Surgical treatment of the hallux rigidus.

Numerous surgical procedures are available for the treatment of hallux rigidus. The choice of a particular one depends on the severity of the disease, patient activity level, and expectations about the surgery. Cheilectomy is recommended for early disease and may be associated with an osteotomy of the proximal phalanx. For active patients who have severe hallux rigidus, arthrodesis and biologic interposition arthroplasties have shown good results. Keller arthroplasty is reserved for low demand patients; prosthetic replacements are not recommended at this time.

Hemiarthroplasty in the treatment of hallux rigidus.

BACKGROUND: Osteoarthritis of the first metatarsophalangeal (MTP) joint is a painful, disabling condition. It can interfere with the ability to run and even walk without pain. An implant of cobalt-chrome steel alloy to replace the base of the proximal phalanx is one solution. The purpose of this study is to review our results with one of these implants. MATERIALS AND METHODS: A series of 23 cases of hemiarthroplasty for the treatment of hallux rigidus from June 2000 to October 2001 has been evaluated using the AOFAS rating system, and the results are presented. RESULTS: The average preoperat

Arthrodesis in the treatment of hallux rigidus.

We reviewed 34 patients (38 joints) with hallux rigidus treated from 1989 to 1999 with arthrodesis of the first metatarsophalangeal joint. Average patient age at time of surgery was 52 (24-71) years, and the mean follow-up was 54 (18-116) months. There were six superficial infections, and all arthrodeses united. There was a good functional result with a significant pain reduction. The mean postoperative American Orthopaedic Foot and Ankle Society (AOFAS) score was 53 (5-84) points.

A simple operation for hallux rigidus.

Dissatisfaction with the Keller-type operation and the arthrodesis of the first metatarsophalangeal joint in hallux rigidus led to the use of a wedge osteotomy method at the base of the proximal phalanx. Short-term results in 8 patients suggest that further testing of this method should be worthwhile.

Keller's arthroplasty in adults with hallux valgus and hallux rigidus.

BACKGROUND: The role of Keller's resection arthroplasty in the management of adult hallux valgus with hallux rigidus is debatable. There are no studies addressing this particular problem. METHODS: This study is a retrospective review of 32 patients (49 feet), conducted by an independent assessor. Subjective and objective criteria were used to assess the results of surgery. There were 30 women and 2 men with an average age at surgery of 62.5years. RESULTS: The results of surgery in terms of relief of pain, cosmesis and use of regular footwear were satisfactory. Excellent and good subjective r

The use of osteotomies in the treatment of hallux limitus and hallux rigidus.

Though osteotomies for relief of hallux limitus and rigidus have been around since the earliest surgical corrections, no sound clinical studies have been performed to warrant their use over the standard accepted techniques of cheilectomy and arthrodesis. These operations are surely more technically demanding than such standard procedures, and involve significant increased risk and postoperative immobilization than cheilectomy alone. Sound theories such as metatarsus primus elevatus and excessive metatarsal length contributing to hallux rigidus have never been proven, and no accurate way to dia

[Minimally invasive surgery for Hallux rigidus].

Hallux rigidus is one of the most common pathologies in foot and ankle surgery. Surgical treatment options can be divided into joint-preserving and non-joint-preserving techniques. Open procedures for both forms have been well established and successfully performed for decades. In recent years, minimally invasive surgical techniques have become increasingly widespread, particularly in forefoot surgery. Cheilectomy, osteotomies of the metatarsal and proximal phalanx, as well as first metatarsophalangeal joint arthrodesis, can all be performed effectively using minimally invasive approaches. Rec

[Treatment algorithm proposed for hallux rigidus].

Hallux rigidus is the most frequent arthrosis of the foot and ankle and the second most important condition involving the first metatarsophalangeal joint after hallux valgus. If left untreated, it may cause important alterations in gait, mobility and activities of daily living. The purpose of this review article is to describe a complete treatment algorithm for all grades of this condition, after a thorough bibliographic review.

Minimally Invasive Cheilectomy for Hallux Rigidus.

Hallux metatarsophalangeal joint cheilectomy is a joint-sparing technique that involves resection of the dorsal metatarsal head osteophytes; this may be achieved through minimally invasive and arthroscopic techniques. General indications for minimally invasive surgery (MIS) cheilectomy are mild-to-moderate hallux rigidus (Grades I-II) with symptomatic dorsal osteophytes causing dorsal impingement and/or shoe wear irritation in those who have failed extensive nonoperative management. The literature confirms equivalent outcomes to open cheilectomy; however, it is somewhat inconsistent regarding

Hallux Rigidus: Update on Conservative Management.

Hallux rigidus is a degenerative arthritic condition affecting the first metatarsophalangeal joint. Prevalence in patients aged 50 years and above is estimated at 20% to 30%, with a portion being symptomatic. Conservative treatment's efficacy is linked to initial pain levels; though shoe modifications and insoles are commonly recommended, their true effectiveness lacks strong evidence. Injection therapy, including corticosteroids and hyaluronic acid, demonstrates varied outcomes, with about 50% of patients undergoing surgery within 1 to 2 years. The condition's etiology remains elusive, but re

Hallux Rigidus: Relevant Anatomy and Pathophysiology.

Hallux rigidus is a painful condition of the great toe characterized by restriction of the metatarsophalangeal joint arc of motion and progressive osteophyte formation. Precise cause of hallux rigidus remains under debate. Anatomic variations and historical, clinical, and radiographic findings have been implicated in the development and progression of hallux rigidus. Radiologic findings associated with hallux rigidus include metatarsal head osteochondral defects, altered metatarsal head morphology, and an elevated hallux interphalangeus angle measure. Associated historical findings include a p

First metatarsophalangeal arthrodesis for hallux rigidus.

Hallux rigidus leads to a restricted and painful motion at the first metatarsophalangeal (MTP 1) joint. Decision making of the appropriate surgical procedure mainly refers to the stage of hallux rigidus. If conservative measures fail, operative procedures can be taken into consideration. Arthrodesis of the MTP 1 joint is widely accepted as the gold standard for end-stage hallux rigidus. Despite the fusion of a key joint, there is little adverse effect on gait, and weight bearing of the first ray can be restored.
