Transcatheter aortic valve-in-valve-in-valve implantation for a failed xenograft.

A 78-year-old gentleman with a diagnosis of symptomatic severe xenograft aortic stenosis with multiple comorbidities was referred for transcatheter aortic valve implantation, that is, a "valve-in-valve" procedure. Transcatheter aortic valve implantation was performed by transapical approach using a balloon-expandable bioprosthesis. During valve deployment, the prosthesis moved toward the left ventricle and a second balloon-expandable valve was implanted within the first one-a "valve-in-valve-in-valve" to avoid further ventricular embolization of the first bioprosthesis. Echocardiography at hos

Transcatheter heart valves for the treatment of aortic stenosis: state-of-the-art.

UNLABELLED: Degenerative aortic stenosis is the most frequent heart valve disease. As an alternative to surgical aortic valve replacement, several companies are working on the development of new prosthesis designed to be deployed by transcatheter approaches. Both transfemoral and transapical techniques are feasible, and initial trials in high-risk patients show good procedural outcomes and mid-term (up to 2 years) functionality. Two first-generation prosthesis (Edwards-SAPIEN and CoreValve Revalving System) are commercially available in Europe, and a number of other second-generation valves (w

Patient Prosthesis Mismatch After SAVR and TAVR.

Patient-prosthesis mismatch (PPM) remains one out of many factors to be considered during decision-making for the treatment of aortic valve pathologies. The idea of adequate sizing of a prosthetic heart valve was established by Rahimtoola already in 1978. In this article, the author described the phenomenon that the orifice area of a prosthetic heart valve may be too small for the individual patient. PPM is assessed by measurement or projection of the prosthetic effective orifice area indexed to body surface area (iEOA), while it is recommended to use different cut point values for non-obese a

Normally Functioning Cooley-Cutter Mechanical Prosthesis Associated With Ascending Aorta Aneurysm 39 Years After Implantation.

The Cooley-Cutter prosthesis is a caged-disc mechanical heart valve introduced by Dr Cooley in the 1960s. Although the valve has been implanted worldwide in more than 3,000 patients, there is a lack of detailed data on its long-term performance, only anecdotal reports. We report the case of a patient who had undergone aortic valve replacement with a Cooley-Cutter prosthesis 39 years earlier for rheumatic disease on a tricuspid aortic valve and in whom a proximal aortic aneurysm had subsequently developed.

Cardiogenic Shock Due to Leaflet Migration of On-X Aortic Mechanical Prosthesis.

Cardiogenic shock is a life-threatening condition requiring fast and efficient diagnostic and therapeutic measures. In this case, the history of several cardiac surgeries hindered finding the correct diagnosis initially. After an ultima ratio cardiac redo operation the underlying cause was found intraoperatively: a defective mechanical valve prosthesis with migration of one of the two leaflets. Strikingly, this happened with a contemporary On-X prosthesis only 4 years after implantation. Timely echocardiography is of utmost importance in patients with prosthetic heart valves presenting in hear

HeartMate 3 implantation in a patient with mechanical mitral valve prosthesis: intentional leaflet fracture and sewing ring preservation.

Management of a pre-existing mechanical mitral valve prosthesis during durable left ventricular assist device (LVAD) implantation remains controversial. Retaining the prosthesis may predispose to leaflet immobility, thrombosis, and inflow obstruction, whereas full prosthetic replacement adds substantial operative risk, particularly in redo cardiac surgery. We describe a novel surgical strategy combining removal of the mobile prosthetic components with preservation of the sewing ring during HeartMate 3 implantation. A 49-year-old woman with advanced heart failure and previous mechanical mitral

[Acute mitral valve insufficiency caused by a broken mitral valve prosthesis].

Breaks in a mechanical heart valve prosthesis with resulting acute valvar insufficiency are extremely rare. The case is reported of a 49-year-old man who, two-and-a-half years after implantation of a mitral valve prosthesis, suddenly went into acute cardiogenic shock, after having been in good health since the valve replacement. Although the symptoms were unequivocal, the correct diagnosis was not made for more than 40 hours. Emergency re-operation could not prevent death from severe interstitial pulmonary oedema with hyaline membranes. Within the last 20 years 25 similar cases have been repor

Mitral valve replacement in infants and children 5 years of age or younger: evolution in practice and outcome over three decades with a focus on supra-annular prosthesis implantation.

OBJECTIVE: Successful mitral valve replacement in young children is limited by the lack of small prosthetic valves. Supra-annular prosthesis implantation can facilitate mitral valve replacement with a larger prosthesis in children with a small annulus, but little is known about its effect on the outcomes of mitral valve replacement in young children. METHODS: One hundred eighteen children underwent mitral valve replacement at 5 years of age or younger from 1976-2006. Mitral valve replacement was supra-annular in 37 (32%) patients. RESULTS: Survival was 74% +/- 4% at 1 year and 56% +/- 5% at

Transcatheter Mitral Valve Implantation Using the HighLife System.

OBJECTIVES: This study is the first report of 2 cases of HighLife (HighLife, Paris, France) implantation in humans. BACKGROUND: Transcatheter mitral valve implantation represents a promising approach to treating mitral regurgitation in patients at increased risk of perioperative mortality. The HighLife transcatheter mitral valve is a 2-component system. The valve is implanted in the mitral position and is anchored by interacting and then reaching an equilibrium position with a previously positioned subannular implant. METHODS: The procedures were successfully performed in a 69-year-old man a

Second transcatheter aortic valve implantation for treatment of suboptimal function of previously implanted prosthesis: review of the literature.

OBJECTIVES: To systematically review reported cases of second transcatheter aortic valve deployment within a previously implanted prosthesis (TAV-in-TAV). BACKGROUND: TAV-in-TAV deployment is one of the rescue strategies undertaken due to an unsuccessful or suboptimal transcatheter aortic valve implantation (TAVI) result. Currently, there are no clear indications for second valve implantation and outcomes of patients with 2 prostheses deployed remain poorly known. METHODS: The MEDLINE and PubMed databases were searched for cases of TAV-in-TAV implantations of aortic valve. RESULTS: Forty-th

Hybrid trans-apical atrioventricular valve-in-valve implantation in Fontan circulation.

Trans-apical approach has been proved successful in failing surgical bio-prosthesis in both mitral and aortic position in adult patients. Recently, valve-in-valve treatments have been applied even in patients with complex congenital heart disease. Here, we report the case of a 32 years old lady with left atrial isomerism, complete AV septal defect, interrupted inferior vena cava with azygos continuation who underwent Kawashima procedure with atrial Fontan. Severe systemic atrioventricular valve regurgitation necessitated a 33 mm Perimount valve implantation and conversion to lateral tunnel Fon

Percutaneous treatment with the 'valve-in-valve' technique in congenitally corrected transposition of the great arteries and systemic atrioventricular valve with severe double lesion: case report.

BACKGROUND: Congenitally corrected transposition of the great arteries (CCTGA) is a rare congenital heart disease characterized by atrioventricular (AV) and ventriculoarterial discordance, allowing near-normal circulation without early surgical correction. However, long-term complications such as valvular dysfunction, systemic right ventricular (RV) failure, and pulmonary hypertension are common. Management becomes particularly challenging in patients with prior interventions, where anatomical complexity and elevated surgical risk limit therapeutic options. CASE SUMMARY: We present the case o

Controversy pro: Mechanical AVR for better long-term survival of 50-70 years old.

Selection of the most appropriate type of aortic valve prosthesis (mechanical or biologic) for patients 50-70 years of age is a matter of frequent debate. The purpose of this article is to review overlooked concepts and misconceptions in valve-related complications, prosthesis durability, and late survival to aid decision making in contemporary practice. A trend favoring improved long-term survival was found among patients who receive a mechanical prosthesis compared to a biologic substitute. Additionally, an acceptably low rate of long-term valve-related thromboembolism and hemorrhage was fou

Prosthetic heart valves with silver-coated sewing cuff fabric: early morphological features in two patients.

BACKGROUND: The current generation of prosthetic heart valves are excellent examples of engineering that work well, for long periods of time. Unfortunately, host-related problems and consequent valve failure continue to occur, including infective endocarditis. St Jude Medical Inc introduced a new model of mechanical heart valve prosthesis with silver-treated fabric to try to reduce the incidence of infective endocarditis. Two cases of failure of this prosthesis, with a clinical diagnosis of infective endocarditis, are presented. MATERIALS AND METHODS: Of the 176 St Jude Medical Silzone-coated

Compassionate use of the self-expandable medtronic CoreValve prosthesis for the treatment of pure aortic regurgitation in a patient at prohibitive risk for surgical valve replacement.

Transcatheter aortic valve implantation (TAVI) is a viable treatment option for high- and prohibitive-risk patients with severe, calcified pure or predominant aortic valve stenosis, but not for pure aortic valve regurgitation. In fact, the use of TAVI for this indication is even considered unlikely due to the lack of calcium which appears essential for anchoring the stent-valve and prevents dislocation. We report a case of a patient with severe, symptomatic pure aortic regurgitation, and a history of two previous open-heart surgeries who was successfully treated by compassionate use implantati

Transapical mitral valve-in-valve implantation: a novel approach guided by three-dimensional transoesophageal echocardiography.

Prosthesis deterioration rate, years after a previous surgical valve replacement, is rising. Usually, the standard management is reoperation, but for very high risk patients an alternative has arisen: the valve-in-valve approach. We present an 84-year-old Caucasian woman with a mitral bioprosthesis (Mosaic II, number 29) since 1994. Over the last few months the patient displayed worsening heart failure symptoms, until her current admission in NYHA III-IV functional class, because of a severely degenerated mitral prosthesis (severe regurgitation, severe pulmonary hypertension). The transapical

Combined percutaneous treatment of severe aortic stenosis and left main disease in a high surgical risk patient.

We report the case of a 77-year-old man with heart failure (NYHA class IV), angina (CCS class III) and multiple comorbidities. The evaluation showed severe aortic sten osis and left main coronary disease. Due to the very high perioperative risk, conventional surgery was denied. It was therefore decided to perform a combined percutaneous intervention. First, left main PCI was successfully performed. A few days later, a percutaneous aortic valve prosthesis was implanted via a femoral approach using the CoreValve ReValving system. Immediately after prosthesis implantation, the control angiogram s

Intraoperative ECMO assistance during TAVI for aortic insufficiency in an Asian patient with LVAD support.

A 69-year-old male underwent concomitant left ventricular assist device (LVAD) implantation and aortic valve repair procedure for moderate aortic insufficiency (AI). Due to recurrence of symptomatic heart failure and post-repair AI deterioration from trivial to moderate-to-severe insufficiency 2 months later, transcatheter aortic valve implantation (TAVI) was arranged. During TAVI, intraoperative extracorporeal membrane oxygenation was used to maintain stable haemodynamics and to ensure a safe environment for prosthesis deployment. Postoperative 5 month follow-up revealed no AI and no paravalv
