Urbanization-Associated Microbial Diversity Loss and Its Impact on Elderly Health: Integrating Environmental and Host Microbiome Dynamics.

Urbanization is strongly associated with microbial diversity loss in both built environments and elderly gut microbiomes, with significant health consequences. This study across 12 rural-to-urban care facilities in eastern China analysed 320 environmental and 120 faecal samples. Urbanization progressively reduced microbial alpha diversity in air, surfaces, and resident guts. Urban environments showed increased opportunistic pathogens and antibiotic resistance genes (ARGs), alongside decreased beneficial taxa and short-chain fatty acid pathways. Strong environment-host microbiome concordance wa

"There is no place to dispose them. What would you have me do?'': A qualitative study of menstruation in the unique physical and social environment in informal settlements in Nairobi, Kenya.

Menstrual Health and Hygiene (MHH) is an essential component of individual and population-level health and is inextricably linked to the environment. Few scholars have explored women's day-to-day experiences of managing their monthly period within the unique environment of informal settlements. We used data from in-depth interviews with women 18-55 years in Mathare informal settlement in Nairobi, Kenya. Findings suggest that aspects of the social environment, particularly menstrual stigma and social and gender inequities, combined with resource limitations, such as lack of water, sanitation, d

Evaluation of individual and facility factors that promote hand washing in aged-care facilities in Japan.

Internationally, it has been found that regular and timely hand washing is part of hand-hygiene practices that can reduce rates of infection in health-care facilities, but research has shown that there is a low level of compliance with hand washing in hospitals worldwide, including Japan. The number of aged-care facilities is growing throughout the world as our populations age, but hand-washing compliance appears even lower in such settings where there are vulnerable and frail elderly persons. This study used a correlational, cross-sectional design to clarify, for the first time, individual an

Losing women along the path to safe motherhood: why is there such a gap between women's use of antenatal care and skilled birth attendance? A mixed methods study in northern Uganda.

BACKGROUND: Thousands of women and newborns still die preventable deaths from pregnancy and childbirth-related complications in poor settings. Delivery with a skilled birth attendant is a vital intervention for saving lives. Yet many women, particularly where maternal mortality ratios are highest, do not have a skilled birth attendant at delivery. In Uganda, only 58 % of women deliver in a health facility, despite approximately 95 % of women attending antenatal care (ANC). This study aimed to (1) identify key factors underlying the gap between high rates of antenatal care attendance and much l

Measuring family planning quality and its link with contraceptive use in public facilities in Burkina Faso, Ethiopia, Kenya and Uganda.

The individual impacts of several components of family planning service quality on contraceptive use have been studied, but the influence of a composite measure synthesizing these components has not been often investigated. We (1) develop a composite score for family planning service quality based on health facility data from Burkina Faso, Ethiopia, Kenya and Uganda and (2) examine the influence of structural quality on contraceptive practice in these four countries. We used nationally representative cross-sectional survey data of health facilities and women of reproductive age. First, we cons

Compliant flooring to prevent fall-related injuries: a scoping review protocol.

INTRODUCTION: Fall-related injuries can have serious consequences for older adults, including increased risk of dependence in daily activities and mortality. Compliant flooring is a passive intervention that may reduce the incidence and severity of fall-related injuries in healthcare settings, including acute and long-term care, but few sites have implemented compliant flooring, in part because synthesised evidence about key performance aspects has not been available. METHODS AND ANALYSIS: We will conduct a scoping review to address the question: what is presented about the biomechanical effi

Epidemic Legionnaires' disease two decades later: old sources, new diagnostic methods.

In July 1995 we investigated a pneumonia outbreak in a Pennsylvania town. We conducted epidemiological and molecular microbiological studies to determine the outbreak source and interrupt transmission of disease. Legionnaires' disease (LD) was quickly identified by urine antigen testing, and a newly developed immunohistochemical stain confirmed nosocomial transmission to a hospital inpatient. LD was confirmed in 22 patients. Case-patients were more likely than controls to have been within 1,000 feet of the hospital (matched odds ratio, 21.0; 95% confidence interval, 2.9-368) during the 2 weeks

Antimicrobial-Resistant Enterobacterales Recovered from the Environment of Two Zoological Institutions Include Enterobacter cloacae Complex ST171 Producing KPC-4 Carbapenemase.

Environmental surfaces can serve as reservoirs for pathogens and antimicrobial-resistant (AMR) bacteria in healthcare settings. Although active surveillance programs are used in veterinary and human healthcare, unconventional settings like zoological facilities are often overlooked, even though antimicrobials are used to maintain the health of their animal collections. Here, we used electrostatic cloths to conduct active environmental surveillance over a 2-year period at two zoological institutions to determine contamination prevalence of human-only and mixed animal-human touch environments wi

The role of NGOs in mainstreaming services for podoconiosis into healthcare facilities: the case of two NGOs in Ethiopia.

BACKGROUND: Non-Governmental Organizations (NGOs) have played an important role in supporting progress towards the goals of the WHO Neglected Tropical Disease (NTD) Roadmap 2021-30. Several NGOs have shifted from directly delivering NTD services to mainstreaming them to existing health systems. There is little information about the NGOs themselves – their organisational environments and their influence in mainstreaming NTD services and associated challenges. We studied two NGOs that have jointly implemented an intervention to mainstream services for the NTD podoconiosis to local healthcare fac

Types of intensive care units with the healthiest, most productive work environments.

BACKGROUND: The quality of nurses' work environments in hospitals is of great concern. The American Association of Critical-Care Nurses has specified 6 standards essential to a healthy (ie, satisfying and productive) work environment. These standards are sufficiently aligned to the Essentials of Magnetism processes to make this tool suitable for measuring healthy work environments. OBJECTIVES: To identify differences in staff nurses' perceptions of the work environment by type of intensive care unit. METHODS: A cross-sectional descriptive design with strategic sampling was used in this secon

Urban planning, informal settlements, and maternal health outcomes: Investigating the role of built environment in antenatal and postnatal care utilization in China.

China's rapid urbanization has spawned extensive informal settlements, where millions of migrant families live in overcrowded, underserved areas. These environments, characterized by inadequate housing, poor sanitation, unsafe conditions, and limited healthcare access, significantly impede the utilization of maternal healthcare, specifically Antenatal (ANC) and Postnatal Care (PNC). This study examines how built environment characteristics influence ANC and PNC use among women in these settlements across four major Chinese cities. A cross-sectional study collected data from 800 women who had g

Improving primary health care facility performance in Ghana: efficiency analysis and fiscal space implications.

BACKGROUND: Health centers in Ghana play an important role in health care delivery especially in deprived communities. They usually serve as the first line of service and meet basic health care needs. Unfortunately, these facilities are faced with inadequate resources. While health policy makers seek to increase resources committed to primary healthcare, it is important to understand the nature of inefficiencies that exist in these facilities. Therefore, the objectives of this study are threefold; (i) estimate efficiency among primary health facilities (health centers), (ii) examine the potent

Lessons Learned from COVID-19 Response in Correctional and Detention Facilities.

The COVID-19 pandemic disproportionately affected persons held in and working in correctional and detention facilities, causing facilities' traditional priorities to shift when healthcare and public health needs temporarily drove many aspects of operations. During July-August 2022, we interviewed members of health departments and criminal justice organizations to document lessons learned from the COVID-19 response in correctional settings. Participants valued enhanced partnerships, flexibility, and innovation, as well as real-time data and corrections-specific public health guidance. Challenge

Psychometric validation of a multidimensional health facility HIV stigma instrument among healthcare workers in Taiwan.

BACKGROUND: HIV-related stigma within healthcare settings remains a major barrier to HIV prevention, treatment engagement, and equitable care. Existing stigma instruments in Taiwan primarily assess individual-level attitudes and do not adequately capture institutional or structural determinants of stigma. A multidimensional health facility-level stigma instrument is needed to support routine monitoring and stigma-reduction efforts. This study aimed to culturally adapt and psychometrically validate a Traditional Chinese version of the WHO/USAID Health Facility Staff HIV Stigma and Discriminatio
