Temporal Trends in Incidence Rates of Lower Extremity Amputation and Associated Risk Factors Among Patients Using Veterans Health Administration Services From 2008 to 2018.

IMPORTANCE: Lower extremity amputation (LEA) is associated with significant morbidity and mortality. However, national temporal trends of LEA incidence rates among US veterans and associated factors have not been well characterized. OBJECTIVE: To describe the temporal trends of LEA, characterize associated risk factors, and decompose the associations of these risk factors with changes in temporal trends of LEA among US veterans using Department of Veteran Affairs (VA) services between 2008 and 2018. DESIGN, SETTING, AND PARTICIPANTS: This cohort study used VA data from 2008 to 2018 to estima

Health Services Use and Outcomes for Hospital Admissions With a Major Cardiovascular Event Recorded in Health Care Administrative Data in Patients Receiving Maintenance Hemodialysis: A Retrospective Cohort Study.

BACKGROUND: Administrative data are used in studies of hemodialysis care to report cardiovascular-related hospitalizations. Showing recorded events are associated with significant health care resource use and poor outcomes would confirm that administrative data algorithms identify clinically meaningful events. OBJECTIVE: The objective of this study was to describe the 30-day health service use and outcomes when a hospital admission with myocardial infarction, congestive heart failure, or ischemic stroke is recorded in administrative databases. DESIGN: This is a retrospective review of linked

Disability in childhood and the equity of health services: a cross-sectional comparison of mass drug administration strategies for soil-transmitted helminths in southern Malawi.

BACKGROUND: School-based approaches are an efficient mechanism for the delivery of basic health services, but may result in the exclusion of children with disabilities if they are less likely to participate in schooling. Community-based 'door to door' approaches may provide a more equitable strategy to ensure that children with disabilities are reached, but disability is rarely assessed rigorously in the evaluation of health interventions. OBJECTIVES: To describe the prevalence and factors associated with disability among children aged 5-17 years and to assess the relative effectiveness of ro

Leveraging Natural Language Processing to Identify Veterans Who Inject Drugs to Assess Preexposure Prophylaxis and Sexually Transmitted Infection Testing Services at the Veterans Health Administration.

BACKGROUND: People who inject drugs (PWID) face disproportionate risks for infectious diseases yet remain difficult to identify within health care systems. Natural language processing (NLP) offers potential solutions for identifying PWID to improve access to harm reduction services. METHODS: We evaluated an NLP dashboard designed to identify Veterans with evidence of injection drug use across 6 Veterans Health Administration facilities between August and October 2024. Four independent reviewers assessed electronic health records to confirm recent injection drug use and evaluated preventive ca

Use of healthcare services by patients with multiple sclerosis in France over 2010-2015: a nationwide population-based study using health administrative data.

BackgroundMost of the knowledge about people with multiple sclerosis (PwMS) in France comes from cohorts, which may suffer from recruitment bias or from the unique registry located in Lorraine, East France.ObjectiveTo describe use of care in the French population of PwMS, over 2010-2015.MethodsAll PwMS in the French national health data system (97% of the general population covered) were included. Demographics, and use of care were described (visits with general practitioners (GPs), neurologists, nurses, physiotherapists and hospitalisations). A focus on the neurological follow-up was also con

The Role of Military Sexual Trauma History and Frequency of Use of Veterans Health Administration Homeless and Justice Program Services Among Male and Female Homeless Veterans.

Military sexual trauma (MST) is a prevalent experience associated with deleterious chronic health and psychiatric conditions. The Department of Veterans Affairs (VA) offers MST-related care free of charge; however, MST history can impact perceptions of trust and safety, affecting willingness to access such care. MST is also more prevalent among Veterans with a history of homelessness and criminal legal involvement. These Veterans are often clinically complex and may benefit from higher rates of VA service use to meet their health and social care needs. No research to date has examined the pote

Incorporating HIV prevention into the medical care of persons living with HIV. Recommendations of CDC, the Health Resources and Services Administration, the National Institutes of Health, and the HIV Medicine Association of the Infectious Diseases Society of America.

Reducing transmission of human immunodeficiency virus (HIV) in the United States requires new strategies, including emphasis on prevention of transmission by HIV-infected persons. Through ongoing attention to prevention, risky sexual and needle-sharing behaviors among persons with HIV infection can be reduced and transmission of HIV infection prevented. Medical care providers can substantially affect HIV transmission by screening their HIV-infected patients for risk behaviors; communicating prevention messages; discussing sexual and drug-use behavior; positively reinforcing changes to safer be

The cumulative incidence and trends of rare diseases in South Korea: a nationwide study of the administrative data from the National Health Insurance Service database from 2011-2015.

BackgroundThe burden of rare diseases on society and patients' families has increased in Korea. However, because of the infrequency of rare diseases, there is a lack of resources and information to address these cases and inadequate funding for the management of these patients. We investigated the average annual cumulative incidence of rare diseases and the trends in annual cumulative incidence from 2011 to 2015 in Korea by using nationwide administrative data from the Korean National Health Insurance Service (NHIS) database for patients registered with the co-payment assistance policy for rar

Early antenatal care (ANC) booking and associated factors among pregnant women attending ANC services at public health facilities in Bahir Dar City Administration, Northwestern Ethiopia, 2023: a cross-sectional study.

Antenatal care (ANC) is vital for improving maternal and fetal health by enabling the early detection and management of pregnancy-related complications. The World Health Organization recommends a minimum of eight ANC contacts to optimize maternal and neonatal outcomes and reduce perinatal mortality. Increased ANC contacts enhance the opportunity to identify and address potential complications, and studies show that eight or more ANC visits can reduce perinatal deaths by up to 8 per 1,000 births compared to fewer visits. Therefore, the main objective of this study is to assess early ANC booking

Receipt of social services intervention in childhood, educational attainment and emergency hospital admissions: longitudinal analyses of national administrative health, social care, and education data in Wales, UK.

BACKGROUND: Research consistently finds poorer health and educational outcomes for children who have experienced out-of-home care relative to the general population. Few studies have explored differences between those in care and those in receipt of intervention from social services but not in care. Children receiving social services interventions often experience Adverse Childhood Experiences (ACEs), and deprivation, which are known to negatively impact outcomes. We aimed to estimate the association of different social services interventions with educational outcomes and hospital admissions,

Health service utilization, substance use treatment response, and death in patients with opioid use disorder and comorbid hepatitis C findings from prospective cohort study with administrative database linkage.

BACKGROUND: Among patients with opioid use disorder (OUD), high rates of overdose and death have been reported in subgroups with Hepatitis C Virus (HCV). Evidence on the comorbid effect of HCV on clinical and substance use trajectories has been limited by small sample sizes, short follow-up, and heavy reliance on administrative data which lacks granularity on important prognostic factors. Additionally, few studies include populations on substance use treatment. AIM: To establish the impact of HCV exposure (antibody positivity) on health care utilization patterns, substance use treatment respo

Hospital admissions among adolescents with local authority care experience or special educational needs in England: a population-based cohort study using linked administrative data from health, education and social care services.

INTRODUCTION: Children and young people in state care or with special educational needs (SEN) experience disproportionately higher rates of long-term physical and mental illness compared with their peers. However, few large-scale studies have explored the intersection of social care, education systems and healthcare services during adolescence. We aimed to quantify planned and unplanned hospital admissions during adolescence for pupils with SEN and/or experience of state care in England. METHODS: We analysed linked administrative records from hospital, education and social care systems for ad

Global Prison Health Care Governance and Health Equity: A Critical Lack of Evidence.

The large and growing population of people who experience incarceration makes prison health an essential component of public health and a critical setting for reducing health inequities. People who experience incarceration have a high burden of physical and mental health care needs and have poor health outcomes. Addressing these health disparities requires effective governance and accountability for prison health care services, including delivery of quality care in custody and effective integration with community health services.Despite the importance of prison health care governance, little i
