Relapse and therapeutic resistance remain critical challenges in multiple myeloma (MM). This study investigates the macromolecular drivers of metabolic reprogramming in MM to identify novel therapeutic targets. INTRODUCTION: Antimicrobial resistance (AMR) is a global issue that needs addressing. While antibiotic stewardship has improved often by restricting antibiotic use, some antibiotics that are still sold legally over the counter (OTC), notably in sore throat medications. Recent findings suggest OTC antibiotics could trigger cross-resistance to antibiotics used in clinical treatments, whether systemic or topical. Tityustoxin (TsTX), a toxin obtained from the venom of the Brazilian scorpion Tityus serrulatus, stimulates Na+ influx through tetrodotoxin (TTX)-sensitive Na+ channels which, in turn, promotes both Ca(2+)-dependent and Ca(2+)-independent release of glutamate from rat cerebrocortical synaptosomes. Neisseria meningitidis is an uncommon cause of acute bacterial conjunctivitis. One case of primary meningococcal conjunctivitis in a healthy 6-year-old boy is reported. The patient was initially treated with a topical instillation of polymyxin B, neomycin and gramicidin in ophthalmic solution, and this was followed by systemic rifampin once the diagnosis had been established. No ocular or systemic complications developed.

A patient with a history of allergic reaction to an antibiotic cream (Mycolog [mixture of nystatin, neomycin sulfate, gramicidin, and triamcinolone acetonide]) was treated with aminophylline. A generalized exfoliative dermatitis resulted. The reaction was attributable to ethylenediamine hydrochloride, which is a component of both agents. Recognition of the potential for systemic medications to induce similar reactions in patients who are allergic to related topical preparations is important when prescribing systemic agents.
