Does thigh compression improve venous hemodynamics in chronic venous insufficiency?

OBJECTIVE: The aim of this study was to investigate the hemodynamic effects of thigh compression in patients with deep venous incompetence. PATIENTS AND METHODS: This diagnostic test study was set in a municipal general hospital. Twelve patients with venous leg ulcers (CEAP classification, C6 Es Ad Pr; four men and eight women), with a mean age of 56.5 +/- 16.8 years, with popliteal venous reflux of more than 1 second detected with duplex scan, underwent investigation with the following methods: 1, the pressure exerted under thigh-length compression stockings class II and short-stretch adhesi

G-LOC recovery with and without G-suit inflation.

During acceleration (+Gz) training in the human centrifuge, the anti-G suit (AGS) is usually deflated as acceleration decreases upon termination of the exposure, regardless of the reason for termination, including +Gz-induced loss of consciousness (G-LOC). This is when the trainee most needs the support provided by the AGS. A method to reduce the time of incapacitation resulting from G-LOC was evaluated. The standard CSU15-P suit worn by 30 aircrew while undergoing +Gz tolerance training was inflated to 10 psi immediately upon G-LOC (GS group). Incapacitation times and flailing activity were r

Do graduated compression stockings and pneumatic boots have an additive effect on the peak velocity of venous blood flow?

Graduated compression stockings and intermittent pneumatic compression boots reduce the incidence of deep vein thrombosis. Recent studies suggest that the simultaneous use of these devices may have a synergistic prophylactic effect; however, conflicting reports also exist. Using duplex imaging, we analyzed the effect on peak venous velocity in the superficial femoral vein produced by the individual and simultaneous use of graduated compression stockings and intermittent pneumatic compression boots. Normal volunteers and postoperative patients were examined. The use of intermittent pneumatic co

Anterior tibial compartment syndrome following use of MAST suit.

A case of anterior tibial compartment syndrome following the use of the military antishock trousers (MAST) suit is reported. Early recognition of this complication is necessary to ensure a good outcome. Monitoring of the MAST pressure and use of the lowest pressure that is effective may decrease the likelihood of this complication.

Hemodynamic effects of Medical Anti-Shock Trousers (MAST garment).

Despite widespread use of the Medical Anti-Shock Trousers (MAST) little is known about the exact mechanism by which they increase arterial pressure. It is assumed that an autotransfusion occurs. To examine this question, blood pressure, heart rate, forearm blood flow, cardiac output, and stroke volume were measured in ten healthy adults, supine and during 60 degree headup tilt with MAST garment pressures of 409 and 100 mm Hg. Supine, the garment produced no net 'autotransfusion,' but raised blood pressure (27%) by increasing peripheral resistance (48%) with decreased stroke volume and cardiac

MAST 96.

The Military Anti-Shock Trouser, or MAST suit, is a controversial device that has been used to support blood pressure in hypotensive trauma patients. Most studies on humans have shown that the device has limited clinical utility. In this study, a telephone survey of all 50 State Emergency Medical Services was conducted to determine the nature and extent of MAST suit usage in the United States. The trend in MAST suit usage in San Diego County over the last 7 years was also analyzed. Thirty (60%) states still require MAST suits to be carried on ambulances. In San Diego County, MAST suit inflatio

Pneumatic antishock garment-associated compartment syndrome in uninjured lower extremities.

Pneumatic antishock garment-associated compartment syndrome is a rare and yet underrecognized complication that when it occurs, frequently results in myonecrosis and loss of limb function, and occasionally loss of a limb or even death. We report a case of pneumatic antishock garment-associated compartment syndrome in a trauma patient without lower extremity injury and review similar published reports. It is only with a high index of suspicion, early recognition, and prompt treatment of this complication by fasciotomy and proper wound care that associated morbidity and potential mortality of th

Mechanisms and management of anaphylactic shock not responding to traditional therapy.

Anaphylaxis results from the generation and release of a variety of mediators and their effects on various organs. Involvement of the respiratory and cardiovascular systems is of primary importance to the attending physician. Prompt recognition and initial therapy directed to maintenance of an effective airway and circulatory system is critical. Cardiovascular collapse may involve the parallel operation of several mechanisms including hypovolemia, alterations in pulmonary or systemic vascular resistance, depressed myocardial contractility, dysrhythmias, and exogenous drug administration. Depression of respiratory function by pneumatic antishock trousers in traumatic quadriplegia.

A case of mild hypoventilation associated with an incompletely inflated pneumatic antishock suit in traumatic quadriplegia is presented. Pulmonary function studies were performed on the same patient 23 days later. Base line studies were performed and then repeated with the antishock suit inflated to 40 and then 100 mm Hg. Measurements were made of tidal volume (Vt), inspiratory reserve volume (IRV), expiratory reserve volume (ERV), forced vital capacity (FVC), forced vital capacity in one second (FEV1), and maximum voluntary ventilation (MVV). With increasing suit pressures, all parameters dec

Comparative hemodynamic effects of antishock suit and volume expansion in normal human beings.

The effects of pneumatic antishock trousers on central venous pressure (CVP) and mean arterial pressure (MAP) were compared with those of volume expansion with normal saline in a group of healthy young men. CVP increased from 4.8 +/- 1.9 to 7.3 +/- 2.5 mm Hg (P less than .001) with the suit and from 4.7 +/- 1.9 to 7.3 +/- 2.2 mm Hg (P less than .001) with volume expansion. During suit use, MAP increased from 85 +/- 6.7 to 93 +/- 6.8 mm Hg (P less than .001), while during volume expansion it did not change. Therefore, despite equivalent increases in CVP, the pneumatic antishock suit significant

Effects of pneumatic antishock trousers on canine intracranial pressure.

The military antishock trouser (MAST) has become an accepted prehospital treatment modality for hypovolemic shock. Our interest in the effect of MAST on intracranial pressure (ICP) was aroused by the manufacturer's written warning that use of the device was contraindicated in patients with suspected head injury. This study was undertaken to determine the effect of MAST device inflation on intracranial pressure in dogs with normal and artificially elevated ICP in both normovolemic and hypovolemic states. The largest increase in hemodynamic parameters was seen in mean arterial pressure (MAP) in

Efficacy of subcutaneous heparin in prevention of venous thromboembolic events in trauma patients.

Trauma patients have an incidence of deep venous thrombosis (DVT) of 7% to 65%. Conflicting data exists regarding the impact of prophylactic anticoagulant therapy. The purpose of this study is to assess the efficacy of DVT prophylaxis based upon our data and a meta-analysis of the current literature. Literature review revealed five recent studies that met the following criteria: 1) Adult patients sustaining major trauma (injury severity score (ISS) > 10), 2) prospective surveillance for DVT using lower extremity duplex examinations, 3) clear documentation of method of DVT prophylaxis with part

[Reaction to decreased circulating blood volume in persons subject and resistant to motion sickness (airsickness)].

The study of 54 healthy male test subjects demonstrated significantly different responses of motion sickness susceptible and resistant people to 10-minute occlusion of their femoral veins. The changes in limb rheograms, as well as heart rate and stroke volume indicated that in motion sickness susceptible subjects the circulating blood volume in the upper body decreased, whereas in motion sickness resistant subjects it remained unaltered or increased in response to the occlusion.

Combined analysis of the non-pneumatic anti-shock garment on mortality from hypovolemic shock secondary to obstetric hemorrhage.

BACKGROUND: Obstetric hemorrhage is the leading cause of maternal mortality, particularly in low-resource settings where women face significant delays in accessing definitive treatment. The Non-pneumatic Anti-Shock Garment (NASG) is a first-aid device to stabilize women in hypovolemic shock secondary to obstetric hemorrhage. Prior studies on the effectiveness of the NASG have suffered from small sample sizes and insufficient statistical power. We sought to generate a summary effect estimate of this intervention by combining data from all previous quasi-experimental studies. METHODS: Five quas

Impact of the Non-pneumatic Antishock Garment on pelvic blood flow in healthy postpartum women.

OBJECTIVE: The Non-pneumatic Antishock Garment (NASG) is a compression device that has shown significantly decreased blood loss in cases of obstetric hemorrhage. However, there are no physiologic studies of the NASG in postpartum women. This study used Doppler ultrasound to measure the resistive index (RI) in the internal iliac artery, thus approximating blood flow to the pelvis with and without the garment applied. STUDY DESIGN: In this study, RI of the internal iliac artery was measured in a sample of 10 postpartum volunteers with and without the NASG applied. Median RI was calculated and c

Numerical study of arterial flow during sustained external acceleration.

A computer model of the human arterial tree based upon the one-dimensional flow equations is used to study the effects of sustained acceleration (+Gz) on eye-level arterial pressure and flow. Steady-state physiological compensatory mechanisms are modelled using human centrifuge data found in the literature. Cases investigated include the supine and standing human (+1 Gz), the application of resistive and occlusive anti-G suits, and the occurrence of a stenosis proximal to the carotid bifurcation. Results indicate that the simulated eye-level pressures and flows satisfactorily correspond with t
