BACKGROUND: Between 35% to 50% of traumatic brain injury (TBI) patients are under the influence of alcohol. Alcohol intoxication may limit the ability of the Glasgow Coma Scale (GCS) to accurately assess severity of TBI. We hypothesized that alcohol intoxication significantly depresses GCS scores of TBI patients.
METHODS: A 10-year, retrospective analysis of a Level I trauma center registry was undertaken. The study population consisted of all blunt injured TBI patients tested for blood alcohol concentration (BAC, n = 1,075). Patients were divided into two groups; intoxicated (mean BAC 202 +/

This brief report discusses the relationship between verbal function, disorders of consciousness, and neurological follow-up after acute brain injury. It provides valuable insights for improving the accuracy and reliability of Verbal Glasgow Coma Scale scoring in clinical practice. The report addresses the need for standardized training and underlines the importance of physiological stabilization before assessment. Clarity in communication, recognition of non-verbal cues, and serial assessments are emphasized as critical factors to reduce the Verbal Glasgow Coma Scale inconsistencies. It also

OBJECTIVES: To study whether combining age and the Glasgow Coma Scale (GCS) with the shock index (SI) - SIA/G - during the initial care of polytraumatized patients can improve the ability of the SI alone to predict mortality. To compare the predictive performance of the SIA/G combination to other prognostic scales: the addition of points for the GCS, age and systolic blood pressure (GAP); the Revised Trauma Score (RTS); and the Injury Severity Score (ISS).
MATERIAL AND METHODS: Observational cohort study of patients with severe trauma admitted to the intensive care unit of a tertiary care hos

BACKGROUND: Current trauma activation guidelines do not clearly address age as a risk factor when leveling trauma patients. Glasgow coma scale (GCS) and mode of injury play a major role in leveling trauma patients. We studied the above relationship in our elderly patients presenting with traumatic head injury.
METHODS: This study was a retrospective analysis of patients who presented to the emergency department with traumatic brain injuries. We classified the 270 patients into two groups. Group A was 64 years and younger, and group B was 65 years and older. Their GCS, ISS, age, sex, comorbidi

AIM: Hemorrhagic stroke (HS) represents one of the major causes of death in China. The research work focuses on the correlation between Glasgow Coma Scale (GCS) score and D-dimer level, and the role of modified factors such as platelet count in HS patients.
METHODS: This was a cross-sectional study that was conducted at the First People's Hospital of Lianyungang, Lianyungang City, China. HS Patients were divided into three groups according to their GCS scores: GCS < 9 (severe, n = 43), 9 ≤ GCS < 13 (moderate, n = 97), and GCS ≥ 13 (mild, n = 487).
RESULTS: The analysis comprised 627 particip

Traumatic brain injury (TBI) is a leading cause of death and disability. While the Glasgow Coma Scale (GCS) guides initial assessment, single values miss evolving neurological change. In this multicenter ICU cohort integrating NSICU, MIMIC-IV, and eICU databases, we analyzed adults (≥18 years) with TBI who had ≥3 GCS measurements within the first 120 ICU hours. Using 12-hourly measures, latent class growth modeling identified four dynamic GCS trajectories (Stable High, Rapidly Improving, Persistently Moderate, Persistently Low), and we quantified cumulative neurological burden with a mean thre

BACKGROUND: Pretreatment predictors of death from tuberculous meningitis (TBM) are well established, but whether outcome can be predicted more accurately after the start of treatment by updated clinical variables is unknown. Hence, we developed and validated models that dynamically predict mortality using time-updated Glasgow Coma Scale (GCS) and plasma sodium measurements, together with patient baseline characteristics.
METHODS: We included 1048 adults from 4 TBM studies conducted in southern Vietnam from 2004 to 2016. We used a landmarking approach to predict death within 120 days after tre

INTRODUCTION: Up to 70% of early deaths in traumatic brain injuries are due to withdrawal of care. However, many of these patients could have made a good recovery. A reliable prognostic indicator would thus be useful for early decision-making. We sought to determine the association between the Base Deficit, International Normalized Ratio, and Glasgow Coma Scale (GCS) (BIG) score and functional outcomes within 1 y postinjury.
METHODS: A meta-analysis was conducted using the Federal Interagency Traumatic Brain Injury Research Informatics System. Two multicenter prospective studies from 2006 to

BACKGROUND: Multicenter electronic health records (EHRs) can support quality improvement and comparative effectiveness research in critical care. However, limitations of EHR-based research include challenges in abstracting key clinical variables, including a patient's level of consciousness.
OBJECTIVE: This study aimed to develop a natural language processing model to predict Glasgow Coma Scale (GCS) scores from daily EHR notes.
METHODS: The study included adult patients (aged ≥18 years) admitted to Mass General Brigham (MGB) hospitals (2017-2024) and patients from the Medical Information Ma

BACKGROUND: Glasgow Coma Scale (GCS) is fundamental for neurological assessment in traumatic brain injury (TBI) patients. Traditional single-point GCS measurements fail to capture the dynamic nature of brain injury progression, limiting nurses' ability to identify at-risk patients and implement timely interventions.
AIMS: To investigate the dynamic trajectories of the GCS during the first 24 h in ICU and their association with in-hospital mortality in adult TBI patients, providing evidence for nursing neurological assessment and care planning.
RESULTS: Five distinct GCS trajectory groups wer

Background Acute coronary syndrome is the most common cause of mortality; cerebral vascular accident ranks second. Stroke is the fourth most common cause of disability worldwide, with nearly 20 million people suffering a stroke every year around the world and an estimated five million dead. Slightly more than 85.5% of stroke-related deaths take place in developing countries. In short, blockage (thrombus or emboli) and decreased blood supply for cerebral tissues lead to a stroke that permanently damages brain tissue. A stroke is clinically defined as rapidly developing clinical symptoms of foca

BACKGROUND: Assessment of patients with mild traumatic brain injury (mTBI) is predominantly done using the Glasgow Coma Scale (GCS). While the GCS is universally accepted for assessment of severity of traumatic brain injury, it may not be appropriate to rely on the GCS alone when assessing patients with mTBI in prehospital settings and emergency departments.
OBJECTIVE: To determine whether administering the Revised Westmead Post-traumatic Amnesia (PTA) Scale (R-WPTAS) in addition to the GCS would increase diagnostic accuracy in the early identification of cognitive impairment in patients with

Background Altered consciousness is a neurological emergency in pediatrics, with high morbidity and mortality. The Pediatric Glasgow Coma Scale (pGCS) is commonly used to assess altered sensorium in children. The Alert, Verbal, Pain, Unresponsive (AVPU) scale is a simple and easy-to-use alternative assessment tool that records patient response to verbal, painful stimuli, and unresponsiveness. Objective To compare the effectiveness of the Alert, Verbal, Pain, Unresponsive with the pediatric glasgow coma scale in assessing conscious level in infant and children with altered sensorium. Method Thi

IntroductionPrevious studies have demonstrated variability between prehospital (PH) and Emergency Department (ED) Glasgow Coma Scale (GCS) for patients suffering traumatic brain injuries (TBIs). Understanding the relationship between PH and ED GCS, as well as the factors that may contribute to any observed differences, is crucial for optimizing trauma triage protocols and resource allocation.MethodsThe National Trauma Data Bank (NTDB) was surveyed for adults aged ≥18 years following blunt TBI. PH and ED GCS scores were compared, including subgroup analysis of different TBI severities, Injury S
