Rapid Assessment of Ebola-Related Implications for Reproductive, Maternal, Newborn and Child Health Service Delivery and Utilization in Guinea.

INTRODUCTION: Since March 2014, Guinea has been in the midst of the largest, longest, and deadliest outbreak of Ebola Virus Disease ever recorded. Due to sub-optimal health conditions prior to the outbreak, Guinean women and children may have been especially vulnerable to worsening health care conditions. A rapid assessment was conducted to better understand how the delivery and utilization of routine RMNCH services may have been affected by the extraordinary strain placed on the health system and its client population by the Ebola outbreak in Guinea. METHODS: Data were collected January-Febr

Obtaining Longitudinal Built Environment Data Retrospectively across 25 years in Four US Cities.

BACKGROUND: Neighborhood transportation infrastructure and public recreational facilities are theorized to improve the activity, weight, and cardiometabolic profiles of individuals living in close proximity to these resources. However, owing to data limitations, there has not been adequate study of the influence of timing and placement of new infrastructure on health over time. METHODS: This protocol details methods of the four cities study to perform retrospective field audits in order to capitalize on existing longitudinal health data from the coronary artery risk development in young adult

Facility-Based Delivery during the Ebola Virus Disease Epidemic in Rural Liberia: Analysis from a Cross-Sectional, Population-Based Household Survey.

BACKGROUND: The Ebola virus disease (EVD) epidemic has threatened access to basic health services through facility closures, resource diversion, and decreased demand due to community fear and distrust. While modeling studies have attempted to estimate the impact of these disruptions, no studies have yet utilized population-based survey data. METHODS AND FINDINGS: We conducted a two-stage, cluster-sample household survey in Rivercess County, Liberia, in March-April 2015, which included a maternal and reproductive health module. We constructed a retrospective cohort of births beginning 4 y befo

Deinstitutionalization and mortality: findings of a controlled research design in New Jersey.

In previous studies of the mortality of deinstitutionalized persons with disabilities in California and Pennsylvania, investigators failed to employ a prospective controlled research design. We identified a sample of 150 "movers" scheduled for placement from an institution and a matched sample of 150 institutional "stayers." The matching and other risk variables were measured in 1994. Visits to all residences occurred between 1997 and mid-2000 at specified intervals after movers left the institution. Logistic regression analyses identified the 1994 preclosure risk variables of age, low self-ca

Cecal volvulus following appendectomy in a teenage patient: A case report.

UNLABELLED: Patients mimicking appendicitis symptoms in a rural setting or those post-appendectomy, indicating cecal volvulus, should always be considered. Swift action can prevent catastrophic consequences. ABSTRACT: We present a case of a 14-year-old female who initially underwent open appendectomy for acute appendicitis and subsequently experienced symptoms of abdominal distention, vomiting, and fever. Her condition deteriorated following the appendectomy, despite a prior appendectomy for similar symptoms at a different facility. A computed tomography (CT) scan identified cecal volvulus as

Implementation of Bundled Interventions to Reduce Surgical Site Infections in Pediatric Patients Undergoing Cardiothoracic Surgery: A Quality Improvement Project.

Pediatric patients undergoing cardiothoracic surgery are at increased risk for health care-associated infections, especially surgical site infections (SSIs). Delayed sternal closures are associated with prolonged lengths of stay and contribute to infection risks and morbidities. At a single-site academic pediatric center, we created an SSI-prevention bundle targeting the preoperative, intraoperative, and postoperative care periods. Using standard criteria, we determined the epidemiology of SSIs and measured SSI rates per 100 procedures from 2017 through 2023 before and after bundle implementat

The Effect of Ebola Virus Disease on Maternal and Child Health Services and Child Mortality in Sierra Leone, 2014-2015: Implications for COVID-19.

During Sierra Leone's 2014-2015 Ebola virus disease (EVD) epidemic, early reports warned of health system collapse and potential effects on other-cause mortality. These same warnings are reverberating during the COVID-19 pandemic. Consideration of the impacts of EVD on maternal and child health services from facility data can be instructive during COVID-19. We surveyed all peripheral healthcare units (PHUs) in Sierra Leone in October 2014 and March 2015 to assess closures, staffing, amenities, medicines, supplies, and service utilization during May 2014-January 2015 and October 2013-January 20

COVID-19: Were Public Health Interventions and the Disclosure of Patients' Contact History Effective in Upholding Social Distancing? Evidence from South Korea.

BACKGROUND: The coronavirus disease 2019 (COVID-19) pandemic has prompted a global-scale public health response. Social distancing, along with intensive testing and contact tracing, has been considered an effective vehicle to reduce new infections. In this study, we aimed to estimate the effect of South Korean public health measures on behavioral changes with respect to social distancing without a nationwide lockdown. The results of this study may provide insights to countries who are planning to relax their aggressive restrictions though still having an unflattened curve of infections. METHO

Quantitative risk assessment for the induction of allergic contact dermatitis: uncertainty factors for mucosal exposures.

BACKGROUND: Early in South Africa's HIV response, donor-funded organizations directly provided HIV treatment through Comprehensive HIV Care, Management and Treatment sites (CCMTs), using their own and government staff. From 2012 to 2014 the donor-funded CCMT model was phased out, leaving nurses in South Africa's public clinics responsible for delivery of antiretroviral treatment (ART) services. We aimed to examine the impact on resources, staff workloads, and service delivery throughout this period of integration of HIV treatment into primary health clinics. METHODS: We conducted an Interrupt

Delayed primary wound closure in upper extremity soft tissue infections.

Health care costs for inpatient care have been escalating. Homeless and indigent patients may be unable to have access to clean toilet facilities to perform adequate wound care. Evaluation of delayed primary closure of upper extremity soft tissue infections after incision and drainage in 34 patients was done. Patients were discharged at an average of 8.3 days (range, 5-21 days). This is 11 days less than the reported average granulation time for wounds to heal. No patient required readmission for surgery, wound care, or intravenous antibiotics. The authors' institution charges a rate of $2800

Rural Hospital Closures and Mortality From Time-Sensitive Conditions in Texas.

IMPORTANCE: Rural hospital closures in the US have accelerated since 2010, with Texas experiencing more rural closures than any other state. Whether closures are associated with population-level mortality remains unclear, and rural hospital closures in Texas have not been examined. OBJECTIVE: To examine the association between rural hospital closures in Texas and county-level mortality from 4 time-sensitive conditions: acute myocardial infarction, stroke, sepsis, and asthma or chronic obstructive pulmonary disease. DESIGN, SETTING, AND PARTICIPANTS: This county-year cohort study of Texas cou
