Molecular mechanisms of peritoneal dialysis-induced microvascular vasodilation.

Peritoneal dialysis (PD) solutions dilate microvessels by undefined mechanisms. This vasodilation directly affects ultrafiltration and solute exchange during a PD dwell and is thought to account for the variable mass transfer area coefficient for small solutes during a glucose-based hypertonic dwell. We hypothesized that PD-mediated vasodilation occurs by endothelium-dependent mechanisms that involve endothelium energy-dependent K+ channels (K(ATP)), adenosine A1 receptor activation, and NO release. We used intravital videomicroscopy to study 3 levels of microvessels (A1 inflow arterioles abou

[Carbohydrates in parenteral nutrition solutions in pediatrics--a critical evaluation].

Application of carbohydrates in pediatric infusion therapy has recently been limited to glucose and xylitol. Fructose and sorbitol, which formerly had been used widely as energy sources in parenteral nutrition, have meanwhile been banned in order to prevent fatal complications in patients with undiscovered hereditary disturbances in fructose metabolism. The aim of this review is to focus the attention on potential side effects and limitations of glucose administration in pediatric infusion therapy. With special regard to total parenteral nutrition in preterm infants, sufficient glucose convers

Treatment of 242 neonates with dehydrating diarrhea with an oral glucose-electrolyte solution.

Two hundred thirty-four of 242 neonates (96.7%) with a mean dehydration of 5.1% body weight were rehydrated with a glucose-electrolyte solution given orally in an average time of 7 44 hours. Eight (3.3%) neonates required intravenous fluid therapy. Emesis was not an obstacle for complete oral rehydration. Hypernatremia, hyponatremia, and acidosis present on admission were corrected within a few hours. Oral rehydration is an entirely satisfactory choice for treatment of uncomplicated dehydration in neonates.

Pancreatitis in patients with Reye's syndrome.

Three patients who died of a Reye's syndrome had biochemical or pathological evidence of pancreatitis. None of the 38 survivors had clinical or biochemical evidence of pancreatic dysfunction. The use of hypertonic glucose solutions and exogenous insulin may reverse the metabolic abnormalities seen in Reye's syndrome and may spare the pancreas, thereby preventing the development of pancreatitis. In addition, serum calcium and glucose concentrations appear to correlate with pancreatic function and may be of value in detecting the occurrence of pancreatitis. It is our impression that the developm

Evaluation of a nutritive oral rehydration solution for the treatment of calf diarrhoea.

The essential constituents of a conventional oral rehydration solution (ORS) are sodium, glucose and a bicarbonate precursor. The glucose promotes sodium uptake but because these solutions are isotonic, it is insufficient to sustain calorie requirements. This paper examines the performance of a novel ORS with over three times the conventional glucose concentration, by comparing it with two leading commercial ORSs in calves with induced Escherichia coli diarrhoea. This solution showed greater ability than the current market-leading ORS to repair extracellular fluid and plasma volume and to corr

Assessing ureteral patency using 10% dextrose cystoscopy fluid: evaluation of urinary tract infection rates.

BACKGROUND: Intravenous indigo carmine has routinely been used to confirm ureteral patency after urogynecologic surgery. Recent discontinuation of the dye has altered clinical practice. In the absence of indigo carmine, we have used 10% dextrose in sterile water (D10) as cystoscopic fluid to evaluate ureteral patency. Glucosuria has been associated with urinary tract infection (UTI) in vivo and significantly enhanced bacterial growth in vitro. The concern is that the use of D10 would mimic a state of glucosuria albeit transient and increase the risk of postoperative UTI. OBJECTIVES: The objec

[Value of total parenteral nutrition after total pancreatectomy].

The authors report 5 cases of Total Pancreatectomy (T.P.) treated with Total Parenteral Nutrition (T.P.N.) in which the non-protein calorie supply consisted of lipids and carbohydrates ("dual energy system"). This form of nutrition, which provided a high energy supply with a reduced glucose intake, prevented the problems associated with hypertonic glucose solutions in the unstable diabetes which follows total pancreatectomy. The authors compared these cases with an earlier series of 11 cases of Total pancreatectomy who were not treated with Total Parenteral Nutrition and observed that the pati

Effect of composition of gastric contents on resistance to emptying of liquids from stomach in humans.

Experiments were conducted in normal healthy volunteers to investigate whether factors other than the intragastric pressure induced by tonic contraction of the gastric fundus could regulate the gastric emptying of liquids. The emptying of solutions of different osmolality and composition from the stomach was measured, while maintaining the intragastric pressure constant with a barostat system that employed an external reservoir. Preliminary experiments showed a linear relationship between intragastric pressure and emptying rate, but indicated that a normal intragastric pressure of 7 cm water w

Glycemic Control in Patients with Diabetes on Peritoneal Dialysis: From Glucose Sparing Approach to Glucose Monitoring.

Optimized glycemic management is crucial for controlling atherosclerosis and consequent cardiovascular morbidity in patients with diabetes. Due to the continuous glucose burden from glucose-containing peritoneal dialysis (PD) solutions, PD patients with diabetes experience difficulties in glucose level regulation with glucose hypervariability and worsening dyslipidemia. Even in non-diabetic PD patients, glucose-containing PD solutions aggravate insulin resistance and cause overweight. Additionally, glucose degradation products (GDP) from glucose-based PD solutions provoke oxidative stress and

A New Peritoneal Dialysis Solution Containing L-Carnitine and Xylitol for Patients on Continuous Ambulatory Peritoneal Dialysis: First Clinical Experience.

Peritoneal dialysis (PD) is a feasible and effective renal replacement therapy (RRT) thanks to the dialytic properties of the peritoneal membrane (PM). Preservation of PM integrity and transport function is the key to the success of PD therapy, particularly in the long term, since the prolonged exposure to unphysiological hypertonic glucose-based PD solutions in current use is detrimental to the PM, with progressive loss of peritoneal ultrafiltration capacity causing technique failure. Moreover, absorbing too much glucose intraperitoneally from the dialysate may give rise to a number of system

Amino acid metabolism in acute renal failure: influence of intravenous essential L-amino acid hyperalimentation therapy.

A solution of 8 essential I-amino acids and hypertonic dextrose was administered to 5 patients in acute postoperative renal failure in a program of hyperalimentation designed to decrease the patient's catabolic state and to accrue certain metabolic benefits. A sixth patient receiving intravenous glucose alone served as a control. The pretreatment plasma concentrations of amino acids in all 6 patients did not differ significantly from normal; following intravenous essential amino acids at a dose of approximately 12.6 gm/24 hours, no significant elevations out of the normal range of these substa

Elevated pleural fluid glucose: a risk for tension hydrothorax.

We have presented a case of tension hydrothorax that developed after hyperosmolar hyperalimentation fluids were infused into the pleural space through a misplaced jugular venous line. The hyperosmolar state of the pleural fluid appears to have played a prominent role in the development of the tension hydrothorax. We believe there is a risk of tension hydrothorax in any clinical situation in which the pleural fluid glucose concentration is significantly higher than the serum concentration.

Mechanisms of pancreatic hypersecretion following intraduodenal hyperosmolar glucose infusion in rats.

The effects of intraduodenal glucose infusions on pancreatic exocrine secretion were studied in anesthetized and in conscious rats with pancreatic fistula. In the anesthetized model a stimulating effect of glucose on pancreatic flow and bicarbonate output was evidenced. This effect was dose-related and depended in part upon osmolarity of the solution. No nervous mechanism could be demonstrated, while release of secretin in portal blood might be involved in the observed effect. In conscious rats an increase of bicarbonate concentration and output was observed after 30% intraduodenal glucose whi

Cardiovascular effects of hypertonic sodium chloride solutions when injected into the liquor space of anaesthetized cats.

1 In cats anaesthetized with chloralose, hyper- and hypotonic solutions were injected into the cisterna magna (in 0.5 ml) or into a lateral cerebral ventricle (in 0.2 to 0.3 ml), with aqueduct cannulated to prevent the injected solution from entering the subarachnoid space, and the effects on blood pressure and heart rate were examined. 2 Cisternal injections of hyper- and hypotonic solutions of NaCl (0.51 M and 0.05 M), glucose (1.03 M and 0.10 M), or sucrose (1.02 M and 0.10 M), as well as distilled water produced a rise in arterial blood pressure with tachycardias. Clinical Use of Hypertonic Lactate-Current Evidence and Emerging Perspectives.

CONTEXT: Lactate has traditionally been viewed as a metabolic byproduct associated with metabolic acidosis, exercise-induced fatigue, and cancer metabolism. However, accumulating evidence highlights its valuable role as an alternative fuel and a key signaling molecule. Hypertonic sodium lactate infusions have gained interest for their potential therapeutic applications, particularly in neurology and cardiology. This review examines the established and potential clinical uses of exogenous hypertonic lactate treatment. EVIDENCE ACQUISITION: A literature search was conducted in PubMed and Medlin

Circulatory effects of potassium, glucose and insulin following open-heart surgery.

The cardiovascular effects of an acute infusion of potassium, glucose and insulin (PGI) were investigated in 10 subjects following valve replacement surgery. All required circulatory support with isoprenaline and no additional improvement in cardiac function could be demonstrated following PGI. Pressures in both atria rose out of proportion to the volume infused and there were biochemical changes which suggested that haemodilution had occurred. These findings were attributed to the administration of hypertonic dextrose solution and this also resulted in hyperglycaemia, glycosuria and an osmoti

How gastric emptying of carbohydrate affects glucose tolerance and symptoms after truncal vagotomy with pyloroplasty.

It is commonly assumed that both abnormal glucose tolerance and postcibal symptoms after truncal vagotomy with pyloroplasty (V&P) are the result of rapid gastric emptying of carbohydrate; yet such correlations have not been established. These studies measured gastric emptying in symptomatic patients with V&P and normal subjects and sought correlations between rates of emptying and the time-courses of serum glucose and insulin as well as between emptying and symptoms. Gastric emptying in the V&P varied widely with different isocaloric meals of hypertonic glucose, taken in the erect vs. supine p

Randomized controlled study of icodextrin on the treatment of peritoneal dialysis patients during acute peritonitis.

BACKGROUND: The clinical benefits of using icodextrin during acute peritonitis in peritoneal dialysis are uncertain. On the premise that high glucose concentration might jeopardize the peritoneal defense during peritonitis, icodextrin administration during acute peritonitis could have the potential to improve the peritonitis outcome whilst improving ultrafiltration. METHODS: We conducted a single-center, open-label, randomized controlled trial in which 53 adult continuous ambulatory peritoneal dialysis patients underwent randomization to receive either icodextrin or original glucose-based dia
