Pathergy of a Medial Heel Schwannoma.

Schwannomas are benign soft-tissue tumors derived from Schwann cells of the peripheral nervous system. They typically appear on the head, neck, or trunk, and are often asymptomatic or present with mild tenderness to palpation and numbness due to nerve compression. A 17-year-old male was referred to our dermatology clinic for evaluation and treatment of an asymptomatic, pink, flesh-colored subcutaneous nodule on the medial right heel. A biopsy was performed to rule out malignancy, with the pathology report consistent with the diagnosis of schwannoma. Following the biopsy, the patient developed

Imaging the patient with heel pain.

Heel pain is a common presentation in primary care and affects a wide range of the population but predominantly elderly, obese and athletic patients. History and clinical assessment are paramount in the management of this condition but the presentation can confound clinicians, necessitating the use of imaging to confirm or clarify the diagnosis when there is clinical uncertainty. This article illustrates the various conditions producing heel pain to help clinicians determine the appropriate imaging modality to image the common causes of heel pain. A linked article detailing the management of h

Epidermal inclusion cyst of the heel.

An inclusion cyst develops from the introduction of epidermal fragments into a heterotopic location, where it grows through accumulation of epithelial and keratinous debris. This is caused by a combination of increased physical activity and the imposition on the site. Inclusion cysts are seen most frequently in younger and more active patients, rather than in older individuals. This case is unique because the epidermal inclusion cyst occurred on the plantar aspect of the foot of an elderly, sedentary patient who had not sustained any injury to the area.

Comparison of ultrasonography and radiography in assessment of the heel pad compressibility index of patients with plantar heel pain syndrome. Measurement of the fat pad in plantar heel pain syndrome.

AIMS: The shock absorption of the heel fat pad is assessed by heel-pad compressibility index measuring the thickness of the heel pad on the lateral radiographs unloaded and loaded by body-weight. Ultrasonography is an easily applicable method without radiation and magnification inherent in radiographs. There are no reports comparing these measurements by diagnostic ultrasonography and direct radiography in healthy individuals and in different clinical entities. The efficacy of ultrasonography in assessment of heel-pad compressibility index in patients with plantar heel pain syndrome was invest

Extramedullary Disease-Achilles Heel in Myeloma?

Despite advances in therapy, extramedullary disease (EMD) remains an aggressive form of multiple myeloma associated with poor outcomes. Patients with true EMD, in which plasmacytomas have become completely independent of bone, have a particularly poor prognosis. The pathogenesis of EMD is driven by complex mechanisms involving loss of adhesion molecules, heterogeneous genetic and epigenetic changes, and a solid tumor-like architecture within the microenvironment. Although the introduction of advanced imaging techniques and immunotherapy has led to improved detection and more promising outcomes

Percutaneous drilling for chronic heel pain.

The authors report a retrospective study involving 25 feet in 21 patients who underwent percutaneous drilling for chronic heel pain. Patients with increased activity of the heel were considered for surgical treatment if there was increased uptake on the delayed bone scans. The average follow-up was 21 months (range, 6 to 30 months). All patients were treated in day surgery with local anesthesia. Three small holes were bored in the medial cortex of the calcaneus. Clinical evaluation of the parameters of pain, walking distance, fascial tenderness, paresthesias, and ankle and subtalar joint motio

Transient vibrations caused by heel strike.

Transient vibrations caused by heel strike and travelling vertically through the body have been monitored using accelerometers taped to the skin. A correction for skin movement based on the transmissibility function is described. The average velocity between the foot and the head for the heel-strike transient is 220 m/s. This velocity is greatest in the legs, 610 m/s, and least in the spine, 90 m/s. Shock absorption occurs mainly in the legs and to a lesser degree in the spine.

MR Imaging Findings in Heel Pain.

Heel pain is common and due to a variety of osseous and soft tissue disorders. Causes of heel pain can be classified by the anatomic structure in which they arise and include disorders of the (1) Achilles tendon, (2) plantar fascia, (3) calcaneus, (4) bursae, (5) nerves in the hindfoot, (6) and heel pad. Although careful history taking and physical examination are important, imaging plays a vital role in the diagnosis and management of heel pain. MR imaging is the best imaging test to assess pain in the heel. This review discusses the common causes of heel pain, focusing on MR imaging appearan

Skewfoot (forefoot adduction with heel valgus).

Skewfoot, a malalignment of tarsal and metatarsal bones, is recognized clinically by forefoot adduction and heel valgus. Synonyms include S-shaped foot, serpentine foot, and Z-foot deformity. Skewfoot must be differentiated from metatarsus adductus, metatarsus varus, and metatarsus adductovarus. Only 50 cases have been reported, and only two primary articles have been published in the English literature. Four patients who were treated at the Mayo Clinic are described. The ages of the patients at the time of presentation ranged from 6 months to 3 years. Three patients underwent operation. Becau

Heel deformity in hereditary spastic paraplegia.

The association of pes cavus deformity and neurological disorder is well known. Varus deformity of the heel is often associated with, and may even precede the development of pes cavus. Clinical and radiographic examinations of the feet of members of three kindreds of hereditary spastic paraplegia, a rare neurologic disorder, suggested that the autosomal dominant form manifests a significantly higher incidence and degree of heel varus deformity than the autosomal recessive form of the disease. Therefore, heel varus deformity may be a sign suggestive of autosomal dominant inheritance of heredita

Convective burn from use of hairdryer for heel warming prior to the heel prick test - a case report.

BACKGROUND: Blood sampling through heel lancing is the most common invasive painful procedure performed on newborn infants. CASE PRESENTATION: We report the case of a five day old infant who sustained burns to the left foot and leg after the mother's hairdryer was used by the midwife to warm the baby's heel prior to capillary blood sampling (CBS) with an automated device. CONCLUSION: Heel warming is not recommended for routine CBS although it is often practiced. If pre-warming is to be practiced, standardised devices should be used rather than improvised techniques. This will reduce the risk

Comparative study of MRI and power Doppler ultrasonography of the heel in patients with spondyloarthritis with and without heel pain and in controls.

INTRODUCTION: Imaging of heel enthesopathy in spondyloarthritis (SpA) could potentially be useful for diagnosis and monitoring. The aim of this study was to assess the diagnostic capacities of MRI and power Doppler ultrasonography (PDUS) of the heel to distinguish patients with SpA from controls and to distinguish between patients with SpA with and without enthesopathy. METHODS: A cross-sectional single-centre study was performed in 51 patients (102 heels) with definite SpA according to Amor's criteria. Patients with degenerative non-inflammatory low back pain (n=24, 48 heels) were included a

Mitochondria: The Retina's Achilles' Heel in AMD.

Strong experimental evidence from studies in human donor retinas and animal models supports the idea that the retinal pathology associated with age-related macular degeneration (AMD) involves mitochondrial dysfunction and consequent altered retinal metabolism. This chapter provides a brief overview of mitochondrial structure and function, summarizes evidence for mitochondrial defects in AMD, and highlights the potential ramifications of these defects on retinal health and function. Discussion of mitochondrial haplogroups and their association with AMD brings to light how mitochondrial genetics

[Hidradenocarcinoma of the heel associated with inguinal metastases].

BACKGROUND: Hidradenocarcinoma is a rare malignant tumour involving the sweat glands. It classically arises de novo, only rarely resulting from pre-existing hidradenoma. The literature contains few reports of lymph node metastasis in this tumour. We report a case of a patient with hidradenocarcinoma of the heel associated with inguinal node metastases. PATIENTS AND METHODS: We report the case of a 64-year-old patient with a history of chronic smoking, who in the last two years developed a painless nodule in his right heel, with no prior injury, and which gradually increased in size to become
