fibromatosis which is caused primarily by phenytoin intake. In cases of gingival fibromatosis, either the teeth are delayed in emergence or most of the crowns are embedded in the soft tissue even after full eruption. The objective of this study was to examine the basis of excess co

[Compartment syndrome of the legs and scurvy].

Old people living alone and in poverty are most at risk for developing scurvy, but the diagnosis may be missed unless the physician is aware of it. A 42-year-old immigrant living in poverty was treated surgically at the Hôtel-Dieu Hospital in Montreal for a compartment syndrome of the leg. She had ecchymoses on the skin of the leg and an interstitial hematoma in both posterior compartments. Three weeks later, she was readmitted with more swelling in the leg, gingivitis, hemorrhagic folliculitis, petechiae and gastrointestinal hemorrhage. Her response to vitamin C was remarkable and confirmed t

Noninvasive Imaging Techniques as Modern Diagnostic Tools in Desquamative Gingivitis: Focus on RCM, OCT, and LC-OCT.

BACKGROUND: Desquamative gingivitis is a clinical condition characterized by erythema, blisters, and erosions within the gingiva. It may be a manifestation of oral lichen planus or autoimmune bullous diseases, such as pemphigus vulgaris and mucous membrane pemphigoid. The identification of the cause of desquamative gingivitis typically involves the analysis of clinical manifestations, histopathology, and immunopathology evaluation. This article reviews available data on the application of noninvasive imaging techniques (reflectance confocal microscopy, optical coherence tomography, and line-fi

Plasma Cell Gingivitis: An Occasional Case Report.

Plasma cell gingivitis, an infrequently observed oral condition, has been clinically characterized by diffuse gingival enlargement, erythema and sometimes desquamation. These lesions are usually asymptomatic, but invariably the patient will complain of a burning sensation in the gingiva and bleeding from the mouth. The diagnosis requires hematological screening in addition to clinical and histopathological examinations. This case report outlines one such case of plasma cell gingivitis in a 15-year-old female caused by use of an herbal, homemade toothpowder. The case presented here highlights t

Progressive laser treatment of hereditary hemorrhagic telangiectasia of the gingiva: case report.

Hereditary hemorrhagic telangiectasia (HHT) is an inherited vascular malformation commonly found in the oral region. The symptoms range from minor epistaxis to severe, life-threatening hemorrhage. Red spots on the skin and in the mouth could lead to a diagnosis of HHT. The telangiectases can be irritated by normal dental procedures, and prophylactic antibiotics should be administered. The gingival HHT in this case demonstrates its significance to the dental community. A knowledgeable practitioner could be the first to recognize the potential diagnosis of HHT. Lesion variables will determine tr

Complication After Cryosurgery Treatment for Gingival Melanin Pigmentation: A Case Report.

INTRODUCTION: Gingival melanin pigmentation (GMP) is considered a physiologic or racial situation. Complaints of "black gums" are common, and demands for depigmentation are usually made for esthetic reasons. In this case report, a cryosurgical depigmentation procedure is described and the effectiveness and complications of cryosurgery are examined. CASE PRESENTATION: A 40-year-old, systemically healthy female presented with esthetic problems associated with GMP. Cryosurgery was performed from the midline by cross-segments from canine to canine for the treatment of maxillary and mandibular gin

External cervical root resorption involving multiple maxillary teeth in a patient with hereditary hemorrhagic telangiectasia.

Hereditary hemorrhagic telangiectasia (HHT) is an inherited syndrome characterized by mucocutaneous telangiectases that commonly involve the tongue, lips, fingers, and conjunctiva. While root resorption has been reported in association with central hemangiomas of bone, the association of HHT with external cervical root resorption has not been described to date. We report a case of a 57-year-old female with HTT who presented with advanced cervical root resorption involving multiple maxillary anterior teeth. Histologic examination of the gingival tissue adjacent to the area of root resorption de

Firsthand clinical observations of hemorrhagic manifestations in Ebola hemorrhagic fever in Zaire.

About 5 weeks after the beginning of the outbreak of Ebola virus fever in Yambuku, Zaire, several acute cases of the disease were observed. All of those affected had the following common signs and symptoms: sudden onset of high fever, with chills, headache, myalgia, anorexia, nausea, abdominal pain, sore throat, expressionless face, and profound prostration. In some cases, on around the fifth day of the acute phase, the appearance of an exanthematous rash on the trunk announced the hemorrhagic manifestations: hemorrhagic conjunctivitis, bleeding ulcerations in the mouth and on the lips, gingiv

Evaluation of bleeding on probing and gingival crevicular fluid enzyme activity for detection of periodontally active sites during supportive periodontal therapy.

This study aimed to analyze the enzyme activity in gingival crevicular fluid (GCF) and its association with clinical parameters, especially bleeding on probing (BOP), and thus reconsider the significance and accuracy of recording BOP. A total of 184 patients who had entered supportive periodontal therapy were selected and GCF was collected from 401 sites before recording the clinical parameters, probing pocket depth (PPD), BOP, clinical attachment level, gingival index and plaque index. The enzyme activity of neutrophil elastase and aspartate aminotransferase and amount of protein in GCF were

Pemphigus vulgaris presenting as a gingival lesion. A case report.

A 26-year-old hispanic male presented with a nonspecific gingival lesion initially presumed to be of infectious etiology. During continued follow-up over several weeks, the lesion evolved into one typical of desquamative gingivitis. Direct immunofluorescent testing and routine histopathology resulted in a diagnosis of pemphigus vulgaris, which was confirmed by indirect immunofluorescence. Although it is unusual for pemphigus vulgaris to present with the gingiva as the sole primary site of involvement, this case serves to enhance our awareness of the gingiva as a site at which systemic disease

Thrombocytopenia possibly induced by etretinate in a psoriatic patient.

A patient with psoriasis vulgaris developed multiple purpura on the extremities and hemorrhage at the oral mucosa and gingiva with marked thrombocytopenia. This thrombocytopenia was possibly induced by a retinoid agent, etretinate, from the clinical course and data. The total amount of etretinate administered was 2,410 mg over 191 days (41.9 mg/kg body weight, average 15.3 mg/day). A platelet transfusion partially restored the platelet count and the purpura and gingival hemorrhage disappeared approximately 10 days after the cessation of etretinate. However, the platelet count remains at 60-80 …

Advanced stage of oral myiasis in children: a clinical case report.

Human myiasis is characterized by larval infestation of body tissues or cavities of live individuals. Although most frequently observed in underdeveloped and tropical countries, reports of human myiasis have been encountered throughout the world. Conventional treatment is the mechanical removal of the larvae, one by one, which is painful and embarrassing and repugnant to both health professional and patient. Even though human myiasis is not considered rare, most dental professionals have very little knowledge of the diagnosis and treatment of this pathology. The purpose of this article, theref

Prevalence of plaque, gingivitis and caries in 11-12-year-old children in South Wales.

The overall eruption and dental disease status of a group of 1015 11-12-yr-old schoolchildren in South Wales is presented. The mean number of teeth erupted was 21.8 with a distribution pattern reflecting continuing eruption of teeth at this age. Almost equal numbers of contralateral and opposing tooth types were present with exceptions including maxillary canines and lateral incisors. DMFT, DMFS, and DFS scores were consistent with other recent studies. The mean plaque score was 2.44 and showed a normal distribution for the group. All children had gingivitis at one or more sites as evidenced b
