Is Routine Hardware Removal Following Open Reduction Internal Fixation of Tarsometatarsal Joint Fracture/Dislocation Necessary?

Open reduction internal fixation (ORIF) is an accepted treatment for displaced tarsometatarsal joint (TMTJ) fracture dislocations. In general, hardware is routinely removed after 4 months to allow restoration of joint motion and avoid complications of hardware failure. Because few studies report outcomes of TMTJ fractures with retained hardware, there is little consensus regarding the optimal time for hardware removal or if hardware retention leads to adverse outcomes. We retrospectively reviewed the radiographic outcomes of retained hardware after ORIF of TMTJ fractures/dislocations in 61 pat

Frequency and Reasons for Fixation Hardware Removal After Orthognathic Surgery in Patients Treated in One Center.

Background and Objectives: Despite the well-established position of orthognathic surgery as a field of surgical treatment of deformities within the facial skeleton, it has not been possible to develop unanimous recommendations on how to approach fixation hardware after the healing period. In the absence of clear guidelines from opinion leaders and scientific societies on how to approach osteosynthesis after surgery, the decision to leave or remove fixation hardware is made individually by treatment centers, mostly based on their own experience. It is also important whether or not surgical proc

Management of orthopedic plate exposure: a case series evaluating advanced wound care and delayed hardware removal.

BACKGROUND: Plate exposure following open reduction and internal fixation (ORIF) poses significant challenges, including infection risk and impaired fracture healing. Traditional management often involves aggressive surgical interventions. METHODS: This retrospective case series examines seven patients treated between 2020 and 2025 who developed plate exposure after ORIF of lower extremity fractures. Management included local wound care with advanced dressings (Apligraf®, NuShield®), negative pressure wound therapy, targeted antibiotic therapy when indicated, and delayed hardware removal post-

Hip Fracture in the Sportive Adult: Case Report of Complete Functional Recovery After Removal of Hardware.

CONTEXT: Pertrochanteric hip fractures in sportive young adults are mainly caused by a high-energy trauma and treated in the same way as in the older population, using an osteosynthesis immediately followed by a rehabilitation program for several months. The current standard is not to remove osteosynthesis material, similar to the case of older patients. CASE PRESENTATION: A 45-year-old male cyclist experienced a right pertrochanteric femoral fracture, treated with cephalomedullary nails. After 9 months of adequate rehabilitation, weakness of the quadriceps musculature and functional complain

Radial Forearm Flap for Esophageal Perforation After Anterior Cervical Hardware Removal: Surgical Technique and Case Report.

CASE: A 51-year-old woman, who had previously undergone C5-C7 anterior cervical discectomy and fusion, presented with symptomatic hardware failure and subsequently underwent instrumentation removal. Her postoperative course was complicated by an esophageal perforation. Despite initial repair using a rotational flap, the leak persisted, prompting esophageal reconstruction with a radial forearm free flap (RFFF). CONCLUSION: Persistent esophageal perforation is exceedingly rare and difficult to treat. This report discusses the surgical technique for RFFF, an excellent option for revising failed

True antiglide fixation of Danis-Weber B fibula fractures has lower rates of removal of hardware.

INTRODUCTION: Plating of unstable OTA/AO 44-B2 equivalent lateral malleolus (Danis-Weber B) fractures has been associated with pain, peroneal irritation, and the need for subsequent hardware removal (ROH). The purpose of this study is to retrospectively compare the rates of ROH in unstable Weber B fractures fixed with a posterior, true antiglide plate with no screws in the distal segment versus those that were fixed with a similarly placed posterior, neutralization construct that included screws in the distal fragment. Thus, evaluating the role of the distal screws in rates of ROH. METHODS: S

Multiply Broken Metallic Hardware Removal for United Patella Fracture Applied Twenty Years Before: A Case Report.

Breakage of tension band wires, used to treat the patella fracture, is not uncommon several years after the fracture fixation. Broken wires may migrate to surrounding neurovascular structures, other vital organs like heart and may cause potentially fatal complications. Once the wires have been broken, it is very difficult to remove the broken pieces of metal wires. We report a 50 years old male patient with broken tension band wires at multiple sites for patella fracture. The broken wires were removed 20 years after the initial surgery without any undue complications, however patient sustained

Union Rate and Rate of Hardware Removal Following Plate Fixation of Metatarsal Shaft and Neck Fractures.

BACKGROUND: There have been very few studies related to the treatment of first, second, third, and fourth (MT) metatarsal shaft and neck fractures. In order to reduce metatarsal fracture malunion, many surgeons have turned from K-wire to plate fixation of these fractures. This study reports the healing rates, final fracture angulation, and need for hardware removal of operatively treated first to fourth MT shaft and neck fractures with plate fixation. METHODS: A retrospective review was performed on all metatarsal fractures at our institution between 2008 and 2014 to identify all first to fou

Complications in Craniovertebral Junction Instrumentation: Hardware Removal Can Be Associated with Long-Lasting Stability. Personal Experience.

BACKGROUND: The causes of craniovertebral junction (CVJ) instabilities include trauma, rheumatological diseases, tumors, infections, congenital malformations, and degenerative disease processes; these complex pathologies often require CVJ instrumentation. Hardware complications were analyzed in a personal series of 48 treated patients. In light of the analysis of very unusual radiological and clinical findings, the authors tried to better investigate the related mechanisms and to reach possible useful conclusions. METHODS: In a series of 48 patients who underwent CVJ instrumentation and fusio

Distal Positioning of the Ulnar Shortening Plate Increases Symptomatic Removal: A Comparative Analysis of Ulnar Shortening Osteotomy Plate Positioning and Symptomatic Hardware Removal.

BACKGROUND: Ulnar shortening osteotomy (USO) is a common procedure to treat symptomatic ulnar-sided wrist symptoms in patients with positive ulnar variance who fail nonoperative management. Plate placement on the volar, dorsal, or subcutaneous border of the ulna has been described. There remains debate regarding the optimal plate placement to minimize soft tissue irritation and the need for plate removal. The goal of this study is to determine whether plate position along the volar cortex versus along the subcutaneous border, as well as the distance of the plate from the ulnar styloid, affects

Decreased Risk of Periprosthetic Joint Infection with Concurrent Hardware Removal During Conversion Total Hip Arthroplasty Compared to Staged Removal: A Matched Cohort Analysis.

INTRODUCTION: Hardware removal before conversion total hip arthroplasty (cTHA) is a challenging task for the orthopaedic surgeon, although there is little consensus on the timing of hardware removal to mitigate risk of surgery-related complication following cTHA. METHODS: Using a national insurance database, we evaluated patients who underwent hardware removal either on the same day or within 1 year before cTHA, resulting in a total of 7,756 patients. After matching based on demographic factors and comorbidities, both staged and concurrent groups consisted of 2,752 patients. The 90-day and 1-

Cosmetic and Functional Dissatisfaction as Drivers of Elective Hardware Removal After Clavicle Fracture Fixation: A Narrative Review.

Clavicle fractures are a common orthopedic issue requiring a comprehensive approach to management. When surgery is indicated, postoperative concerns and discussions based on psychosocial measures should take place to help patients obtain the best individualized outcomes. Traditional indications for hardware removal include infection, mechanical failure, or significant pain, but patients can elect to undergo removal for non-clinical purposes such as hardware visibility, clothing interference, or activity limitations. These patient-centered indications for removal are currently underreported in

Superior clavicle plating using low-profile, precontoured locking plates has low complication and low hardware removal rate.

OBJECTIVE: The purpose of this study was to describe our experience using superiorly applied low-profile locking plates for the operative fixation of displaced midshaft clavicle fractures (MCFs). STUDY DESIGN: This was a retrospective analysis performed at an academic institution. RESULTS: Eighty-three patients who presented with displaced, shortened MCFs were treated operatively by a single surgeon at our institution over a 10-year period. All patients received a precontoured low-profile locking plate applied in the superior position. A displaced midshaft clavicle fracture was fixed operati

Development of a Synovial Fistula Through a Screw Hole Following Supramalleolar Osteotomy Hardware Removal: A Case Report.

A synovial fistula is the communication between the synovial space and the skin. In most cases, the fistula tract is located within the soft tissue; therefore, excision and closure of the fistula have been described as surgical treatment. Rarely, fistulas may form within the bone following procedures around the joint, such as core biopsy and bone tunneling for ligament reconstruction. In such cases, the insertion of materials filling the bone tunnel with cement or bone graft was introduced. This report describes a case of synovial fistula in the distal tibiofibular joint through a screw hole f

Association of hardware removal with secondary osteonecrosis following femoral neck fractures: a systematic review and meta-analysis.

BACKGROUND: It has been controversial that whether hardware removal will increase the risk of osteonecrosis of femoral head (ONFH) in fracture-healed patients who underwent internal fixation for femoral neck fractures (FNFs). This meta-analysis aimed to clarify the association of hardware removal with secondary hardware removal-induced ONFH (HR-ONFH). METHODS: Four electronic databases (PubMed, Embase, Web of Science, Cochrane Library) were searched for eligible studies published up to March 10, 2023. Studies reporting the relative risk of hardware status (i.e., risk rate, odds ratio [OR], or

Examination of the pronator quadratus muscle during hardware removal procedures after volar plating for distal radius fractures.

BACKGROUND: It is not clear whether the pronator quadratus (PQ) muscle actually heals and provides a meaningful pronation force after volar plating for distal radius fractures (DRFs). We aimed to determine whether the length of the PQ muscle, which is dissected and then repaired during volar plating for a DRF, affects the forearm rotation strength and clinical outcomes. METHODS: We examined 41 patients who requested hardware removal after volar plating. We measured the isokinetic forearm rotation strength and clinical outcomes including grip strength, wrist range of motion, and disabilities o

The benefits of hardware removal in patients with pain or discomfort after fracture healing of the ankle: a systematic review protocol.

INTRODUCTION: For any orthopaedic surgeon working with trauma; ankle fractures are one of the most common injuries treated. The treatment of ankle fractures can be conservative, using external fixation, but more commonly the fractures are treated with open reduction and internal fixation. Residual pain and discomfort are common in patients after surgical treatment of fractures of the ankle. Sometimes it is difficult to determine whether the pain or discomfort is due to the implants left in situ or the primary injury itself. In many cases, the decision is made to remove the implants. Subcutaneous emphysema of the leg after hardware removal and bone allografting for infected non-union of the distal femur.

BACKGROUND: Infected non-unions of the supracondylar region of the femur are uncommon. Even though hardware removal is a common procedure, it may lead to complications, including neurovascular injury, refracture, worsening pain or recurrence of deformity. CASE PRESENTATION: We report on a male who developed subcutaneous emphysema of the leg after hardware removal and bone allografting for an infected non-union of the distal femur. He was managed by debridement of the surgical wound, antibiotic therapy, multiple fasciotomies, and application of a VAC (vacuum-assisted closure) system. CONCLUSI

Incidence of Femur Fracture Post Hardware Removal in Children With Cerebral Palsy Who Have Undergone Varus Derotational Osteotomy.

BACKGROUND: Children with cerebral palsy (CP) often undergo hip reconstruction through proximal femoral varus derotation osteotomy (VDRO), with orthopaedic implants used for fixation. Posthealing, hardware may be retained or removed, either reactively due to complications or prophylactically. The controversy surrounding implant removal persists. This study aimed to compare the rate of proximal femoral fractures in children with CP who retained their hardware versus those who had it removed after VDRO. A secondary aim was to identify fracture risk factors after VDRO and report complications rel
