[The heart].

The growing incidence of heart failure and the limitations of cardiac transplantation and totally implantable artificial hearts justify the search for alternative therapies. Self-reconstruction of the heart might be one of them. It is based on the use of an animal-derived decellularized scaffold reseeded with cells able to reconstitute a contractile, vascular and valvular pattern affording normal cardiac function. Whereas decellularization techniques are currently well controlled, the choice of cell type to generate the different constituents of cardiac tissue (cardiomyocytes, endothelial cell

Heart transplant evaluation in adults with congenital heart disease presenting with advanced heart failure.

BACKGROUND: Adults with congenital heart disease (CHD) with stage D heart failure (HF) have 1-year mortality risk of almost 40% without heart transplantation, and as a result, heart transplant evaluation is the logical next step in the management of these patients. The purpose of this study was to describe the clinical characteristics and outcome of adults with CHD referred for heart transplant evaluation. METHOD: Adults with CHD referred for heart transplant evaluation were divided into 3 treatment pathways after multidisciplinary transplant evaluation. (i) Transplant-listed group (patients

Heart Talk: Emotional Inner Speech Increases Heart Rate.

In two preregistered experiments (exp.1: n = 44; exp.2: n = 46), we investigated whether emotional inner speech influences heart rate. Participants were asked to engage in sessions of either: self-encouraging positive inner speech, self-degrading negative inner speech, or inner counting while their heart rate was monitored. Participants were lying on a bed and asked to remain still during the inner speech trials. Experiment 1 consisted of two negative, two positive, and four counting trials. Experiment 2 had four trials of each type. Trials lasted 180 s in Experiment 1 and had a mixed duration

Modeling heart rhythm using human engineered heart tissues.

Heart rate is both an indicator and modulator of cardiovascular health. Prolonged elevation in heart rate or irregular heart rhythm can trigger the onset of cardiac dysfunction, a condition termed 'tachycardia-induced cardiomyopathy'. While large animals have historically served as the primary model for studying this condition owing to their similar resting heart rates to humans, their use is limited by cost and throughput constraints. We recently developed the first engineered model of tachycardia-induced cardiomyopathy to overcome this technical bottleneck. Our model uses matured human engin

Evaluating predicted heart mass in adolescent heart transplantation.

BACKGROUND: Predicted Heart Mass (PHM) has emerged as an attractive size matching metric in adult cardiac transplantation. However, since PHM was derived from a healthy adult cohort, its generalizability to the pediatric population is unclear. We hypothesize that PHM can be extended to older adolescents, and potentially broaden the donor pool available to this group. METHODS: The United Network for Organ Sharing database was retrospectively analyzed for patients aged 13 to 18 undergoing heart transplantation. Recipients were divided into quintiles (Q1-Q5) based on donor-to-recipient predicted

Heart Failure: Heart Failure With Preserved Ejection Fraction.

The incidence and prevalence of heart failure (HF) in the United States are high, with an estimated 6.2 million cases, and these numbers are expected to increase. Age is a nonmodifiable risk factor for HF development. Hypertension, diabetes, and ischemic heart disease are modifiable risk factors that can be addressed to reduce the morbidity and mortality associated with HF. Improvements in understanding of the pathophysiology of HF have led to changes in terminology. Terms such as congestive heart failure, systolic heart failure, and diastolic heart failure should no longer be used. Currently,

Rehabilitation in Pediatric Heart Failure and Heart Transplant.

Survival of pediatric patients with heart failure has improved due to medical and surgical advances over the past decades. The complexity of pediatric heart transplant patients has increased as medical and surgical management for patients with congenital heart disease continues to improve. Quality of life in patients with heart failure and transplant might be affected by the impact on functional status that heart failure, heart failure complications or treatment might have. Functional areas affected might be motor, exercise capacity, feeding, speech and/or cognition. The goal of rehabilitation

Imaging of Valvular Heart Disease in Heart Failure.

Valvular heart disease (VHD) and heart failure (HF) are major health issues that are steadily increasing in prevalence in Western populations. VHD and HF frequently co-exist, which can complicate the accurate diagnosis of the severity of valve stenosis or regurgitation and affect decisions about therapeutic options. Transthoracic echocardiography is the first-line imaging modality to determine left ventricular (LV) systolic function, to grade valvular stenosis or regurgitation and to characterise the mechanism underlying valvular dysfunction. 3D transoesophageal echocardiography, cardiovascula

Heart failure in grown-up congenital heart disease.

The increasing survival to adulthood of patients with congenital heart disease (CHD) has changed the epidemiology of adult CHD (ACHD) patients and has led to an increment in hospitalization rates due to heart failure (HF). ACHD patients hospitalized for HF have a five-fold higher risk of death than those compensated. HF occurs predominantly in patients with tetralogy of Fallot, single ventricles, and after the Mustard operation for transposition of the great arteries. Diagnostic strategies applied in acquired HF patients are usually used to evaluate ACHD patients, but sometimes this can postpo

4-Valve Heart Disease and Right Heart Failure.

Carcinoid heart disease is a rare form of heart disease due to secretion of vasoactive compounds, including serotonin, from gastrointestinal tumors. This E-challenge examines the case of a patient with advanced carcinoid heart disease who presented to the operating room (OR) for a tricuspid valve replacement. Once the patient was in the OR, intraoperative transesophageal echocardiography was used to discover a patent foramen ovale and involvement of all 4 valves with regurgitant lesions. The patient underwent tricuspid valve replacement, pulmonic valve replacement, right ventricular outflow tr

Heart transplantation in adults with congenital heart disease.

With the advances in congenital cardiac surgery and postoperative care, an increasing number of children with complex congenital heart disease now reach adulthood. There are already more adults than children living with a congenital heart defect, including patients with complex congenital heart defects. Among these adults with congenital heart disease, a significant number will develop ventricular dysfunction over time. Heart failure accounts for 26-42% of deaths in adults with congenital heart defects. Heart transplantation, or heart-lung transplantation in Eisenmenger syndrome, then becomes

CNS-disease affecting the heart: brain-heart disorders.

There are a number of hereditary and non-hereditary central nervous system (CNS) disorders, which directly or indirectly affect the heart (brain-heart disorders). The most well-known of these CNS-disorders are epilepsy, stroke, subarachanoid bleeding, bacterial meningitis, and head injury. In addition, a number of hereditary and non-hereditary neurodegenerative disorders may impair cardiac functions. Affection of the heart may manifest as arrhythmias, cardiomyopathy, or autonomic dysfunction. Rarer cardiac complications of CNS disorders include heart failure, systolic or diastolic dysfunction,

New issues in heart transplantation for heart failure.

Heart transplantation is the preferred therapy for patients with end-stage heart failure with refractory symptoms despite optimal medical and device therapy. The major impediment to survival is rejection and infection in the short term and cardiac allograft vasculopathy and malignancy in the long term. Current therapies are focused on the prevention and treatment of rejection and limiting the long-term problems of cardiac allograft vasculopathy and malignancy. Advances in monitoring assays now allow better assessment of rejection and the level of immune response. This will allow clinicians, in

Indications for heart transplantation in congenital heart disease.

In this review we have looked at indications for cardiac transplantation in congenital heart disease. An outline of the general principles of the use of transplant as a management strategy both as a first line treatment and following other surgical interventions is discussed. We explore the importance of the timing of patient referral and the evaluations undertaken, and how the results of these may vary between patients with congenital heart disease and patients with other causes of end-stage heart failure. The potential complications associated with patients with congenital heart disease need

Neonatal congenital heart disease and "complex" heart failure.

Hypoplastic left heart syndrome is a complex conglomerate of congenital cardiac abnormalities encountered in early life. Heart failure compensation and survival are intricately dependent on maintenance of ductal patency and avoidance of hyperoxic ventilation. While medical therapy is woefully inadequate, the staged Norwood procedure or cardiac transplantation remain the better options for treatment. Critically dependent on surgical outcomes, 5-year survival appears better with cardiac transplantation.

[Treatment of advanced heart failure by heart transplantation].

BACKGROUND: Heart transplantation currently provides the most effective treatment for advanced heart failure. However, medical therapy for this condition has also improved, heart donors are scarce and the cost of the procedure is high. Therefore the indications and management of these patients need reevaluation. AIM: To analyze the results of 24 patients submitted to heart transplantation for end-stage heart failure needing repeated hospitalizations and i.v. inotropes for compensation. PATIENTS AND METHODS: The group was comprised by 21 men and 3 women with a mean age of 36.8 years, mean lef
