Impact of government subsidy reforms on primary health care efficiency in rural eastern China: an interrupted time series analysis.

BACKGROUND: Despite rising government subsidies, inefficiencies persist in China’s primary health-care system. We evaluated an eastern rural pilot that shifted subsidies from passive grants to strategic purchasing via fixed salaries plus activity-based payments using resource-based relative value units (RVUs). METHODS: Interrupted time series (January 2015 - December 2022) assess the instantaneous and long-term effects of the reform (implemented January 2017) on monthly RVUs per employee across all primary health institutions (PHIs) and three sub-categories (central town/street PHIs, other PHI

COVID-19 Emergency Reforms in Massachusetts to Support Behavioral Health Care and Reduce Mortality of People With Serious Mental Illness.

People with serious mental illness are at disproportionate risk of COVID-19 morbidity and mortality because of high rates of risk factors that directly parallel those related to poor coronavirus outcomes, including smoking, chronic obstructive pulmonary disease, cardiovascular disease, and diabetes, along with housing instability, homelessness, food insecurity, and poverty. Community-based behavioral health organizations are also at risk of adverse outcomes because of dramatic declines in revenues and a diminished workforce. The State of Massachusetts has responded to this crisis by rapidly im

Cross-sector collaboration to reduce health inequalities: a qualitative study of local collaboration between health care, social services, and other sectors under health system reforms in England.

BACKGROUND: Policymakers across countries promote cross-sector collaboration as a route to improving health and health equity. In England, major health system reforms in 2022 established 42 integrated care systems (ICSs)-area-based partnerships between health care, social care, public health, and other sectors-to plan and coordinate local services. ICSs cover the whole of England and have been given explicit policy objectives to reduce health inequalities, alongside other national priorities. METHODS: We used qualitative methods to understand how local health care and social services organiza

Challenges of the general practitioner's role in integrated primary health care: a qualitative study of trainees in a reform-oriented network in Northern Italy.

BACKGROUND: Integrated primary health care (PHC) is essential for efficient, equitable healthcare, especially amid rising chronic conditions and an aging population. It ensures coordinated care across services, with GPs as key gatekeepers. Yet, European systems face obstacles like service fragmentation, workforce shortages, inadequate funding, digital barriers, inconsistent policies and poor integration across services. OBJECTIVE: This study explores the perspectives of trainee doctors in Northern Italy on the challenges of integrated PHC and suggests best practices to enhance integrated care

"A campaign won as a public issue will stay won": using cartoons and comics to fight national health care reform, 1940s and beyond.

On March 23, 2010, President Barack Obama signed the Patient Protection and Affordable Care Act (ACA) into law. As it went through Congress, the legislation faced forceful resistance. Individuals and organizations opposing the ACA circulated propaganda that varied from photographs of fresh graves or coffins with the caption "Result of ObamaCare" to portrayals of President Obama as the Joker from the Batman movies, captioned with the single word "socialism." The arguments embedded in these images have striking parallels to cartoons circulated by physicians to their patients in earlier fights ag

Stigma of mental illness in China: Exploring the intersectional effects of gender, ethnicity and disorder at the start of national mental health care reform.

Drawing on a large-scale survey in China, we examine how the unequal position of women, the political/cultural prejudice facing the Uyghur ethnic community, and the type of mental health problem simultaneously shaped public stigma from the majority Han Chinese population during the first quarter of this century. The Stigma in Global Context - Mental Health Study in China (N = 5610) deployed a survey experimental design, finding significant, unexpected and complex intersectional effects. For Uyghurs, being female and with schizophrenia (vs. depression) resulted in the highest levels of social d

Sexually transmitted infection clinics as safety net providers: exploring the role of categorical sexually transmitted infection clinics in an era of health care reform.

BACKGROUND: For many individuals, the implementation of the US Affordable Care Act will involve a transition from public to private health care venues for sexually transmitted infection (STI) care and prevention. To anticipate challenges primary care providers may face and to inform the future role of publicly funded STI clinics, it is useful to consider their current functions. METHODS: Data collected by 40 STI clinics that are a part of the Sexually Transmitted Disease Surveillance Network were used to describe patient demographic and behavioral characteristics, STI diagnoses, and laborator

Interdisciplinary training on noncommunicable diseases and sustained change in primary health care in the context of health reform, COVID-19, and war in Ukraine: A repeated qualitative study, 2016-2022.

BACKGROUND: Noncommunicable diseases (NCDs), account for nearly 90% of deaths in Ukraine, led by cardiovascular diseases and cancers. Common risk factors include tobacco and alcohol use, unhealthy diet, and physical inactivity. To improve the NCD care at the primary health care (PHC) level, the Ministry of Health and World Health Organization (WHO) introduced a two-day interdisciplinary training intervention for PHC providers - managers, doctors, nurses, and feldshers, grounded in the WHO Package of Essential Noncommunicable Disease Interventions (WHO PEN). Conducted across seven pilot regions

[The value of prophylactic serological tests for syphilis in the region of Białystok in years 1994-2004 (before and after the introducing the health care system reform in Poland)].

The prophylactic serological tests for syphilis (psts), belong to the principal methods of controling the spread of the disease. The aim of the study was to to analyze the epidemiological efficacy of psts performed in Bialystok region before and after the health care system reform. The results of psts performed in 1994-1998 (N = 752.787) and in 1999-2004 (N = 407.366) were analyzed. The average annual number of psts declined from 150.557 to 68.687 in two periods analyzed, respectively. The lowest number of tests performed (51.654) was noticed in 2004. Primary health care and equity: the case of lesbian, gay, bisexual, trans and intersex Australians.

The current period of health reform in Australia offers an opportunity for positive actions to be taken to address the significant challenges that lesbian, gay, bisexual, trans, intersex and other sexuality, sex and gender diverse (LGBTI) people face in the health system. This paper provides analysis of why this group should be considered a priority health group using a social determinants of health framework, which has, to date, largely been ignored within primary health care policy reform in Australia. Several key areas of the primary health care reform package are considered in relation to

Understanding pathways from primary health care to universal health coverage outcomes: a realist review.

INTRODUCTION: Primary Health Care (PHC) rooted in community empowerment, integrated service delivery, and multisectoral action is central to efforts toward Universal Health Coverage (UHC), yet little is known about how PHC reforms influence UHC outcomes across contexts. This review examines how PHC approaches affect population coverage, service coverage, and financial protection by identifying what works, for whom, under what circumstances, and why. METHODS: A realist review was conducted using studies from countries with the greatest gains in the UHC Service Coverage Index between 2000 and 2

Music in the Park. An integrating metaphor for the emerging primary (health) care system.

BACKGROUND: Metaphors are central to the human understanding of complex issues; through the immediate associations they evoke and frame problems and suggest solutions. Our suggestion of Music in the Park as a metaphor for health systems reform brings to the forefront the environmentally diverse but bounded spaces of health services that offer a variety of attractors within their confines, while pushing into the background organizational and economic concerns. REFLECTIONS: Parks, like health services, are embedded in their local landscape, serving their communities, but most importantly parks

Free establishment of primary health care providers: effects on geographical equity.

BACKGROUND: A reform in 2010 in Swedish primary care made it possible for private primary care providers to establish themselves freely in the country. In the former, publicly planned system, location was strictly regulated by local authorities. The goal of the new reform was to increase access and quality of health care. Critical arguments were raised that the reform could have detrimental effects on equity if the new primary health care providers chose to establish foremost in socioeconomically prosperous areas. The aim of this study is to examine how the primary care choice reform has affec

Oral health can be a driver of the UK health transformation agenda.

Oral health is a critical yet underutilised lever for health system transformation in the UK. Despite being among the most prevalent noncommunicable diseases (NCDs), oral conditions remain dominated by costly, intervention-based care models. The World Health Organization's Global Oral Health Action Plan (2023-2030) and recent UK health reform agendas present an opportunity to embed oral health within primary care and universal health coverage frameworks. This paper argues for a paradigm shift toward prevention, integration, and equity, addressing shared risk factors across NCDs through team-ba

Access to outpatient psychotherapy and utilisation of services after structural reform in Germany: analysis of administrative claims data for people with depression.

BACKGROUND: Early initiation of therapy can be crucial for the course of mental health problems. Among other new care elements, the structural reform of the outpatient psychotherapy system in Germany introduced psychotherapeutic consultations to enable early low-threshold access to psychotherapists. This study aims to compare access to outpatient psychotherapeutic care for patients diagnosed with a first-time diagnosis of depression before and after the reform, and to analyse the utilisation of other relevant health services. METHODS: A retrospective cohort-based analysis was conducted using

The Caribbean Netherlands: Health System Review.

This analysis of the health system of the Caribbean Netherlands reviews developments in governance, organization, financing and delivery of care, health reforms and health system performance on the islands of Bonaire, St Eustatius and Saba (the BES islands). Since the dissolution of the Netherlands Antilles in 2010, residents of the BES islands live in special municipalities of the Netherlands, each with its own government (public entity). The Ministry of Health, Welfare, and Sport (VWS) in The Hague, through the Department of Care and Youth Caribbean Netherlands (ZJCN), oversees the health sy
