A proposal to prevent severe neonatal hyperbilirubinemia and kernicterus.

To address systems failures and promote a safer management of newborn jaundice, we propose an 'aviation safety standard' for newborn health-care services during the first week after birth. Systems failure in newborn jaundice management has been characterized by lapses in concern, loss of continuity, and delays in care by multiple providers at multiple sites. Components for a six-step national strategy to prevent severe neonatal hyperbilirubinemia and possibly kernicterus are being implemented as delineated in the 2004 AAP guidelines. The clinical guidelines for safer and evidence-based practic

Lessons learned from supporting local planning for extreme heat in North Carolina.

The impacts of extreme heat are broad and systemic, affecting not only human health and well-being, but also infrastructure, energy systems, and public services. Many of the best strategies to address these impacts are implemented at the local level, by local governments and their partners. This paper covers the approaches taken by the North Carolina State Resilience Office (SRO), the North Carolina State Climate Office (SCO) and their partners to support the development of heat action plans by local governments in their state. These agencies collaborated to develop planning support resources

Reforming Food, Drug, and Nutraceutical Regulations to Improve Public Health and Reduce Healthcare Costs.

Neglecting preventive healthcare policies has contributed to the global surge in chronic diseases, increased hospitalizations, declining quality of care, and escalating costs. Non-communicable diseases (NCDs)-notably cardiovascular conditions, diabetes, and cancer-consume over 80% of healthcare expenditure and account for more than 60% of global deaths, which are projected to exceed 75% by 2030. Poor diets, sedentary lifestyles, regulatory loopholes, and underfunded public health initiatives are driving this crisis. Compounding the issue are flawed policies, congressional lobbying, and conflic

National Quality Measures in Perinatal Medicine.

There are numerous measures of perinatal quality endorsed by national agencies such as the National Quality Forum (NQF). The sheer number of metrics may lead to confusion about what these measures truly assess, and how to interpret variation in these measures across hospitals, health care systems, and geographic regions. This review presents a conceptual model for the numerous validated measures, an overview of the types of measures endorsed for perinatal quality by NQF in 2016, and potential measures absent from endorsement by these national bodies.

The World Health Organization guideline for non-surgical management of chronic primary low back pain in adults: implications for equitable care and strengthening health systems globally.

The World Health Organization (WHO) released its first guideline on non-surgical management of chronic primary low back pain (LBP) in adults in 2023. The Guideline signals a timely and important global response to the current and projected burden attributable to LBP, particularly amongst populations made vulnerable, e.g. older people and people in low- and middle-income countries. The Guideline offers recommendations on which interventions for chronic primary LBP should be offered in most contexts, and those interventions that should not be offered as part of routine care. As a WHO technical p

Social Media as a Tool for Public Health: Reviewing the Use of Social Media for Disease Surveillance, Behavioral Change, and Misinformation Control.

Social media has transformed public health communication from a one-sided approach to a more interactive model, facilitating discussions on symptoms, treatments, and personal experiences among patients and caregivers, often without health professionals' involvement. Public health agencies recognised social media's potential for cost-effective, rapid outreach and real-time feedback, influencing health perceptions and behaviors. However, this platform also risks amplifying misinformation and conflicting narratives. Beyond communication, social media supports disease surveillance, influences heal

Revising the paradigm for jail diversion for people with mental and substance use disorders: Intercept 0.

A conceptual model for community-based strategic planning to address the criminalization of adults with mental and substance use disorders, the Sequential Intercept Model has provided jurisdictions with a framework that overcomes traditional boundaries between the agencies within the criminal justice and behavioral health systems. This article presents a new paradigm, Intercept 0, for expanding the utility of the Sequential Intercept Model at the front end of the criminal justice system. Intercept 0 encompasses the early intervention points for people with mental and substance use disorders be

Guillain-Barré syndrome after AstraZeneca COVID-19-vaccination: A causal or casual association?

We report a case of Guillain-Barré syndrome (GBS) following the first dose of Oxford/AstraZeneca COVID-19 vaccine with papilledema as atypical onset. As the COVID-19 vaccination campaign progresses worldwide, GBSs vaccine-related have been increasingly reported. After reviewing the available literature, considering the annual incidence of GBS, in this historical moment, the public health systems cannot afford an unjustified distrust in vaccines, caused by misinterpretation of epidemiological data. Nonetheless, it is important for clinicians to promptly recognize neurological complications pote

Patient inducement, provider priorities, and resource allocation in public mental health systems.

Public mental health systems are increasingly facing demands from the criminal justice system and social services agencies to provide services and support in cases in which mental illness contributes to crime, homelessness, or poverty. In this article we analyze how policies from outside public mental health systems affect resource allocation within these systems, using examples from criminal justice. These policies use two types of mechanisms: inducing patients to consume treatment (by offering rewards or imposing penalties) and inducing clinicians to provide treatment (by creating priorities

Systems of care for treatment of adolescent substance use disorders: background, principles and opportunities.

Adolescent substance abuse is a serious social problem facing the United States. Despite numerous recent advances in the clinical effectiveness of treatment approaches for this population, not enough attention has been paid to the adolescent treatment and service delivery infrastructures. The right services must be delivered through carefully organized and systematic community partnerships among agencies that serve the youth and families most in need. This article provides a working definition for the systems of care approach, reviews the movement's history within children's mental health serv

Sparganosis mimicking a soft-tissue tumor: A diagnostic challenge.

Human sparganosis is a rare but important food borne zoonosis and could be attributed to increased consumption of raw meat of fish, frogs, snakes etc. Sparganosis may involve varied organ systems but subcutaneous sparganosis remains the one of the commonly reported clinical condition. Rarity of this problem reinforces the necessity of sensitising the treating physicians of the differential possibility of this infection in patients with history of practice of consuming raw meat. Expansion of health communication and provision of safe food and water by the civic agencies can be a part of powerfu

Preparing the Health System to Respond to Ebola Virus Disease in New York City, 2014.

The world's largest outbreak of Ebola virus disease began in West Africa in 2014. Although few cases were identified in the United States, the possibility of imported cases led US public health systems and health care facilities to focus on preparing the health care system to quickly and safely identify and respond to emerging infectious diseases. In New York City, early, coordinated planning among city and state agencies and the health care delivery system led to a successful response to a single case diagnosed in a returned health care worker. In this article we describe public health and he

Improving Identification of Gig Workers in National Health and Behavior Surveys.

This paper describes the work-related information collected in several important U.S. national health and behavior surveys, to highlight data gaps that prevent identifying responses by vulnerable workers in the gig economy, with emphasis on the growing digital platform sector of the work force. The national information systems used to understand health status and health behaviors, including drug use, rely on outdated census categories for self-employed workers. This paper describes the importance of understanding the needs of this growing part of the labor sector and describes how some of the

Failure of many United States Department of Health Web sites to provide accurate information about the female condom.

Uptake of the female condom (FC) in the United States has historically been low; inadequate promotion may be one barrier faced by potential users. We performed a content analysis of state and municipal health department Web sites to describe how the FC is being promoted for pregnancy and disease prevention. We found that only a slim majority (60.8%) of health department Web sites mention the FC at all and those that do include numerous inaccuracies in their messaging. These inaccuracies may discourage uptake of the FC or have a detrimental impact on the experience when using the product for th

Challenges and potential opportunities for improving One Health surveillance in low-resource settings: Insights from rabies surveillance in Malawi.

The growing threat of zoonotic diseases has driven an increased interest in “One Health”, a framework that demands a more holistic approach to understanding, preventing and controlling the emergence and spread of health risks at the human-animal-environment interface. This approach requires systems that capture data across human, animal, and environmental health sectors. However, in resource-limited settings, critical actors, namely public health departments, veterinary services, and environmental monitoring agencies, are often underfunded and lack robust surveillance systems. Additionally, th
