Glomus jugulare tumor: tumor control and complications after stereotactic radiosurgery.

BACKGROUND: We evaluated toxicity and long-term efficacy of stereotactic radiosurgery in patients with symptomatic or progressive glomus jugulare tumors. METHODS: Twenty-five consecutive patients (age, 30-88 years; 17 women, 8 men) who underwent stereotactic radiosurgery with the Leksell Gamma Knife (dose, 12-18 Gy) were prospectively followed. MRI and clinical examinations were performed at 6 months and 1, 2, and 3 years, and then every 2 years. RESULTS: None of the tumors increased in size, 17 were stable, and 8 decreased (median imaging follow-up, 35 months; range, 10-113 months). Symptom

[Magnetic resonance tomographic studies of paragangliomas of the glomus caroticum and glomus jugulare using Gd-DTPA].

The value of magnetic resonance tomography in the diagnosis of paragangliomas in the head and neck has been studied and compared with CT and angiography. Magnetic resonance tomography on its own equals the accuracy of CT, but the use of Gd-DTPA improves diagnostic accuracy. In 19 patients with a glomus jugulare tumour, MR tomography with Gd-DTPA accurately diagnosed all tumours larger than 5 mm. In seven patients with a carotid body tumour, it was possible to arrive at an exact differential diagnosis. Sensitivity was better than that of CT or of sonography. Where there is clinical suspicion of

Facial nerve palsy: an atypical sign of glomus jugulare tumour recurrence.

We present a rare case of recurrent glomus jugulare tumour, manifesting as an isolated facial nerve palsy. Glomus jugulare tumours are slow-growing paragangliomas of the jugular bulb adventitia. We report the case of a 73-year-old female referred for oculoplastic management of a facial nerve palsy, with a history of a right-sided glomus jugulare tumour treated surgically 40 years prior. Her 6-month history of facial palsy had initially been attributed towards Bell's palsy, particularly in the context of communication challenges related to residual dysphonia and hearing loss following her prior

Jugular Foramen Paraganglioma (Glomus Jugulare): A 10-Year Single-Center Experience.

BACKGROUND: Jugulotympanic paragangliomas (JTPs) are rare, hypervascular skull base tumors. Surgery and stereotactic radiosurgery (SRS) are often framed as competing options, yet outcome data from Latin American public referral centers are scarce. We evaluated a function-first surgical strategy and related tumor stage to outcomes. METHODS: Retrospective single-center study of 24 consecutive patients with histopathologically confirmed JTP who underwent 32 resections (2016-2025), analyzed at patient and procedure level. Fisch class (A/B vs. C/D) was related to extent of resection, cranial nerve

Unmasking Villaret's syndrome: a diagnostic challenge of glomus jugulare mimicking mastoiditis.

INTRODUCTION: Villaret's syndrome involves unilateral palsy of cranial nerves IX to XII, often with Horner's sign, commonly caused by benign neoplasms or vascular issues. This report highlights glomus jugulare-induced Villaret syndrome, stressing careful evaluation for persistent otologic symptoms. CASE PRESENTATION: A 43-year-old woman with a history of chronic suppurative otitis media (CSOM) presented with recurrent ear discharge, pain, and severe mixed hearing loss. Despite a left tympanoplasty, symptoms persisted, leading to a canal wall down mastoidectomy. Six months later, she developed

Stereotactic Radiosurgery for Glomus Jugulare Tumors: Systematic Review and Meta-Analysis.

BACKGROUND: Glomus jugulare tumors (GJTs) are benign paragangliomas of the jugular foramen. Traditional management of these tumors involves surgical resection; however, considering the proximity of these tumors to important neurovasculature, stereotactic radiosurgery (SRS) may be an appropriate noninvasive treatment to consider. The aim of this meta-analysis was to evaluate SRS as a treatment option for GJTs. METHODS: An online search using PubMed, Web of Science, Scopus, and Cochrane databases was performed in March 2019 for articles on radiosurgery treatment of GJTs. The screening process f

Simultaneous contralateral vestibular schwannoma and glomus jugulare tumor: a case report.

To the best of our knowledge, only 3 cases of a simultaneous vestibular schwannoma and a glomus jugulare tumor have been previously reported in the literature. In all 3 cases, the lesions were located on the same side. We report a new case of simultaneous vestibular schwannoma and glomus jugulare tumor that is unique in that the two lesions arose on opposite sides. The glomus tumor was treated with embolization followed by radiotherapy, while the schwannoma was managed via radiologic observation.

Gamma Knife radiosurgery of the glomus jugulare tumour - early multicentre experience.

Leksell Gamma Knife was used to treat 66 patients with glomus jugulare tumour at 6 European sites between 1992-1998. The age of the patients ranged between 18-80 years (median 54 years). Gamma Knife radiosurgery was a primary treatment in 30 patients (45. 5%). Open surgery preceded radiosurgery in 24 patients (36.4%), embolisation in 14 patients (21.2%) and fractionated radiotherapy in 5 patients (7.6%). The volume of the tumour ranged 0.5-27 cm(3) (median 5,7 cm(3)). The minimal dose to the tumour margin ranged between 10-30 Gy (median 16.5 Gy). After radiosurgery 52 patients were followed, t

Leksell gamma knife radiosurgery of the tumor glomus jugulare and tympanicum.

We have treated 14 patients with glomus tumor during the 4 years (of 1993 to 1997) using Leksell Gamma Knife radiosurgery. The male: female ratio was 1:3.7, and the mean age 48.6 years (range 22-75 years). The mean tumor volume was 5.5 cm3 (range 0.7-11.3 cm3). The mean maximum dose was 37.4 Gy (range 20-44 Gy). The mean margin dose was 19.4 Gy (range 10-25 Gy). In 3 patients, infrabasal spread of the tumor could not be delineated on peroperative stereotactic CT scans. As a result, this portion of the tumor was treated in 2 patients at a second stage using stereotactic MRI. Follow-up in 11 pat

Management of a glomus jugulare tumour with internal carotid artery involvement.

A previously healthy 40-year-old female presented for surgical resection of a large glomus jugulare tumour with extensive involvement of the carotid siphon and intracranial extension. Conduct of anaesthesia with specific reference to cerebral protection is discussed. A combination of induced hypothermia, barbiturate therapy, normotension, normocarbia and prior clamping of the distal internal carotid artery was chosen. The role of barbiturates as a therapeutic intervention is debated.

[Imaging and assessment of glomus jugulare in MRI and CT techniques].

INTRODUCTION: Paragangliomas are rare group of tumors originating from paraganglionic tissue. The most common site of occurrence of the head and neck paragangliomas are: carotid body, jugular foramen (glomus jugulare) and tympanic cavity (glomus tympanicum). Magnetic resonance (MR), computed tomography (CT) and angiography are the modalities of choice in detecting and characterizing paragangliomas. AIM: The aim of this study was to compare different imaging techniques of CT and MR in visualization, detecting and characterizing paragangliomas of the jugular foramen. METHODY: Usefulness of CT a

Radiosurgery for glomus jugulare tumors: experience treating 16 patients in Iran.

OBJECT: Glomus jugulare tumors (GJT) have traditionally been treated by surgery or fractionated external-beam radiotherapy. The aim of this retrospective study was to determine the tumor control rate, clinical outcome, and short-term complications of stereotactic radiosurgery in subsets of patients who are poor candidates for these procedures, based on age, medical problems, tumor size, or prior treatment failure. METHODS: The Leksell Gamma Knife was used to treat 16 patients harboring symptomatic, residual, recurrent, or unresectable GJTs. The age of the patients ranged from 12 to 77 years (

Glomus jugulare tumors: historical overview of the management of this disease.

The treatment of glomus jugulare tumors presents the surgeon with a significant management problem. Because the neoplasm originates in the region of the jugular bulb, it frequently involves the lower cranial nerves, with occasional extension into the posterior fossa. Despite extensive work on the development of surgical and radiation treatment strategies, considerable controversy still exists regarding the optimal management of these lesions. A historical review of the development of management options for glomus jugulare tumors is presented in an effort to offer a foundation for understanding

Conservation surgery for glomus jugulare tumors: the value of early diagnosis.

The results of lateral cranial base surgery for glomus jugulare tumors are gratifying when normal anatomy and function can be preserved. The goal of conservation surgery is to preserve normal ear anatomy and cranial nerve function. In general, conservation surgery is tumor-size dependent. Thus, excellent states of functional recovery depend upon accurate early diagnosis. This paper reviews the technical aspects of transtemporal conservation skull base tumor surgery while also reviewing our experience with nearly 100 glomus jugulare patients. Adjuvants to early diagnosis will be highlighted fro
