Digital technology to address HIV and other sexually transmitted infection disparities: Intentions to disclose online personal health records to sex partners among students at a historically Black college.

Patient portals are creating new opportunities for youth to disclose high-fidelity sexually transmitted infection (STI) laboratory test result histories to sex partners. Among an online survey sample, we describe latent constructs and other variables associated with perceived behavioral intentions to disclose STI test history using patient portals. Participants were co-ed students aged 18 to 25 years (N = 354) attending a southern United States Historically Black College and University in 2015. Three reliable latent constructs were identified by conducting psychometric analyses on 27 survey it

Use of an Electronic Health Record Algorithm to Identify Persons With Human Immunodeficiency Virus Out Of Care in United States Emergency Departments.

OBJECTIVE: Emergency department (ED) human immunodeficiency virus (HIV) screening programs are designed primarily to identify persons with undiagnosed HIV and, therefore, systematically exclude persons with diagnosed HIV (PWHDx). PWHDx who are out of care (OOC) contribute disproportionately to HIV transmission and are a priority population within Ending the HIV Epidemic (EHE) initiatives. Because PWHDx OOC are not routinely identified through screening, complementary strategies are needed to support re-engagement in care. We evaluated an electronic health record algorithm (EHRa) designed to id

Machine learning characterization of a rare neurologic disease via electronic health records: a proof-of-principle study on stiff person syndrome.

BACKGROUND: Despite the frequent diagnostic delays of rare neurologic diseases (RND), it remains difficult to study RNDs and their comorbidities due to their rarity and hence the statistical underpowering. Affecting one to two in a million annually, stiff person syndrome (SPS) is an RND characterized by painful muscle spasms and rigidity. Leveraging underutilized electronic health records (EHR), this study showcased a machine-learning-based framework to identify clinical features that optimally characterize the diagnosis of SPS. METHODS: A machine-learning-based feature selection approach was

Development and external validation of a lightweight, explainable clinical decision support system for personalized perioperative risk stratification using electronic health records.

Late-preoperative risk stratification after final surgical scheduling may support perioperative risk communication, monitoring escalation, and resource coordination, yet many established calculators are difficult to automate within structured EHR workflows. We developed and externally validated LiteSurgFormer, a lightweight explainable MLP-attention risk-stratification model, in a retrospective multicenter cohort of 58,630 adult grade III-IV surgical patients treated at six Chinese sites from 2021 to 2024. Predictions were anchored after final operating-room schedule confirmation and primary s

Recovery in personality disorders: the development and preliminary testing of a novel natural language processing model to identify recovery in mental health electronic records.

INTRODUCTION: The concept of recovery is of great importance in mental health as it emphasizes improvements in quality of life and functioning alongside the traditional focus on symptomatic remission. Yet, investigating non-symptomatic recovery in the field of personality disorders has been particularly challenging due to complexities in capturing the occurrence of recovery. Electronic health records (EHRs) provide a robust platform from which episodes of recovery can be detected. However, much of the relevant information may be embedded in free-text clinical notes, requiring the development o

Integrating explainable artificial intelligence with multiomics systems biology and electronic health record data mining for personalized drug repurposing in Alzheimer's disease.

Alzheimer's disease (AD) is characterized by region- and patient-specific molecular heterogeneity, which hinders therapeutic design. In this study, we introduce PRISM-ML (PRecision-medicine using Interpretable Systems and Multiomics with Machine Learning), an open-source integrated analysis pipeline that combines interpretable machine learning with systems biology and electronic health records data mining to elucidate the molecular diversity of AD and predict promising drug repurposing opportunities. First, we integrated and harmonized transcriptomic (bulk RNA-seq) and genomic (genome-wide ass

Deep Reinforcement Learning for personalized diagnostic decision pathways using Electronic Health Records: A comparative study on anemia and Systemic Lupus Erythematosus.

BACKGROUND: Clinical diagnoses are typically made by following a series of steps recommended by guidelines that are authored by colleges of experts. Accordingly, guidelines play a crucial role in rationalizing clinical decisions. However, they suffer from limitations, as they are designed to cover the majority of the population and often fail to account for patients with uncommon conditions. Moreover, their updates are long and expensive, making them unsuitable for emerging diseases and new medical practices. METHODS: Inspired by guidelines, we formulate the task of diagnosis as a sequential

Incorporating personalized gene sequence variants, molecular genetics knowledge, and health knowledge into an EHR prototype based on the Continuity of Care Record standard.

OBJECTIVES: The current volume and complexity of genetic tests, and the molecular genetics knowledge and health knowledge related to interpretation of the results of those tests, are rapidly outstripping the ability of individual clinicians to recall, understand and convey to their patients information relevant to their care. The tailoring of molecular genetics knowledge and health knowledge in clinical settings is important both for the provision of personalized medicine and to reduce clinician information overload. In this paper we describe the incorporation, customization and demonstration

The impact of confidentiality and privacy concerns on behavioral intention to adopt electronic personal health records among chronic patients in southwest Ethiopia in 2023: an application of the UTAUT2 model.

BACKGROUND: The use of electronic personal health record systems (e-PHRS) is essential for chronic patients as they help to improve self-care management and communication between caregivers. However, when implementing e-PHRS, patients often express their concerns regarding privacy and confidentiality issues. Therefore, this study aimed to assess the impact of confidentiality and privacy concerns on the level of intention to use e-PHRS among chronic patients in southwest Ethiopia in 2023. METHOD: A cross-sectional study was conducted among 680 chronic patients from 24 July to 17 September 17 2

Healthcare utilization in older patients using personal emergency response systems: an analysis of electronic health records and medical alert data : Brief Description: A Longitudinal Retrospective Analyses of healthcare utilization rates in older patients using Personal Emergency Response Systems from 2011 to 2015.

BACKGROUND: Personal Emergency Response Systems (PERS) are traditionally used as fall alert systems for older adults, a population that contributes an overwhelming proportion of healthcare costs in the United States. Previous studies focused mainly on qualitative evaluations of PERS without a longitudinal quantitative evaluation of healthcare utilization in users. To address this gap and better understand the needs of older patients on PERS, we analyzed longitudinal healthcare utilization trends in patients using PERS through the home care management service of a large healthcare organization.

The Health Of Patients' Eyes (HOPE) Glaucoma study. The effectiveness of a 'glaucoma personal record' for newly diagnosed glaucoma patients: study protocol for a randomised controlled trial.

BACKGROUND: Glaucoma involves progressive optic nerve fibre loss, subsequently leading to irreversible and disabling visual field defects. In Europe, the prevalence of glaucoma is approximately 2.2 % of all people aged over 40 years; this equates to 12 million people. Glaucoma patients require regular lifelong follow-up, contributing to a large financial and resource burden for the National Health Service (NHS) in the UK. This study aims to determine whether providing newly diagnosed glaucoma patients with a personalised client-held eye health summary ('glaucoma personal record'), improves pat

Analysis of metrics for the usability evaluation of EHR management systems.

Electronic health records are replacing conventional paper-based health records. For a doctor it is a working instrument, which can significantly reduce the time spent on paper work. Patients can benefit from accessing the electronic health records even though they usually do not have a medical background. Therefore, when specifying a graphical user interface (GUI) it is necessary to take into account the requirements of the different users: e.g. the functionality for the doctors and the presentation of data in an understandable manner for the patients. The study aims to review and analyze met
