Advancing Health Equity in the Cardiovascular Device Life Cycle.

Despite advancements in diagnostics and therapeutics for cardiovascular disease, significant health disparities persist among patients from historically marginalized racial and ethnic groups, women, individuals who are socioeconomically under-resourced or underinsured, and those living in rural communities. While transcatheter interventions have revolutionized the treatment landscape in cardiology, populations bearing the greatest burden of disease continue to face inequitable access and poorer outcomes. A notable gap in the literature concerns the role of modern approaches to cardiovascular d

Teaching emergency medicine residents health equity through simulation immersion.

OBJECTIVE: Our aim was to conduct a large, case-based diversity, equity, and inclusion (DEI) simulation exercise with a goal to improve the DEI pillars of cultural and structural awareness for residents. METHODS: Utilizing data resulting in poor health outcomes, the top eight themes were utilized, and via a modified Delphi approach, a diverse group of faculty developed representative cases. A mass simulation effort was organized with the assistance of our local simulation office. Twenty residents in groups of two to three rotated through all scenarios. Each resident group was allotted 15 min

Health Equity Among Black Women in the United States.

Black women in the United States have experienced substantial improvements in health during the last century, yet health disparities persist. These health disparities are in large part a reflection of the inequalities experienced by Black women on a host of social and economic measures. In this paper, we examine the structural contributors to social and economic conditions that create the landscape for persistent health inequities among Black women. Demographic measures related to the health status and health (in)equity of Black women are reviewed. Current rates of specific physical and mental

Eliminating Equity: A Culture-Centered Reading of the Attack on Health Equity in Project 2025's Mandate for Leadership.

Project 2025's Mandate for Leadership proposes a sweeping reorganization of the United States Department of Health and Human Services (HHS). This article reads the Mandate's HHS chapter through the culture-centered approach (CCA), joining its account of structure, culture, agency, voice, and communicative inequality to a critical discourse analysis of the chapter's mission statement, its five governing goals, and its data provisions. The chapter does more than oppose equity policies. It reorganizes the conceptual and infrastructural conditions under which health inequity can be named, measured

Impacts of Colorado's Regional Health Connector Workforce on Health-Related Social Needs and Health Equity.

BACKGROUND: Workforce roles that address social determinants of health, such as regional health connectors (RHCs), can reduce health inequities that disproportionately affect people of lower socioeconomic status and marginalized communities. OBJECTIVES: The aim of this evaluation was to describe, from the perspective of RHCs' community partners, 1) strategies that RHCs employ, 2) social needs that RHCs address, and 3) outcomes associated with RHCs' community efforts. METHODS: This convergent mixed-methods evaluation included a cross-sectional statewide survey of Colorado RHC partners followe

Health equity in rheumatology-the uneven landscape of cellular therapy.

Cellular therapies may effectively "reset" the immune system and fundamentally alter the long-term course of chronic autoimmune rheumatic diseases. Clinical application in systemic lupus erythematosus resulting in drug-free remission has driven inquiry into other rheumatic disorders-systemic sclerosis, inflammatory myositis, Sjogren's disease-with early signals of significant therapeutic response. The substantial costs, complex manufacturing processes, and need for intensive patient monitoring limit cellular therapy delivery to certified academic centers-making it out of reach for many patient

The ethical imperative to pursue neonatal health equity and justice.

There are significant racial and ethnic inequities in neonatal care. These inequities are the result of the multiple levels of racism: structural, institutional, interpersonal, and internalized racism. Recently, there has been significantly increased attention to measuring and addressing inequities for minoritized infants and their families. One way to support these strategies is to clarify the bioethical justification for allocating clinical, research and educational resources to address inequity. In this narrative review, we describe two bioethical frameworks-the four pillars and the ethics

The Appropriate Translation of the Nurse Practitioner Will Foster Health Equity.

Background: In September 2020, Governor Gavin Newsom of California signed Assembly Bill 890 (AB 890) into law, which changed Title 16 of the Nurse Practice Act, granting California nurse practitioners (NPs) a stepwise approach to full practice authority. Written within the law, NPs are required to verbally identify themselves specifically to all Spanish-speaking patients, as enfermera(o) especializada (o). Local Problem: AB 890 requires NPs to identify themselves using inappropriate Spanish terminology. There are concerns regarding the translational accuracy of nurse practitioners under AB 890

Sexual and Reproductive Justice and Health Equity for LGBTQ+ Women.

LGBTQ+ women have long been overlooked in sexual and reproductive health research. However, recent research has established that LGBTQ+ women have unique and specific needs that need to be addressed in order to improve effectiveness of sexual health education and practice with this historically and presently underserved population. Informed by a reproductive justice framework coupled with liberation psychology theory, this review discusses the current state of sexual and reproductive health and technologies among LGBTQ+ women. In particular, we focus on a range of HIV prevention and reproducti

Considerations for addressing bias in artificial intelligence for health equity.

Health equity is a primary goal of healthcare stakeholders: patients and their advocacy groups, clinicians, other providers and their professional societies, bioethicists, payors and value based care organizations, regulatory agencies, legislators, and creators of artificial intelligence/machine learning (AI/ML)-enabled medical devices. Lack of equitable access to diagnosis and treatment may be improved through new digital health technologies, especially AI/ML, but these may also exacerbate disparities, depending on how bias is addressed. We propose an expanded Total Product Lifecycle (TPLC) f

Penicillin Allergy Evaluation and Health Equity: A Call to Action.

Allergists have been at the forefront of addressing the burden of unverified penicillin allergy labels. Coordinated national efforts with infectious diseases, antimicrobial stewardship experts, and pharmacy societies to advocate for formal evaluation of patient-reported penicillin allergy have resulted in improvements in delabeling efforts. Given the poorer health outcomes associated with the penicillin allergy label and the potential health benefits that can be gained with delabeling, improving access to penicillin allergy evaluation is of the utmost importance. Health disparities are widely

Head and Neck Cancers and Health Equity in the United States.

Persistent inequities in cancer care in the United States shape outcomes across the entire cancer continuum, and head and neck cancers offer a compelling lens through which to examine these structural drivers. In their population-based analysis, Tortolero and colleagues demonstrate that non-Hispanic Black and Hispanic patients with oral cavity cancer experience markedly higher odds of late-stage diagnosis, with neighborhood-level social vulnerability further magnifying these disparities. These findings reinforce that delayed diagnosis reflects not individual behaviors but broader determinants,

Health Equity and Secukinumab for Severe Psoriasis in New Zealand.

BACKGROUND/OBJECTIVES: The only funded indication in New Zealand between October 2018 and April 2021 for the biologic secukinumab, an interleukin-17A antibody, was severe psoriasis. This unique period of limited availability of secukinumab allowed the evaluation of who received it and health equity. METHODS: Anonymised dispensing data were obtained from the New Zealand Ministry of Health and matched with the New Zealand deprivation index, ethnicity and census data. RESULTS: Compared to the most socio-economically deprived, the least deprived were significantly more likely to receive secukinu

Immediate Antiretroviral Therapy: The Need for a Health Equity Approach.

Immediate antiretroviral therapy (iART), defined as same-day initiation of ART or as soon as possible after diagnosis, has recently been recommended by global and national clinical care guidelines for patients newly diagnosed with human immunodeficiency virus (HIV). Based on San Francisco's Rapid ART Program Initiative for HIV Diagnoses (RAPID) model, most iART programs in the US condense ART initiation, insurance acquisition, housing assessment, and mental health and substance use evaluation into an initial visit. However, the RAPID model does not explicitly address structural racism and homo

Promoting health equity through health literacy interventions in educational institutions: a scoping review.

BACKGROUND: Disparities in health outcomes have been associated with inequitable access to health literacy and services. Additionally, reduced health literacy may lead to an unhealthy lifestyle, causing a decrease in quality of life and social inequities. Advancing health literacy is vital in educational organizations to curb gaps in social inequities. Adapting strategies to promote health literacy is essential for advancing equitable and effective educational organizations. Therefore, policies and interventions should aim to address any barriers that threaten inequities in the educational org

Adolescent Psychological Assets and Cardiometabolic Health Maintenance in Adulthood: Implications for Health Equity.

Background Positive cardiometabolic health (CMH) is defined as meeting recommended levels of multiple cardiometabolic risk factors in the absence of manifest disease. Prior work finds that few individuals-particularly members of minoritized racial and ethnic groups-meet these criteria. This study investigated whether psychological assets help adolescents sustain CMH in adulthood and explored interactions by race and ethnicity. Methods and Results Participants were 3478 individuals in the National Longitudinal Study of Adolescent Health (49% female; 67% White, 15% Black, 11% Latinx, 6% other [N

School-Based Health Centers to Advance Health Equity: A Community Guide Systematic Review.

CONTEXT: Children from low-income and racial or ethnic minority populations in the U.S. are less likely to have a conventional source of medical care and more likely to develop chronic health problems than are more-affluent and non-Hispanic white children. They are more often chronically stressed, tired, and hungry, and more likely to have impaired vision and hearing-obstacles to lifetime educational achievement and predictors of adult morbidity and premature mortality. If school-based health centers (SBHCs) can overcome educational obstacles and increase receipt of needed medical services in

Africa's Place in Precision Medicine: Implications for Global Health Equity and Health Systems.

Precision medicine, which is based on the idea of tailoring health care for individual genetic, environmental, and lifestyle factors, is rapidly revolutionizing biomedical research and clinical practice across the world. Despite the progress across the world, Africa only has a limited representation in precision medicine research and implementation. This view looks at where Africa stands today in the precision medicine sphere and identifies gaps in genomic data, infrastructure, workforce capacity, and policy frameworks. It examines the implications of these gaps for health equity globally the
