Congestive Heart Failure in an Adolescent With a Ruptured Sinus of Valsalva Aneurysm.

Sinus of Valsalva aneurysm (SVA) is an abnormal dilatation of the aortic root located between the aortic valve annulus and the sinotubular junction and is rare in the pediatric population. This case report describes a unique case of a 16-year-old adolescent patient admitted with progressive heart failure symptoms and diagnosed with a ruptured noncoronary SVA. He underwent surgical repair of the SVA with autologous pericardial patches and had an uncomplicated postoperative course. A genetic workup revealed an underlying 22q11.2 deletion that is infrequently associated with SVA.

Extracranial carotid artery aneurysms: Texas Heart Institute experience.

BACKGROUND AND PURPOSE: Aneurysms of the extracranial carotid artery (ECA) are rare. Large single-institution series are seldom reported and usually are not aneurysm type-specific. Thus, information about immediate and long-term results of surgical therapy is sparse. This review was conducted to elucidate etiology, presentation, and treatment for ECA aneurysms. METHODS: We retrospectively reviewed the case records of the Texas Heart Institute/St Luke's Episcopal Hospital, Houston, and found 67 cases of ECA aneurysms treated surgically (the largest series to date) between 1960 and 1995: 38 pse

Surgical treatment of an aneurysm of the vein of Galen in a newborn with heart failure.

Congestive heart failure is the usual complaint and cause of death in neonates with aneurysms of the great vein of Galen. Direct surgical obliteration of the feeding arteries cures the heart failure, but few such newborns survive the operation. The authors report a case of primary aneurysm of the great vein of Galen treated successfully by clipping the afferent arteries and reduction of the bulk of the aneurysm by bipolar coagulation. Five months after the operation the child's development is normal.

Heparinless left heart bypass for resection of thoracic aortic aneurysms.

Our clinical experience employing heparinless left heart bypass for the resection of twenty-three thoracic aneurysms is presented. In our current technic of heparinless left heart bypass, plastic tubing coated with nonthrombogenic polyurethane-polyvinyl-graphite material and a conventional roller pump are employed. The reduced size of the aneurysm below the aortic clamp during bypass facilitates careful dissection of the aneurysm and the aorta. Twenty-two of twenty-three patients underwent successful thoracic aneurysmectomy with this technic. None had subsequent paraplegia and the postoperativ

Beating heart superior caval vein replacement for giant venous aneurysm: a case report.

BACKGROUND: Superior vena cava (SVC) aneurysms are rare vascular anomalies, with <80 cases reported worldwide. Large aneurysms carry a risk of rupture and thrombosis, eventually necessitating surgical intervention. CASE SUMMARY: We present the case of a 68-year-old male with a progressively enlarging SVC aneurysm and a history of paroxysmal atrial fibrillation. The patient underwent successful caval resection and replacement with a 20 mm expanded polytetrafluoroethylene (ePTFE) graft, accompanied by left atrial appendage exclusion and pulmonary vein isolation. The recovery from surgery was un

Giant pulmonary artery aneurysm in a child: Rare complication of congenital heart disease.

KEY CLINICAL MESSAGE: This case report aims to increase awareness that pulmonary artery aneurysms may occur as a complication of neglected patent ductus arteriosus and should be sought in children with ill-treated congenital heart diseases. ABSTRACT: Pulmonary artery aneurysm is a rare anomaly with an autopsy prevalence of 1:14,000. These aneurysms can arise secondary to various etiologies, with congenital causes identified in 25% of cases and congenital heart diseases (CHD) responsible for more than half of these cases. A 12-year-old boy with CHD in the form of patent ductus arteriosus (PDA)

Klf15 deficiency is a molecular link between heart failure and aortic aneurysm formation.

Current therapies for diseases of heart muscle (cardiomyopathy) and aorta (aortopathy) include inhibitors of the renin-angiotensin system, beta-adrenergic antagonists, and the statin class of cholesterol-lowering agents. These therapies have limited efficacy, as adverse cardiovascular events continue to occur with some frequency in patients taking these drugs. Although cardiomyopathy and aortopathy can coexist in a number of conditions (for example, Marfan's syndrome, acromegaly, pregnancy, and aging), pathogenetic molecular links between the two diseases remain poorly understood. We reasoned

Right ventricular aneurysm following modified Norwood-Sano operation for hypoplastic left heart syndrome.

Hypoplastic left heart syndrome is a rare congenital heart defect characterized by underdevelopment of left-sided heart structures, including the aortic arch. The contemporary surgical management of this anomaly includes the Norwood procedure and provision of pulmonary blood flow by either a modified Blalock-Taussig shunt (MBTS) or a right ventricle-to-pulmonary artery (RV-PA) conduit, commonly referred to as the Sano shunt. We report on an unusual complication of the Sano shunt, that of a giant right ventricular pseudoaneurysm occurring at the shunt insertion site.

Giant coronary artery aneurysm causing superior vena cava syndrome and congestive heart failure.

Coronary artery aneurysms are defined as localized coronary artery dilations >1.5 times the diameter of the adjacent segments. In adults, they are commonly of atherosclerotic origin and are usually asymptomatic. This is the first case report of a coronary artery aneurysm causing superior vena cava syndrome and heart failure. Chest x-ray showed cardiomegaly, and transthoracic echocardiography showed a mass that was confirmed by transesophageal echocardiography, computed tomography, and coronary angiography. In conclusion, giant coronary artery aneurysms can cause superior vena cava syndrome.

Simultaneous surgery for ischemic heart disease, type A aortic dissection and abdominal aneurysm.

The patient was a 72-year-old male with three-vessel coronary artery disease, chronic dissecting aneurysm of the ascending aorta with moderate aortic regurgitation, and abdominal aortic aneurysm 7 cm in diameter. Because staged procedure seemed to exacerbate the risk due to the remaining lesion, simultaneous procedures (double coronary artery bypass, aortic root remodeling, tube replacement of the ascending aorta and Y-graft replacement of the abdominal aneurysm) were performed. The patient recovered completely without any serious complication.

Impact of left heart bypass on the results of thoracoabdominal aortic aneurysm repair.

BACKGROUND: This study evaluated the role of left heart bypass on the results of thoracoabdominal aortic aneurysm (TAAA) operations. METHODS: Two hundred fifty-eight patients had surgical repair of a thoracoabdominal aortic aneurysm between 1981 and 1998 using the inlay technique. Simple cross-clamping was used in 47.7% and left heart bypass (atriodistal) in 52.3%. Further surgical technique was identical: liberal intercostal or lumbar artery reimplantation, cerebrospinal fluid drainage (since 1989), administration of a renal cooling solution, permissive mild hypothermia, and no pharmacologic

Risk of aortic aneurysm surgery as assessed by preoperative gated heart pool scan.

Gated heart pool scan measuring left ventricular ejection fraction (LVEF) was performed preoperatively in 72 patients presenting for elective repair of abdominal aortic aneurysm. Patients with a positive cardiac history were more likely to have a LVEF of less than or equal to 45 per cent (P less than 0.001). The operative mortality rate was 4 per cent. Each of three patients who died had a LVEF less than or equal to 35 per cent and developed cardiac failure which led to renal failure. Five other patients developed cardiac failure manifested by acute pulmonary oedema during the early postoperat

Combined Heart-Lung Transplant for Pulmonary Artery Aneurysm Secondary to Pulmonary Arterial Hypertension.

BACKGROUND: Pulmonary artery aneurysms (PAAs) are rare occurring in approximately 1 in 14,000 cases. While PAA is frequently associated with pulmonary arterial hypertension (PAH), the optimal management strategy remains undefined due to the rarity of the disease. CASE SUMMARY: We report a case of a 58-year-old female with longstanding idiopathic PAH who underwent combined heart-lung transplantation to treat a rapidly expanding 8.95-cm PAA with associated cor pulmonale and severe pulmonic valve regurgitation. Graft function remained excellent; however, an ischemic stroke in the postoperative p

Repair of ascending aortic aneurysm after heart-lung transplantation - A paediatric case report.

Ascending aortic aneurysm is a rare complication occurring after combined heart-lung transplants. Most post-transplant aortic pathologies are infective in origin and typically present as mycotic pseudoaneurysms or with features of aneurysmal rupture leading to hemodynamic instability. We report the case of a 13-year-old girl who underwent heart-lung transplantation 8 months back, presenting with a large unruptured ascending aortic aneurysm with stable hemodynamics. Surgical repair was performed using a collagen-coated polyester tube graft under cardiopulmonary bypass with peripheral femoral ca

On-pump beating-heart technique for managing a ruptured left coronary artery aneurysm.

We present the case of an 86-year-old woman with a ruptured left coronary artery aneurysm associated with coronary-pulmonary artery fistula. She was transferred to our hospital with complaints of chest discomfort. Echocardiography revealed cardiac tamponade, and subsequently, a bloody pericardial effusion was drained. Coronary angiography and computed tomography revealed a ruptured left coronary artery aneurysm originating from a branch of the proximal left anterior descending artery and a coronary-pulmonary artery fistula. In the emergency operation, under cardiopulmonary bypass, the inlet an

Treatment of thoracoabdominal aortic aneurysm by prosthetic vessel replacement under left heart bypass.

OBJECTIVES: Thoracoabdominal aortic aneurysm (TAAA) prosthetic vessel replacement is one of the most complex operations in the field of cardiovascular surgery. The key to success of this operation is to prevent and avoid ischemia of important organs while repairing TAAA. This study aims to summarize and analyze the effect of prosthetic vessel replacement under left heart bypass in the treatment of TAAA. METHODS: Data of 15 patients with TAAA who underwent prosthetic vessel replacement under left heart bypass in Xiangya Hospital of Central South University were retrospectively analyzed. Accord

Improvement of Severe Heart Failure after Endovascular Stent Grafting for Thoracic Aortic Aneurysm.

Afterload is considered to be an important factor regulating heart failure. Aortic structure or pathology may affect afterload to various extents. However, the contribution of aortic diseases, such as aortic aneurysm or aortic dissection, to heart failure status has not been completely elucidated. Here we describe a 78-year-old patient with severe heart failure who made a dramatic recovery from cardiac decompensation following endovascular thoracic aortic aneurysm surgery. He previously underwent graft replacement for impending rupture of the descending aorta and replacement of both the mitral

Expanding arch aneurysm causing a "kink" in a Bentall graft and heart failure.

Marfan syndrome is associated with a high incidence of aortic root aneurysm and life-threatening aortic dissection. With the successful use of surgical aortic root replacement, dissection-related mortality has been significantly reduced. We present the case of a patient with Marfan syndrome who presented with heart failure secondary to an unusual graft-related complication 14 years after a Bentall procedure. Investigations revealed a supra-aortic stenosis resulting from a kink in the Bentall graft caused by pressure from an expanding aortic arch aneurysm. The patient underwent surgery with imp

Ascending aortic aneurysm after pediatric heart transplantation: case report of an unusual complication.

A 28-month-old boy, weighing 11 kg, with severe dilated cardiomyopathy, was transplanted on December 1995. Hypertension and supraventricular tachycardia were detected in the immediate post-operative period, with favorable outcome. After 5 months of clinically asymptomatic follow-up, a dilation in the ascending aorta was observed on routine echocardiogram. Nuclear magnetic resonance imaging (NMRI) confirmed an ascending aortic aneurysm, with a diameter of 38 mm. An operation was performed, a bovine pericardium patch was sutured with reconstruction of the aortic wall, excluding the aneurysm. Goo
