QT interval dynamics and heart rate variability preceding a case of cardiac arrest.

A 71 year old man with hypertensive heart disease and chronic renal failure was wearing a Holter monitor when he had a cardiac arrest. He had ventricular fibrillation (VF) and died despite prompt resuscitation. In the 15 minutes preceding the VF there was a sudden increase in heart rate, followed by a brief period of atrial fibrillation leading to ventricular tachycardia, which in turn rapidly degenerated into VF. The QT interval and heart rate variability were studied half hourly over the seven hours preceding the cardiac arrest, using a computerised Holter system. A further detailed analysis

Bradycardic Arrest in a Child with Complex Congenital Heart Disease Due to Sugammadex Administration.

The neuromuscular blocking drugs rocuronium and vecuronium are often used during general anesthesia. These drugs temporarily paralyze the patient and thus both facilitate placement of an endotracheal tube and prevent any patient movement during surgery. Reversal of neuromuscular blockade is necessary at the end of surgery to avoid postoperative weakness and adverse respiratory events in the recovery room. Neostigmine, the traditional reversal agent, may not completely restore muscle strength. Sugammadex is a reversal agent that is more effective and quicker acting than neostigmine. In adults,

The clinical characteristics of sudden cardiac arrest in asymptomatic patients with congenital heart disease.

Sudden cardiac arrest (SCA) is a major cause of death in patients with congenital heart disease (CHD). Systemic ventricular dysfunction is a reported risk factor for SCA. We retrospectively analyzed the medical records of 46 patients (age >6 years) who experienced SCA. The following underlying cardiac defects were observed: biventricular repair with affected subpulmonary right ventricle (n = 18, 39 %), biventricular repair with systemic right ventricle and Eisenmenger syndrome (n = 6 each, 13 %), Fontan circulation and unrepaired CHD (n = 5 each, 11 %), and others (n = 6, 13 %). Cardiac arrest in an (apparently) healthy heart: A case report of an undiagnosed arrhythmogenic cardiomyopathy.

UNLABELLED: Arrhythmogenic cardiomyopathy is a rare and insidious disease that can be misdiagnosed with the common first-line diagnostic techniques. It can also primarily debut with sustained ventricular arrhythmias that can lead to cardiac arrest. We report the case of a 49-year-old triathlon athlete who during strenuous physical exercise experimented cardiac arrest. After successful resuscitation he was admitted to our unit: electrocardiogram, emergency echocardiogram, and coronary angiography did not show anything abnormal. Holter monitoring showed premature ventricular complexes with two d

Open transcatheter double valve-in-valve replacement for degenerated bioprostheses on the arrested heart.

BACKGROUND: Failing bioprosthesis is an emerging issue due to (i) a shift towards liberal bioprosthesis implantation instead of mechanical prosthesis and (ii) an ageing population. Management of high-risk patients with bioprosthesis degeneration remains challenging. CASE SUMMARY: An 82-year-old patient with history of aortic and mitral valve replacement six years before presents with severe dyspnoea. Echocardiograpic assessment reveals (i) structural valve degeneration of the mitral prosthesis (severe stenosis and regurgitation) with concomitant major annular calcifications and (ii) structura

Recovery of the conduction system from ischemic arrest and cardioplegia in perfused rat heart.

Normothermic ischemic arrest of the perfused rat heart for 1 h was followed by severe reperfusion arrhythmias, depressed excitability, atrioventricular (AV) node and sinoatrial (SA) node conduction blocks and by marked impairment of contractility. Excitability and conduction recovered within 10 to 15 mins whereas contractility remained greatly depressed. This finding is in agreement with a theoretically based assumption of lesser sensitivity of conduction system to ischemia compared to the working myocardium. When the hearts were exposed to ischemia at 20 degrees C, the sinus rhythm recovered

Dual color optogenetic tool enables heart arrest, bradycardic, and tachycardic pacing in Drosophila melanogaster.

In order to facilitate cardiovascular research to develop non-invasive optical heart pacing methods, we have generated a double-transgenic Drosophila melanogaster (fruit fly) model suitable for optogenetic pacing. We created a fly stock with both excitatory H134R-ChR2 and inhibitory eNpHR2.0 opsin transgenes. Opsins were expressed in the fly heart using the Hand-GAL4 driver. Here we describe Hand > H134R-ChR2; eNpHR2.0 model characterization including bi-directional heart control (activation and inhibition) upon illumination of light with distinct wavelengths. Optical control and real-time vis

The Use of Hypothermic Circulatory Arrest During Heart Transplantation Does Not Worsen Posttransplant Survival.

BACKGROUND: Hypothermic circulatory arrest (HCA) has been used as an adjunct to cardiopulmonary bypass for decades, both electively and emergently, to facilitate a bloodless operative field while maintaining cerebral protection. The aim of this study is to determine the impact of HCA during heart transplantation on posttransplant outcomes. METHODS: All adult patients undergoing orthotopic heart transplantation at our institution between 2000 and 2012 were retrospectively reviewed. Patients were stratified based on need for HCA during surgery; patients who required HCA (HCA group, n = 25), and

[Heart arrest in excision of large craniopharyngiomas: two cases with successful resuscitation--pathogenic considerations].

A reversible cardiac arrest with successful resuscitation/cardiac massage occurred in two female patients in the context of the relatively radical extirpation of large mainly solid craniopharyngiomas with supraretrosellar and retrochiasmatic extension. Clinical (including ECG) and pathological anatomical investigations (especially after subarachnoid hemorrhages) and the results of animal experiments indicate that the cardiac arrest was caused by an acute functional disturbance in the dorsocaudal hypothalamus. In particular, the coincidence of hemorrhages/ischemia in the hypothalamus and subend

Increased intracranial pressure and cardiac arrest after heart transplantation. What about the Cushing response in a denervated heart? Case report.

A 13-year-old boy suffered cerebrovascular complications after heart transplantation (ischemic mass effect in the posterior cranial fossa). He had to be resuscitated from cardiac arrest with coma. After a modified cerebellar hemispherectomy the course was favorable.--The most conclusive explanation for the acute event is that a Cushing response was preserved even in the presumably denervated heart.

An 82-Year-Old Man With Sleep-Onset Insomnia, Breathing Arrest, and Heart Failure.

An 82-year-old man presented with 6 months of difficulties of falling asleep. He described a feeling of fading breath culminating in breathing arrest when he becomes drowsy. These recurrent events prevented him from falling asleep. Symptoms would only appear when he went to sleep but not during wakefulness. Medical history comprised several episodes of acute decompensated heart failure due to supraventricular tachyarrhythmia with need for hospitalization during the last 2 years. He additionally had two-vessel coronary artery disease with myocardial infarction, pulmonary hypertension, chronic a

Intra-arrest formation of right-heart thrombi -- a case illustrated by real-time ultrasonography.

The presence of free-floating right-heart thrombus has been reported in a cardiac arrest patient in the periarrest period. Free-floating right atrial thrombus is a rare phenomenon seen in patients developing severe pulmonary embolism, and is associated with increased mortality. However, there have been no previously reported cases of right-heart thrombus formation during a resuscitated cardiac arrest. We present the pre-hospital case of a woman in the clinical setting of cardiogenic shock due to acute myocardial infarction who developed asystolic cardiac arrest on the scene. Recent implementat

Allopurinol neurocardiac protection trial in infants undergoing heart surgery using deep hypothermic circulatory arrest.

OBJECTIVE: This pharmacologic protection trial was conducted to test the hypothesis that allopurinol, a scavenger and inhibitor of oxygen free radical production, could reduce death, seizures, coma, and cardiac events in infants who underwent heart surgery using deep hypothermic circulatory arrest (DHCA). DESIGN: This was a single center, randomized, placebo-controlled, blinded trial of allopurinol in infant heart surgery using DHCA. Enrolled infants were stratified as having hypoplastic left heart syndrome (HLHS) and all other forms of congenital heart disease (non-HLHS). Drug was administer

Ex vivo resuscitation and evaluation of hearts after 22 minutes of normothermic cardiac arrest.

OBJECTIVE: The aim was to resuscitate and evaluate hearts ex vivo after 22 min of cardiac arrest with the goal of increasing the number of usable hearts from controlled donation after circulatory death (cDCD). DESIGN: Eight pigs (39-61 kg) underwent 22 min of ventricular fibrillation, after which the heart was first perfused in vivo for three minutes with an oxygenated, erythrocyte-containing cardioplegic preservation solution. The heart was then explanted and perfused ex vivo with the same solution for three hours at 18 °C in a transportable heart preservation system. Functional evaluation w

Impact of occurrence of cardiac arrest in the donor on long-term outcomes of pediatric heart transplantation.

OBJECTIVE: The impact of cardiac arrest in the donor on long-term outcomes of pediatric heart transplantation has not been studied. METHODS: The UNOS database was queried for primary pediatric heart transplantation (1999-2020). The cohort was divided into recipients who received a cardiac allograft from a donor who had a cardiac arrest (CA) versus a donor who did not (NCA). Univariable and multivariable analysis was done to compare recipient outcomes, followed by survival analysis using the Kaplan-Meier method. RESULTS: A total of 7300 patients underwent heart transplantation, of which 579 (
