The prenatal diagnosis of Robin anomalad.

The Robin anomalad was diagnosed by the sonographic detection of polyhydramnios and fetal micrognathia in a patient at risk because of a previously affected child. Ultrasound in the second trimester failed to demonstrate any facial anomaly, but mandibular hypoplasia was clearly documented in the third trimester. The antenatal diagnosis allowed immediate neonatal assistance to prevent glossoptosis-induced respiratory failure.

Management of airway and feeding difficulties in a neonate with Pierre Robin sequence: a case report.

Pierre Robin sequence is a congenital disorder that affects neonates, and is characterised by the classic triad of micrognathia, glossoptosis, and cleft of the secondary palate. We present the case of a child with Pierre Robin sequence who had severe respiratory distress and feeding problems, and describe a simple technique for forward traction of the mandible to relieve the respiratory distress and activate the tongue.

Twins with Pierre Robin syndrome.

An increasing number of case reports have drawn attention to the familial occurrence of the Pierre Robin syndrome. Postmortem studies on male twins concordant for the syndrome are reported. In addition to the triad of micrognathia, cleft palate and glossoptosis, both twins presented low-set malformed ears. Roentgencephalometric measurements suggested that the extreme micrognathia present in the twins precluded survival. An older sib with similar deformities also died in infancy.

Prenatal Identification of Pierre Robin Sequence: A Review of the Literature and Look towards the Future.

Fetal ultrasonography is an important tool used to prenatally diagnose many craniofacial conditions. Pierre Robin sequence (PRS) is a rare congenital deformation characterized by micrognathia, glossoptosis, and airway obstruction. PRS can present as a perinatal emergency when the retropositioned tongue obstructs the airway leading to respiratory compromise. More predictable and reliable diagnostic studies could help the treating medical team as well as families prepare for these early airway emergencies. The medical literature was reviewed for different techniques used to prenatally diagnose P

[Salivary lithiasis as a complication of surgically treated Robin syndrome].

A 15-year-old girl, operated upon (surgical ankyloglossia) at birth for glossoptosis due to Pierre Robin's syndrome provoking respiratory distress, presented with lithiasis of Wharton's duct. The pathogenesis of the lithiasis is considered to be related to the wounds and scars following the sublingual surgery, findings in this case also strongly suggesting the role of mechanical and inflammatory factors in the etiology of salivary gland lithiasis in general.

Respiratory management of Pierre Robin sequence using nasopharyngeal airway with Kirschner wire.

BACKGROUND: The Pierre Robin sequence (PRS) is a relatively rare symptom complex characterised by glossoptosis, micrognathia and respiratory obstruction. The initial problem that children with PRS face is obstructive dyspnoea, which can result in death without appropriate respiratory management. We designed and used a modified airway with a Kirschner wire (K-airway) in children with PRS who suffered from dyspnoea that did not improve with conservative treatment. METHODS: The subjects were four children diagnosed with PRS at the Department of Plastic Surgery, Shizuoka Children's Hospital, from

"Airway-First, Heart-Early" Strategy: Managing Neonatal Dextro-Transposition of the Great Arteries in Pierre Robin Sequence.

The concurrent presentation of Pierre-Robin sequence (PRS) and dextro-transposition of the great arteries (d-TGA) is extremely rare and presents distinct management challenges. We describe a term male neonate with the classic PRS triad (severe micrognathia, glossoptosis, and a U-shaped cleft palate) who developed profound cyanosis unresponsive to supplemental oxygen. Echocardiography demonstrated d-TGA with an intact ventricular septum, a restrictive patent foramen ovale, and a patent ductus arteriosus. Prone positioning and a nasopharyngeal airway were unsuccessful in improving oxygenation. O

Cerebro-costo-mandibular syndrome.

We describe two boys with the cerebro-costo-mandibular syndrome (CCMS). Both patients presented with Pierre Robin anomaly and respiratory insufficiency and died 12 hours and 10 months after birth. The first boy had muscular hypotonia, severe micrognathia, glossoptosis, short palate, preauricular tag, paraumbilical fibroma, and a small and narrow thorax. His chest roentgenographs showed marked hypoplasia of the first to tenth rib, multiple posterior rib-gaps in the only four ossified ribs. Tracheomalacia and stenosis of the left ureter was observed during autopsy. No structural cerebral anomali

Cleft palate surgery and speech outcomes in children with Robin sequence.

Robin sequence (RS) is diagnosed by the clinical triad of micrognathia, glossoptosis, and airway obstruction. The reported occurrence is variable and noted in 1:8,500 to 1:20,000 live births. Although advances in perinatal imaging, neonatal resuscitation, non-invasive respiratory support and surgical techniques can mitigate against adverse outcomes, there remains much debate as to the best treatment for children born with RS. This review will outline surgical treatment of cleft palate and speech outcomes for children with RS.

[Influence of the soft palate on tongue statics and mandibular growth. Therapeutic conclusions].

The soft palate has a major influence on mandibulo-lingual statics. Thus in the case of congenital cleft palate, there is in general glossoptosis with mandibular retrognathia. Conversely, palatal shortening may be associated with functional mandibular prognathia which it is possible to correct by urano-staphylo-pharyn-goplasty, if carried out sufficiently early. In many cases of mandibular prognathia the soft palate has a special shape and orientation. The problem is to know whether this appearance is secondary to the dysmorphosis or partially responsible for it.

Application of the MicroNAPS Classification for Robin Sequence.

ObjectiveThe only findings consistent among infants with Robin sequence (RS) are the presence of micrognathia, glossoptosis, and upper airway obstruction (UAO). Feeding and growth dysfunction are typical. The etiopathogenesis of these findings, however, is highly variable, ranging from sporadic to syndromic causes, with widely disparate levels of severity. This heterogeneity has created inconsistency within RS literature and debate about appropriate workup and treatment. Despite several attempts at stratification, no system has been broadly adopted.DesignWe recently presented a novel classific

Airway management in Nager Syndrome.

Nager acrofacial dysostosis is a rare congenital syndrome characterized by malformed mandibulofacial structures and pre-axial upper limbs. Trismus and glossoptosis from mandibular abnormalities predisposes infants to life-threatening respiratory distress. A case of a Nager Syndrome mother delivering a similarly afflicted fetus is presented, with approaches to maintaining both tenuous airways described. Distinguishing this condition from similar syndromes is critical for care and prognosis.

Successful weaning of a laryngeal mask airway after a tongue-lip adhesion operation in a case with cerebro-costo-mandibular syndrome.

Cerebro-costo-mandibular syndrome (CCMS) consists of severe micrognathia, glossoptosis, posterior rib-gap defects and developmental delay. It may cause upper airway obstruction andflail chest, resulting in neonatal hypoxia, and possibly death. Early airway management or surgical intervention to maintain a patent airway is critical to avoid hypoxia in CCMS patients. We report a newborn with CCMS who was successfully weaned from a laryngeal mask after undergoing a tongue-lip adhesion operation at 164 days of age.

[An original intubation technic in a child with Pierre Robin syndrome].

Infants with Pierre Robin syndrome often present the anaesthesiologist with the challenge of upper airway obstruction and difficult tracheal intubation. An unconventional method for solving this problem was used in a 2 week old 2.2 kg term male infant who presented with severe micrognathia, a widely cleft palate and extreme glossoptosis. Hyperextension of the head in the prone position distracted the epiglottis from the glottis. Blind nasotracheal intubation was then used. The curve of the nasotracheal tube made it pass behind the epiglottis into the larynx.

Cor pulmonale and acute liver necrosis, due to upper airway obstruction as part of pycnodysostosis.

A male patient with pycnodysostosis suffered from chronic respiratory insufficiency and pulmonary hypertension. This was caused by concomitant upper airway obstruction, resulting from a low implanted uvula and a long soft palate, in combination with glossoptosis and retrognathia due to the flattened mandibular angles. An inter-current respiratory infection gave rise to an acute deterioration, with right-sided heart failure, severe liver damage and coma. Surgical shortening of the soft palate was performed, after which the blood gas values returned to normal.
