Topographical Analysis of the Hamate for Proximal Pole Scaphoid Nonunion Reconstruction.

PURPOSE: To quantify the similarity of the surface topography of the proximal hamate and proximal pole of the scaphoid for nonunion reconstruction. METHODS: Using previously acquired computed tomographic scans of the wrist of 10 patients, the 2 bones were segmented and subsequently aligned using both a manual and automated technique. Surface error between corresponding articular surfaces was computed to determine the similarity of the shape of the 2 bones. RESULTS: The median distance between the 2 articulating surfaces for each patient was 1 mm or less for all cases. Maximum distance varied

The Measurement of the Inclination Angle of the Hamate and Analysis of the Inclination Angle for the Rotation Deformity of the Little Finger in the Fixation of the Carpometacarpal Joint.

OBJECTIVE: Complex base fractures of the fifth metacarpal bone and dislocation of the fifth carpometacarpal joint are more prone to internal rotation deformity of the little finger sequence after fixation with a transarticular plate. In the past, we have neglected that there is actually a certain angle of external rotation in the hamate surface of transarticular fixation. This study measured the inclination angle of the hamate surface relative to the fifth metacarpal surface for clinical reference. METHODS: In a prospective single-center study, we investigated the tilt angle of 60 normal hama

MR imaging of the type II lunate bone: frequency, extent, and associated findings.

OBJECTIVE: Type II lunate bones have an "extra" facet that articulates with the hamate bone, which frequently leads to arthritis. Because the only prior studies, to our knowledge, on this common and clinically significant variant involved dissection of cadavers, we performed an MR imaging study of the type II lunate. MATERIALS AND METHODS: We retrospectively reviewed MR images obtained at 1.5 T of 186 wrists for frequency of type II lunates, size of the extra facet, amount of hamatolunate apposition, and presence of hamate subchondral edema. Of the 186 wrists, 28 also had correlation with fin

The aetiology of fracture and nonunion in the hook of the hamate.

Fractures of the hook of the hamate are traditionally thought to be caused by direct trauma. A review of the anatomy and function of the hamate hook suggests that fracture is more likely as a result of a fatigue response that develops in the hook from repetitive load applied by the adjacent deep flexor tendons. Additional vascular compromise, from direct pressure of the tendons on critical local vessels, reduces blood flow leading to both mechanical and vascular effects that create pathological osseous change and weakening. These changes are likely to predispose to stress fracture and nonunion

Diagnosis of a hamate hook fracture with 3D reconstruction of computed tomography images: A case report and review of literature.

We report the case of a 19-year old man who presented to our institution with a history of pain of one week in the ulnar side of the left wrist and continuous pressing sensation in the palm. According to patient, there was no history of acute hand trauma. The conventional roentgenograms did not show the fracture and computerized tomographic (CT) scanning demonstrated a complete fracture of the hook of the hamate. The patient was given a surgical intervention and the hamate bone was reduced through a palmar skin incision. After surgery, the operated hand was immobilized in a wrist brace and the

Augmented hamate replacement arthroplasty for fracture-dislocations of the proximal interphalangeal joints in 12 patients.

UNLABELLED: We report clinical outcomes in 12 patients with hemi-hamate replacement arthroplasty combined with volar plate arthroplasty. The volar plate was reattached using trans-osseous sutures to reconstruct the ligament-box complex after hamate grafting to augment the stability of the proximal interphalangeal joint. Ten patients had improved joint movement from a mean of 14° before surgery to a mean of 77° at a minimum follow-up of 2 years. Grip strength and pain of the affected hand and patient-rated hand and wrist scores were improved in these 10 patients. Two patients had poor results.

The Effect of Thumb Immobilization on Fractures of the Hook of the Hamate: A Cadaver Study.

Background: Fractures of the hook of the hamate are rare. Nonoperative management has historically been immobilization in a short arm cast (SAC) without thumb immobilization with a high reported incidence of nonunion. The high prevalence of nonunion following nonoperative treatment may be secondary to motion at the fracture site. The transverse carpal ligament's attachment to the hook of the hamate results in movement at the fracture site during thumb motion. Methods: A cadaveric study using 8 fresh frozen cadaver arms amputated at the mid-humeral level was performed. Computed tomography (CT)

A Modified Surgical Approach Through Guyon's Canal and the Proximal Ulnar Border of the Carpal Tunnel Allows for Safe Excision of the Hook of the Hamate.

Hook of the hamate fractures can be treated by various methods including cast immobilization, open reduction, and internal fixation and excision. Usually, those individuals who elect for excision have acute fractures and need to return to sporting activity or work quickly or have nonunions with persistent symptoms. There is a paucity of descriptions in the literature and textbooks of a technique to safely excise the hook of the hamate. The authors present a method of safely exposing and removing the hook of the hamate by visualizing the potential structures at risk: the motor branch of the uln

Posttraumatic Compressive Neuropathy of the Deep Motor Branch of the Ulnar Nerve Caused by Heterotopic Ossification.

We report a case of posttraumatic compressive neuropathy of the deep motor branch of the ulnar nerve occurring in the setting of heterotopic ossification following a direct blow to the hypothenar eminence. Over several weeks, the patient developed ring and little finger claw deformities and atrophy of his first dorsal interosseous and adductor pollicis muscles with sparing of sensation. Electromyography and nerve conduction study localized the area of injury, and computed tomography confirmed the presence of heterotopic bone near the deep motor branch of the ulnar nerve. Intraoperatively, the

Traumatic dislocation of the hamate and pisiform: a case report and review of the literature.

Isolated dislocation of the hamate bone and pisiform bone is rare. We describe the simultaneous complete dislocation of both the hamate and pisiform bones in a 27-year-old man who crushed his right hand in a rolling press. An open reduction and internal fixation with Kirschner wires was performed. Four weeks later, the Kirschner wires were removed and rehabilitation was started. At 6 months follow-up, the patient had minimal pain and full range of motion in the affected wrist joint and fingers. However, grip strength was 50% compared to his unaffected left hand, and sensation of the ulnar nerv

Multifocal giant cell tumor of the carpus: Unusual presentation. Case report and review of the literature.

INTRODUCTION AND IMPORTANCE: Giant cell tumors (GCTs) of bone in the carpus are rare. Carpal GCTs are usually solitary lesions; multifocal involvement is exceptional. These lesions have a higher risk of local recurrence after intralesional curettage than those in other body areas. CASE PRESENTATION: We present a case of a 28-year-old male with a six-month history of a palpable mass in the dorsal aspect of the left wrist. Physical examination revealed a 2 cm, mildly tender mass. Magnetic resonance revealed a large intermediate signal lesion involving completely hamate bone and the distal porti

Structural changes of the carpal tunnel, median nerve and flexor tendons in MRI before and after endoscopic carpal tunnel release.

The purpose of this study is to investigate the structural changes of the carpal tunnel, median nerve, and flexor tendons in magnetic resonance imaging (MRI) before and after endoscopic carpal tunnel release (ECTR). We studied 36 hands undergoing ECTR. In MRI, the cross-sectional area of the carpal tunnel and the median nerve at the hamate and the pisiform levels were measured. The distance from the volar side of carpal bone to the median nerve or tendons and the volar displacement were measured. In post-operative MRI, the transverse carpal ligament could not be well delineated and the carpal

Trapezius and hamate body fractures. Case report of a rare association.

The trapezium is the most radial and mobile bone in the distal row of the carpus, establishing a proximal joint with the scaphoid and a distal one with the first metacarpal. Isolated trapezium fractures are uncommon, accounting for only 1 to 5% of all carpal fractures. Fractures are often associated with other injuries, including the rare hamate body fracture. At initial presentation, diagnosing those fractures can be challenging as clinical findings are often minimal, without obvious deformities. Crushing injuries are frequently associated with trapezium tuberosity fractures while hamate body

The carpus in congenital anomalies of the hand.

Congenital anomalies of the carpus associated with congenital hand and upper limb anomalies were investigated from the point of view of the development of the hand. Defects of the carpus were categorized as follows: 1) Radial ray defect: loss of the scaphoid, trapezium and trapezoid. 2) Central ray defect: loss of the capitate and loss of a part of the trapezium and hamate. 3) Ulnar ray defect: loss of the triguetrum, pisiformis and hamate. Deficiencies of the carpus and more distal parts of the hand proved to be secondary to defects of the forearm bones in both the radial and ulnar ray. On th

Cross-sectional changes of the distal carpal tunnel with simulated carpal bone rotation.

This study simulated the cross-sectional changes in the distal carpal tunnel resulting from inward rotations of the hamate and trapezium. Rotations which decreased the carpal arch width, increased the carpal arch area. For example, simultaneous rotation of 5 degrees around the hamate and trapezium centroids decreased the carpal arch width by 1.69 ± 0.17 mm and increased the carpal arch area by 6.83 ± 0.68 mm2. Although the bone arch area decreased, decompression of the median nerve would likely occur due to the adjacent location of the nerve near the transverse carpal ligament.

Sixteen patients regarding the conservative treatment for hook of hamate fracture.

BACKGROUND: Hook of hamate fractures occur either due to repetitive stress from gripping sports (e.g., golf, tennis, and baseball), leading to fatigue fracture, or as a result of trauma from falls or other injuries. The recommended treatment involves the excision of bone fragments to facilitate athletes' early return to sports; excision surgery is also performed in trauma cases. However, some patients prefer nonsurgical treatment options, and conservative treatment should be considered. AIM: To present a case series of 16 patients conservatively treated for hook of hamate fractures. METHODS:

Capitate Osteochondral Graft for Reconstruction of Unstable Proximal Interphalangeal Joint Injury.

BACKGROUND: Hemi-hamate osteochondral grafting is a surgical technique that is utilized to reconstruct the proximal interphalangeal (PIP) joint in cases of unstable dorsal fracture-dislocation with >50% articular surface involvement. However, hemi-hamate osteochondral grafting can be technically challenging, has been reported to have various technical modifications, and can lead to complications such as overstuffing of the joint. This surgical technique article describes successful PIP joint reconstruction with use of a hemi-capitate osteochondral graft, which may offer a viable alternative to
