Is helmet therapy effective in positional plagiocephaly? A systematic review and proposal for an updated management algorithm.

Positional plagiocephaly, also known as deformational or non-synostotic plagiocephaly, is a common cranial deformity in infancy resulting from sustained external mechanical forces acting on the developing skull. Its prevalence has increased in recent decades, paralleling widespread adoption of supine sleep positioning recommendations. Although generally considered a benign condition, uncertainty persists regarding optimal management strategies, particularly the role and timing of cranial orthotic therapy. Conservative approaches, including repositioning techniques and physical therapy, are fre

Best Practices for Conducting Physical Reconstructions of Head Impacts in Sport.

Physical reconstructions are a valuable methodology for quantifying head kinematics in sports impacts. By recreating the motion of human heads observed in video using instrumented test dummies in a laboratory, physical reconstructions allow for in-depth study of real-world head impacts using well-established surrogates such as the Hybrid III crash test dummy. The purpose of this paper is to review all aspects of the physical reconstruction methodology and discuss the advantages and limitations associated with different choices in case selection, study design, test surrogate, test apparatus, te

The Protective Performance of Modern Motorcycle Helmets Under Oblique Impacts.

Motorcyclists are at high risk of head injuries, including skull fractures, focal brain injuries, intracranial bleeding and diffuse brain injuries. New helmet technologies have been developed to mitigate head injuries in motorcycle collisions, but there is limited information on their performance under commonly occurring oblique impacts. We used an oblique impact method to assess the performance of seven modern motorcycle helmets at five impact locations. Four helmets were fitted with rotational management technologies: a low friction layer (MIPS), three-layer liner system (Flex) and dampers-c

Biomechanical Analysis of Head Subjected to Blast Waves and the Role of Combat Protective Headgear Under Blast Loading: A Review.

Blast-induced traumatic brain injury (bTBI) is a rising health concern of soldiers deployed in modern-day military conflicts. For bTBI, blast wave loading is a cause, and damage incurred to brain tissue is the effect. There are several proposed mechanisms for the bTBI, such as direct cranial entry, skull flexure, thoracic compression, blast-induced acceleration, and cavitation that are not mutually exclusive. So the cause-effect relationship is not straightforward. The efficiency of protective headgears against blast waves is relatively unknown as compared with other threats. Proper knowledge

Comparison of Laboratory and On-Field Performance of American Football Helmets.

The relationship between laboratory and on-field performance of football helmets was assessed for 31 football helmet models selected from those worn by players in the 2015-2019 National Football League (NFL) seasons. Linear impactor tests were conducted with helmets placed on an instrumented Hybrid III head and neck assembly mounted on a sliding table. Based on impacts to each helmet at six impact locations and three velocities, a helmet performance score (HPS) was calculated using a linear combination of the head injury criterion (HIC) and the diffuse axonal multi-axis general evaluation (DAM

The effects of head mounted weight on comfort for helmets and headsets, with a definition of "comfortable wear time".

BACKGROUND: There are difficult tradeoffs when designing head-mounted equipment such as helmets, lights, cameras, or virtual or augmented reality displays. Increased functionality and battery life adds weight, which in turn reduces comfort. A successful product must balance both comfort and functionality to achieve its product engagement goals. OBJECTIVE: This study defines "comfortable wear time" as a new metric, and applies it to the domain of headsets in determining the relationship between headset weight and comfort. METHODS: Sixteen study participants wore four otherwise identical heads

[Plagiocephaly in children: etiology, differential diagnosis and helmet treatment].

Plagiocephaly in a head-and-neck irradiated rat model or rhomboid-shaped head, occurs in at least 1 in 300 live births. In most cases such asymmetry is not caused by synostosis of the unilateral coronal or lambdoid sutures, but is rather a deformity produced by intrauterine and/or postnatal deformational forces. Categorization and diagnosis of plagiocephaly as synostotic or deformational is reliably made by physical examination and computerized tomography. Its differential diagnosis is extremely important because prompt surgical correction is usually indicated for the synostotic type. In contr

Promoting bicycle helmets: a new focus for injury prevention.

Bicycle accidents cause many serious injuries and deaths in the United States, often as a result of head injury in riders not wearing helmets. Helmets reduce the incidence of head injury, and an aggressive safety education campaign has been shown to increase the number of children wearing helmets while riding. Such a campaign has been initiated in the Central Indiana area. The campaign is designed to educate families and to promote bicycle helmet use among children and their parents. Suggestions are given on how physicians can help educate, distribute information and encourage bicycle helmet u

Operation shadow warrior: a quantitative analysis of the ultraviolet radiation protection demonstrated by various headgear.

In the last 10 years, the scientific community and the media have highlighted the depletion of atmospheric ozone and the subsequent increase in ultraviolet radiation (UVR) that penetrates to the earth's surface. This UVR is associated with problems such as sunburn, photoaging, cataracts, and skin cancer. In the United States, skin cancer is among the top three reported cancers, and its incidence is increasing rapidly. Physicians agree that human exposure to UVR should be controlled by protecting the skin or limiting its exposure to UVR. As military physicians, our mission is to protect the fig

Neck muscle strain when wearing helmet and NVG during acceleration on a trampoline.

INTRODUCTION: The helmet-mounted equipment worn by military pilots increases the weight of the helmet system and shifts its center of gravity, increasing the loads on neck structures, especially during acceleration. The aim of this study was to determine neck muscle strain with different head-loads during trampoline-induced G loads (0 to +4 G). METHODS: Under three conditions [no helmet, helmet, helmet with night vision goggles (NVG)], 14 subjects performed trampoline exercises including basic, hand-and-knee, and back bouncing. EMG activity was measured for the sternocleidomastoid (SCM), cerv

Helmet effectiveness in preventing motorcycle driver and passenger fatalities.

Helmet effectiveness in preventing fatalities to motorcycle drivers and passengers was determined by applying the double pair comparison method to the Fatal Accident Reporting System (FARS) data for 1975 through 1986. Motorcycles with a driver and a passenger, at least one of whom was killed, were used. In order to reduce as much as possible potentially confounding effects due to the dependence of survivability on sex and age, the analysis is confined to male drivers (there were insufficient female driver data), and to cases in which the driver and passenger age do not differ by more than thre

Impact Mitigation in Modern Football Helmets: Advances and Limitations of Position-Specific Designs.

It has been established that repetitive head impacts lead to the accrual of changes in brain function, structure, and chemistry. The helmet is the primary piece of protective equipment for participants in American football. The first helmets were constructed from leather. Plastic helmets with chin straps were developed in the 1950s, but testing standards were not developed until 1973. While those standards resulted in improved padding materials, previous work demonstrated that modern helmets mitigate translational accelerations better than rotational accelerations, especially on a helmet's Rea

Potential of Soft-Shell Rugby Headgear to Mitigate Linear and Rotational Peak Accelerations.

Rugby union is a popular sport played across the world. The physical contact inherent in the game means that players are at increased risk of concussive injury. In 2019, World Rugby created a new category of permitted headgear under Law 4 as a medical device. This established a pathway for headgear designed to reduce peak accelerations to be worn in matches. Investigations of the potential of soft-shelled protective headgear to reduce head impact accelerations have been mostly limited to the analysis of linear kinematics. However rotational head impact accelerations have long been implicated a

Do bicycle safety helmets reduce severity of head injury in real crashes?

In the past, evaluation of helmet efficacy has been based on laboratory tests of limited relevance to real crashes. In the present study 894 South Australian bicycling enthusiasts returned mail questionnaires about their most recent bicycle crash and their helmet use at the time. 197 bicyclists reported a crash within the past five years in which they had struck their head or helmet. Helmet status at the time of the crash was reported as: no helmet used (n = 75), hairnet-style helmet (n = 69), hard-shell with soft or no liner (n = 37), or hard-shell helmet with stiff liner (n = 16). Capturing Head Impacts in Boxing: A Video-Based Comparison of Three Wearable Sensors.

Wearable sensors are used to quantify head impacts in athletes, but recent work has shown that the number of events recorded may not be accurate. This study aimed to compare the number of head acceleration events recorded by three wearable sensors during boxing and assess how impact type and location affect the triggering of acceleration events. Seven boxers were equipped with an instrumented mouthguard, a skin patch, and a headgear patch. Contacts to participants' heads were identified via three video cameras over 115 sparring rounds. The resulting 5168 video-identified events were used as re

A Review of On-Field Investigations into the Biomechanics of Concussion in Football and Translation to Head Injury Mitigation Strategies.

This review paper summarizes the scientific advancements in the field of concussion biomechanics in American football throughout the past five decades. The focus is on-field biomechanical data collection, and the translation of that data to injury metrics and helmet evaluation. On-field data has been collected with video analysis for laboratory reconstructions or wearable head impact sensors. Concussion biomechanics have been studied across all levels of play, from youth to professional, which has allowed for comparison of head impact exposure and injury tolerance between different age groups.

A modular impact diverting mechanism for football helmets.

To mitigate the injurious effect of the rotational acceleration of the brain, a modular Impact Diverting Mechanism (IDM) has been developed. The IDM can replace stickers (decals) that normally attach to the exterior of a football helmet. The IDM decals reduce friction and catch points between the covered area with the IDM on the outer shell of the helmet and the impacting surface, thereby decreasing rotational acceleration acting on the player's head. A Riddell Speed helmet's exterior was prepared with the IDM and outfitted to a headform equipped with linear accelerometers and gyroscopes. The
