The natural history of subependymal giant cell astrocytomas in tuberous sclerosis complex: a review.

Tuberous sclerosis complex (TSC) is an auto-somal-dominant inherited condition with an incidence of approximately 1:6000 births, characterised by deregulated mTOR activity with multi-site hamartomas. Subependymal giant cell astrocytomas (SEGA) are one such hamartoma, affecting up to 24% of patients with TSC. Their intraventricular location may lead to life-threatening obstructive hydrocephalus. Current management is hampered by a lack of understanding regarding the natural history, behaviour and growth patterns of SEGA. We review the current literature to summarise what is known about SEGA in

Biology and management of ependymomas.

Ependymomas are rare primary tumors of the central nervous system in children and adults that comprise histologically similar but genetically distinct subgroups. The tumor biology is typically more associated with the site of origin rather than being age-specific. Genetically distinct subgroups have been identified by genomic studies based on locations in classic grade II and III ependymomas. They are supratentorial ependymomas with C11orf95-RELA fusion or YAP1 fusion, infratentorial ependymomas with or without a hypermethylated phenotype (CIMP), and spinal cord ependymomas. Subependymoma with prominent rosenthal fiber formation: case report.

A 62-year-old woman presented with a rare case of subependymoma associated with prominent Rosenthal fibers located in the left lateral ventricle manifesting as right hemiparesis and mild motor aphasia. The tumor was well demarcated and consisted of clusters of round nuclei embedded in an abundant gliofibrillary matrix with some microcysts and prominent Rosenthal fibers. Immunohistochemically, the tumor stained positively for glial fibrillary acidic protein and negatively for synaptophysin. This case of subependymoma containing Rosenthal fiber formation is very unusual.

Traumatic subependymal hematoma during endoscopic third ventriculostomy in a patient with a third ventricle tumor: case report.

Endoscopic third ventriculostomy has become a routine intervention for the treatment of non-communicating hydrocephalus. This technique is largely considered safe and a very low incidence of complications is reported. However, hemorrhage in the course of neuroendoscopy is still a problem difficult to manage. The authors present a case in which endoscopic third ventriculostomy and tumor biopsy were performed in a young patient with a huge tumor growing in the posterior part of the third ventricle. The surgical approach to realize the stoma was difficult because the tumor size reduced the third

Familial occurrence of subependymoma. Report of two cases.

Subependymomas are unusual tumors believed to arise from the bipotential subependymal cell. Previous reports of familial occurrence of subependymoma have involved monozygous twins and siblings. The authors describe the first reported occurrence of fourth ventricular subependymoma in a father and son, suggesting the possibility of direct inheritance.

Lateral ventricle gliomas and central neurocytomas in adults diagnosis and perspectives.

Neuroimaging data of lateral ventricle gliomas and central neurocytomas diagnosed in one institution were reviewed and compared to the corresponding literature data. CT and MRI imaging characteristics of the two tumour types are rather different, both in reported cases as well as in our material. In our series ventricular ependymomas (eight cases) were mostly hyperdense with pronounced contrast uptake. Thirteen subependymomas were hypodense, mostly without enhancement, but occasionally mild or moderate enhancement was noted. Eight subependymal giant cell astrocytomas also displayed hypodense,

Microsurgical Resection of the IV Ventricle Subependymoma: 2-Dimensional Operative Video.

In this video, we demonstrate microsurgical resection of IV ventricle subependymoma. To the best of our knowledge, this is the first video case report of a microsurgical resection of subependymoma of the IV ventricle in the peer-reviewed English literature. Subependymomas are benign central nervous system tumors, typically arising in ventricular spaces, mostly in the IV and lateral ventricles.1-3 They are isointense on T1 and hyperintense on T2-weighted magnetic resonance imaging (MRI) with minimal or no enhancement.4 Microsurgery remains the mainstay treatment. Complete tumor resection is pos

Subependymal glioneuronal hamartoma in the mesencephalic aqueduct of a giraffe.

A 1-day-old male giraffe calf (Giraffa camelopardalis) was submitted for necropsy examination after sustaining postnatal head trauma from the cow. In addition to the expected findings of severe cerebral edema and epidural and subarachnoid hemorrhage, there also was present an incidental finding of a subependymal glioneuronal aqueductal hamartoma. Reports of this type of congenital lesion are rare in the human literature, and the lesion has not, to the authors' knowledge, previously been reported in this or any other veterinary species.

Pilocytic astrocytoma: A rare presentation as intraventricular tumor.

BACKGROUND: Pilocytic astrocytoma (PA) is the most prevalent central nervous system (CNS) tumor in pediatric population and accounts for an approximate of 5-6% of all gliomas. This neoplasm can occur at all levels of the neuraxis, with majority (67%) arising in the cerebellum and optic pathway. PAs are World Health Organization Grade I tumors and are the most benign of all astrocytomas characterized by an excellent prognosis. Other differentials include subependymal giant cell astrocytoma (SEGA), ependymoma, meningioma, and low-grade gliomas such as pilocytic or diffuse astrocytoma; calcificat

Evidence of ambiguous differentiation and mTOR pathway dysregulation in subependymal giant cell astrocytoma.

OBJECTIVE: The exact cell of origin of subependymal giant cell astrocytoma is debated but most currently consider the tumor in the astrocytic category. Mutations and subsequent biallelic inactivation of TSC1 encoding hamartin, or TSC2 encoding tuberin appear to be the underlying genetic aberrations. Inactivation leads to loss of proteins that inhibit mammalian target of rapamycin (mTOR ) disrupting tightly regulated cell functions. MATERIAL AND METHOD: We analyzed the expression of tuberin and hamartin along with an array of neuroepithelial markers in 9 subependymal giant cell astrocytomas. I

[Tumors of the lateral ventricle in child: characteristics and specificities].

Tumors of the lateral ventricle are not as rare in children as in adults and present complex problems which are to be dealt with often in a context of emergency, on account of their large size and the risk of life-threatening bleeding in a small child. Their diagnostic features are different in children, especially in infants and even more so in the fetus. The main surgical problems are the risk of bleeding and of hydraulic complications. From an oncological perspective, the main tumor types are choroid plexus tumors, subependymal giant-cell astrocytoma, and low-grade gliomas. The developmenta

Computed tomography in diagnosis of intracranial tumors versus tubers in tuberous sclerosis.

Intraventricular gliomas producing obstruction of the foramen of Monro were observed by computed tomography in 4 of 8 patients with tuberous sclerosis. Bilateral tumors arising from the caudate nuclei were present in one patient. Characteristic subependymal calcified tubers were also seen, but contrast enhancement occurred only in the tumors and not in the tubers. The location of the tumor prevented communication between the 2 lateral ventricles so that surgical placement of a single ventricular shunt resulted in only unilateral relief of hydrocephalus. Shunting in one case was ineffective bec

Epidemiology and pathology of intraventricular tumors.

Tumors that primarily or exclusively involve the ventricular system constitute a rare and heterogeneous group. Certain histologic tumor types predominantly occur in children, whereas others are more common in adults. Tumor location provides additional clues to correct diagnosis. When used in conjunction with clinical and radiologic data, histopathologic features can distinguish among this wide range of possibilities to provide the correct diagnosis for optimal patient management.

Pedunculated subependymomas of lateral ventricle.

The authors describe two cases of extremely rare, pedunculated subependymomas of lateral ventricle. One patient presented with symptoms of intracranial hypertension (case 2); CT scan revealed evidence of intraventricular mass, but in second patient (case 1) coexisting symptomatic anaplastic astrocytoma masked on CT-scan the presence of asymptomatic intraventricular tumour, which was found no earlier than at autopsy. The cellular origin of this rare intracranial neoplasm is discussed.

Detection of H3K27M mutation in cases of brain stem subependymoma.

Subependymomas are rare, slow-growing, grade I glial tumors of the central nervous system. Recently, diffuse midline gliomas with mutations in the H3.1 or H3.3 genes at the position of amino acid 27, resulting in the replacement of lysine by methionine (K27M), were defined as the new grade IV entity. As H3K27M mutations have been reported in midline gliomas, gangliogliomas, and pilocytic astrocytomas, whether they occur in midline subependymomas has been unclear. We determined whether any such mutations can be found in them and analyzed the prognostic relevance of any such mutations in subepen

[Transcallosal and transcortical approaches for tumors at the anterior part of the lateral ventricle: relations between visualized and ventricular size].

The transcallosal approach has been widely used for tumors located near the midline of the anterior part of the lateral ventricle, while the transcortical approach is known to provide a wide surgical field in cases of hydrocephalus. However, it is not always easy to choose the appropriate approach in individual cases. The size of the visualized field obtained via the surgical approach is one of the most important factors determining the accessibility of the lesion. In this study, we investigated the difference in the sizes of the fields afforded by these two approaches on the basis of our clin

[Intraventricular fibrous tumor: a case report].

Solitary fibrous tumor was first reported in the pleura; however, it has since been reported in many other locations. Solitary fibrous tumors of the central nervous system are uncommon and the intraventricular location is exceptional. To our knowledge, only eight cases of intraventricular solitary fibrous tumor have been reported. We describe a case of intraventricular solitary fibrous tumor. The imaging characteristics of intraventricular solitary fibrous tumors are nonspecific; the differential diagnosis should include other tumors that can affect the ventricular system such as meningioma, hi
