Disease-Specific Patterns of Multidimensional Video Head Impulse Test Parameters in Peripheral Vestibular Disorders.

Video Head Impulse Test (vHIT) multidimensional parameters-namely vestibulo‑ocular reflex (VOR) gain, gain asymmetry (GA), and compensatory saccades (CS)-are core indicators for assessing peripheral vestibular dysfunction. The combined use of vHIT and caloric testing facilitates differential diagnosis of various vertigo syndromes. In this retrospective study, 310 adult patients with unilateral peripheral vestibular disorders were enrolled, comprising 170 cases of Ménière's disease (MD), 110 cases of sudden sensorineural hearing loss accompanied by vertigo (SSHL‑V), and 30 cases of vestibular n

Exploratory saccades data analysis of video head impulse test in different Meniere's disease stages.

BACKGROUND: Unapparent saccades in video head impulse test (vHIT) are usually present in Meniere's disease (MD) patients but tend to be ignored by the clinician. The result of vHIT is constantly questioned in MD patients due to a lack of uniform metrics. A more effective indicator is needed for indicating MD's pathological progress. OBJECTIVES: To get a comprehensive understanding of the nature and usability of saccades in different MD stages. METHODS: 118 patients diagnosed with unilateral MD were recruited in this study. Patient history, audiological examination, caloric test, vHIT were pe

Comparison of three video head impulse test systems for the diagnosis of bilateral vestibulopathy.

INTRODUCTION: A horizontal vestibulo-ocular reflex gain (VOR gain) of < 0.6, measured by the video head impulse test (VHIT), is one of the diagnostic criteria for bilateral vestibulopathy (BV) according to the Báràny Society. Several VHIT systems are commercially available, each with different techniques of tracking head and eye movements and different methods of gain calculation. This study compared three different VHIT systems in patients diagnosed with BV. METHODS: This study comprised 46 BV patients (diagnosed according to the Báràny criteria), tested with three commercial VHIT systems (I

Computing Endolymph Hydrodynamics During Head Impulse Test on Normal and Hydropic Vestibular Labyrinth Models.

Hypothesis: Build a biologic geometry based computational model to test the hypothesis that, in some circumstances, endolymphatic hydrops can mechanically cause enhanced eye velocity responses during clinical conditions of the head impulse test. Background: Some recent clinical and experimental findings had suggested that enhanced eye velocity responses measured with the video head impulse test could not only be caused by recording artifacts or central disfunction but also could be directly caused by the mechanical effect of endolymphatic hydrops on horizontal semicircular canal receptor. Meth

Video Head Impulse Test to Preoperatively Identify the Nerve of Origin of Vestibular Schwannomas.

BACKGROUND: Identification of the nerve of origin in vestibular schwannoma (VS) is an important prognostic factor for hearing preservation surgery. Thus far, vestibular functional tests and magnetic resonance imaging have not yielded reliable results to preoperatively evaluate this information. The development of the video head impulse test (vHIT) has allowed a precise evaluation of each semicircular canal, and its localizing value has been tested for some peripheral vestibular diseases, but not for VS. OBJECTIVE: To correlate patterns of semicircular canal alteration on vHIT to intraoperativ

Video Head Impulse Test: A Prognostic Marker for Patients with Idiopathic Sudden Sensorineural Hearing Loss.

Background/Objectives: Patients with Idiopathic sudden sensorineural hearing loss (ISSNHL) are often devastated by the unknown etiology coupled with the unknown pathway to recovery. The aim of this study was to evaluate whether abnormalities on the video Head Impulse Test (vHIT) performed early in the course of ISSNHL are associated with poorer hearing recovery. Methods: Forty-four patients with ISSNHL were prospectively enrolled between 2019 and 2022 following exclusion of differential diagnoses on clinical and MRI evaluation. vHIT was performed within 1-14 days of symptom onset and within 48

Changes of video head impulse test before and after translabyrinthectomy in patients with acoustic neuroma.

OBJECTIVES: Acoustic neuroma (AN) is a benign tumor that usually affects a patient's hearing and balance function. For the screening and diagnosis of AN, the traditional approach mainly relies on audiological examination and magnetic resonance imaging (MRI), often ignoring the importance of vestibular function assessment in the affected area. As an emerging method of vestibular function detection, video head impulse test (vHIT) has been widely used in clinic, but research on its use in AN diagnosis is relatively limited. This study aims to explore the value of vHIT in the diagnosis of AN, vest

Detection of isolated covert saccades with the video head impulse test in peripheral vestibular disorders.

OBJECTIVES: The function of the semicircular canal receptors and the pathway of the vestibulo-ocular-reflex (VOR) can be diagnosed with the clinical head impulse test (cHIT). Recently, the video head impulse test (vHIT) has been introduced but so far there is little clinical experience with the vHIT in patients with peripheral vestibular disorders. The aim of the study was to investigate the horizontal VOR (hVOR) by means of vHIT in peripheral vestibular disorders. METHODS: Using the vHIT, we examined the hVOR in a group of 117 patients and a control group of 20 healthy subjects. The group of
