Is referral source a risk factor for coronary surgery? Health maintenance organization versus fee-for-service system.

We began performing coronary artery bypass grafting for a large health maintenance organization (HMO) in 1974, as the sole provider of their cardiac surgery. The outcomes of our HMO group of patients were compared with those of our patients treated on a fee-for-service (FFS) basis. The HMO system entails preintervention and multidisciplinary screening conferences and is devoid of self-referral and personal financial incentives. Since 1985, the operative mortality for HMO patients has been consistently lower than for FFS patients. There were 8483 operations during this study period: 3168 (37%)

Long term outcomes after COVID-19 in patients with schizophrenia: a historical cohort study in a health maintenance organization.

BACKGROUND: Severe mental illness may affect health behaviors and outcomes during pandemics. Few studies have assessed whether people living with schizophrenia spectrum disorders (SSD) experienced adverse COVID-19 outcomes. METHODS: In a population-based historical cohort study comprising members of a health maintenance organization, we included 1273 patients with SSD and 12,730 age- and sex-matched controls tested for SARS-CoV-2 between March 2020 and May 2022. We assessed the association between schizophrenia and hospitalization, hospital length-of-stay, 30-day, and one-year mortality, cons

Implementation of a Hepatitis C Screening Program for At-Risk Former Soviet-Bloc Immigrants in a Large Health Maintenance Organization.

BACKGROUND: Hepatitis C virus (HCV) is a leading cause of liver disease worldwide. In 2021, Israel's Ministry of Health launched a national program to screen high-risk populations, especially immigrants from former Soviet Union countries, aiming to reduce HCV-related morbidity and mortality. This study evaluates the success of Maccabi Health Services (MHS) in implementing the screening program before and after the intervention, focusing on detection, treatment, and cure rates. METHODS: A retrospective interrupted time-series cohort study was conducted among MHS members born in former Soviet U

Vaccination Coverage among Immunocompromised Patients in a Large Health Maintenance Organization: Findings from a Novel Computerized Registry.

Immune-compromised patients (IPs) are at high risk for infections, some of which are preventable by vaccines. Specific vaccines are recommended for IP; however, the vaccination rate is suboptimal. The aim of this study is to describe the development of an IP registry and to assess vaccination rates in this population. A population-based registry of IPs was developed using an automated extraction of patient electronic health-record data in Maccabi Healthcare Services (MHS), an Israeli health maintenance organization serving over 2.4 million members. Included in the registry were patients receiv

Comparison of Perceptions and Smoking Cessation Experiences Between Smokers With and Without Serious Mental Illness in a Large Health Maintenance Organization.

OBJECTIVE: Smoking prevalence is threefold higher among people with serious mental illness (PWSMI) than in the general population, yet smoking cessation rates for PWSMI are lower. Numerous reasons have been posited as to why tobacco use is higher and abstinence rates are lower among PWSMI. This study explores smoking cessation perceptions and experiences among PWSMI and people without serious mental illness (SMI). METHODS: Participants in this cross-sectional study were recruited from among members of a large health maintenance organization (HMO) in Israel who had registered for or commenced

Capturing meaningful change: Initiation and maintenance of reductions in World Health Organization risk drinking levels among mandated college students.

OBJECTIVE: Reductions in World Health Organization (WHO) risk drinking levels have been used to capture nonabstinent reductions in drinking in the general population and in clinical trials. We examined mandated college students' reductions in WHO risk drinking levels 1 month postintervention, whether reductions were maintained at 5- to 6- and 12-month follow-ups and whether maintenance to 12 months predicted better alcohol-related outcomes. We also explored sex differences throughout. METHOD: The sample consisted of nonabstinent mandated students who received a brief motivational intervention

Alcoholism, morbidity and care-seeking. The inpatient and ambulatory service utilization and associated illness experience of alcoholics and matched controls in a health maintenance organization.

The issue of whether outpatient treatment of alcoholism is cost-advantageous, in the long run, for health maintenance organizations (HMOs) depends in part on whether alcoholics represent a relatively heavy burden on the health care delivery system. To clarify this issue, the paper asks whether alcoholics utilize HMO services at higher rates and in different ways than do other HMO members, and whether alcoholics experience more illnesses and injuries associated with their service utilization. The study subjects were alcoholics, identified during one year as new clients of an HMO's counseling de

Trends in Biochemical Parameters, Healthcare Resource and Medication Use in the 5 Years Preceding IBD Diagnosis: A Health Maintenance Organization Cohort Study.

BACKGROUND: Few data describing pre-diagnosis changes in patients with inflammatory bowel disease (IBD) exist. We aimed to determine if there is a pattern of change in use of health resources, medications and laboratory results in the years preceding diagnosis. METHODS: This retrospective study used electronic medical records of Maccabi Health Services (MHS). Patients with IBD ≥ 16 years of age and minimum of 5-years follow-up were identified by entry into the MHS IBD registry and included in the analysis. Demographic, clinical, medication and laboratory data were collected. Generalized estim

Prenatal screening using maternal serum alpha-fetoprotein, human chorionic gonadotropin, and unconjugated estriol: two-year experience in a health maintenance organization.

The objective of this study was to evaluate a 2-year experience in a health maintenance organization with mid-trimester maternal serum screening with alpha-fetoprotein (AFP), human chorionic gonadotropin (HCG), and unconjugated estriol (UE) as a screen for fetal Down's syndrome. Women at 15-20 weeks gestation were offered triple marker screening. A patient-specific second trimester risk of 1:295 for Down's syndrome was used as a threshold for referral. Women at risk for trisomy 18 were identified by a protocol with fixed low cutoffs. The AFP threshold for referral for neural tube defects (NTD)

Preventing neonatal group B streptococcal disease: cost-effectiveness in a health maintenance organization and the impact of delayed hospital discharge for newborns who received intrapartum antibiotics.

OBJECTIVES: To estimate the cost and health benefits of implementing a risk factor-based prevention strategy for early-onset neonatal group B streptococcal (GBS) disease, using baseline assumptions and costs from a health maintenance organization. With the risk factor-based strategy, intrapartum antibiotics (IPAs) would be provided to women with fever, prolonged rupture of membranes, or preterm labor. A second objective was to determine the impact of an increased length of stay for well term infants with mothers who received IPAs. METHODS: We used decision analysis to compare the costs and be
