Computational modeling of HHH therapy and impact of blood pressure and hematocrit.

BACKGROUND: After an aneurysmal subarachnoid hemorrhage, cerebral microcirculatory changes occur as a result cerebral vasospasm. The objective of this study is to investigate, with a computational model, how various degrees of vasospasm are influenced by increasing the mean blood pressure and decreasing the blood viscosity. METHODS: Using ANSYS CFX software, a computational model was constructed to simulate steady-state fully developed laminar blood flow through a rigid wall system consisting of the internal carotid artery (ICA), anterior cerebral artery, posterior cerebral artery, and middle

Critical hematocrit and oxygen partial pressure in the beating heart of pigs.

In cardiac surgery the substitution of lost blood volume by plasma substitutes is a common therapeutical approach. None of the currently available blood substitutes has a sufficient oxygen transport capacity. This can limit the functional integrity of the myocardium known as highly oxygen consumptive. The study was aimed to get information about the minimal hematocrit, also known as critical hematocrit (cHct), which guarantees a stable and adequate oxygen partial pressure in the myocardium (pO2). In adult female pigs (n=7) the hematocrit was reduced by isovolemic blood dilution with an intrave

Optical measurement of hematocrit and other biological constituents in renal therapy.

Optical sensors have advanced significantly over the past 2 decades leading to today's noninvasive optical measurement capabilities and their widespread applications in renal therapy. These measurements provide significant advantages to the clinician. For example, a given blood constituent can be monitored in real time (continuously, nondestructively), which facilitates the ability to optimally "titrate" the therapy with immediate visual feedback. Optical methods have another intrinsic advantage in that each biologic constituent has its own unique spectral "signature" allowing for simultaneous

Prospective crossover study of the effect of phlebotomy and intravenous crystalloid on hematocrit.

OBJECTIVE: To compare the changes in hematocrit (Hct) between phlebotomized and nonphlebotomized individuals given IV crystalloid. METHODS: A prospective, crossover volunteer study was performed comparing Hct changes immediately and 30 minutes after IV crystalloid bolus in 20 healthy adults with and without prebolus phlebotomy. In the control portion, volunteers were given a 15-mL/kg bolus of normal saline over 30 minutes with Hct determination before (H1), immediately after (H2), and 30 minutes after (H3) crystalloid infusion. At least 7 days later, the same subjects were phlebotomized 1 uni

Hematocrit and whole blood viscosity in newborns: analysis of 100 cases.

Hematocrit (Hct) and whole blood viscosity was studied at a mean age of ten hours in 100 neonates. Group A (n = 25), were term normal newborns, Group B (n = 25) were preterms, Group C (n = 20) were term small for gestation (SGA) and Group D (n = 30) had perinatal hypoxia. Blood viscosity was estimated in all cases at shear rates 94.5, 51.2, 20.4 and 8.1 and intergroup variability in viscosity compared at shear rate 51.2. The mean hematocrit (Hct) (59.4%) and viscosity (8.2 cps) was higher in Group A as compared to other groups, but the difference was not significant (p greater than 0.05). The

Hematocrit modulates response of ANP to volume expansion in immature rats.

Compared with the adult, the immature kidney responds to acute volume expansion (VE) with reduced diuresis and natriuresis. The underlying mechanisms are poorly understood. Diuresis and natriuresis are blunted despite a lower hematocrit (Hct) in the immature rat, which should enhance the response to VE. The present study was designed to evaluate the role of atrial natriuretic peptide (ANP), its second messenger, guanosine 3',5'-cyclic monophosphate (cGMP), and Hct in the regulation of the renal response to acute saline VE (3% body wt) during postnatal development in rats. Anesthetized Sprague-

Screening for hypoglycemia with plasma in neonatal blood of high hematocrit value.

A model utilizing human umbilical cord blood was used to screen for hypoglycemia in a simulated neonatal situation. The aims of the study were to assess the effect of increasing Hct concentration on Dextrostix readings, and to determine whether plasma samples, in contrast to conventional whole blood samples, were acceptable for Dextrostix determinations in selected cases. Dextrostix readings were determined on 65 whole blood samples of varying Hct, and on plasma specimens, 48 of which were paired with whole blood samples. The results were compared with plasma true glucose values. Plasma Dextro

[Effect of atrial natriuretic factor on the hematocrit of patients in chronic hemodialysis].

A synthetic atrial natriuretic peptide (ANF) has been administered to 7 patients with chronic renal failure to evaluate the role of ANF in the regulation of blood volume. ANF (2 micrograms/min) was infused for 60 minutes before hemodialysis and blood pressure, heart rate, plasma proteins and hematocrit were measured at regular intervals. Although a slight decrease in blood pressure occurred during ANF infusion, no significant changes in hematocrit and plasma proteins were observed. These results suggest that administration of exogenous ANF does not modify the volume distribution in patients wi

Is the erythropoietin-hematocrit feedback control operative in chronic renal failure?

Transfusion studies in 23 chronic uremic patients with low hematocrit (HCT) values showed that (1) the pretransfusional immunodetectable (id) serum erythropoietin (EP) levels were within the normal range for healthy people; (2) the posttransfusional idEP levels remained unchanged; (3) 3 patients with analgesic nephropathy and high pretransfusional bioassayed (bio) EP levels had markedly suppressed posttransfusional bioEP, 6 patients with other kidney diseases had undetectable bioEP levels; (4) the raised pretransfusional reticulocyte (RETI) counts were markedly reduced by transfusions in all p

Time-updated patterns of hemoglobin and hematocrit and the risk of CKD progression.

BACKGROUND: Early intervention and management of anemia, particularly the commonly used measures of hemoglobin (Hb) and hematocrit (HCT), are important in slowing and preventing the progression of chronic kidney disease (CKD). However, the optimal range for regulating Hb and HCT levels remains uncertain. OBJECTIVE: The aim of this study was to elucidate the intrinsic relationship between Hb and HCT and the short- and long-term prognosis of CKD, and determine optimal ranges for Hb and HCT. METHODS: We retrospectively collected demographic and clinical data over a 6-year follow-up period in Li

Serum alanine aminotransferase is correlated with hematocrit in healthy human subjects.

BACKGROUND: Serum alanine aminotransferase (ALT) activity is a widely-used surrogate marker for liver injury. However, mild elevation of serum ALT is frequently observed in apparently healthy individuals, making it sometimes challenging to interpret whether this laboratory abnormality is medically benign or serious. To obtain a better understanding of the factors influencing ALT levels, we examined the relation between ALT and a number of anthropometric and biochemistry measurements in humans. METHODS: We assessed the associations of ALT with hematocrit (HCT) in 1,200 apparently healthy adult

Hyperoxic ventilation at the critical hematocrit: effects on myocardial perfusion and function.

BACKGROUND: Hemodilution reduces hematocrit (Hct) and blood oxygen content. Tissue oxygenation is mainly preserved by increased cardiac output. As myocardial O2-demands increase, coronary vasodilatation becomes necessary to increase myocardial blood flow. Myocardial ischemia occurs at a critical Hct-value (Hctcrit), with accompanying exhaustion of coronary reserve. Hyperoxic ventilation is known to both reverse peripheral tissue hypoxia at Hctcrit and also to induce coronary vasoconstriction. This study aimed to determine whether hyperoxic ventilation at Hctcrit further exacerbates myocardial

Hematocrit as an indicator of blood volume and a predictor of intradialytic morbid events.

Hematocrit (H) levels can change during hemodialysis, and these changes in H are inversely related to changes in blood volume (BV). The objectives of this study were to determine whether mean arterial pressure (MAP) decreases with decreasing BV and rising H during hemodialysis, and to determine the relationship between dialysis induced intravascular volume depletion and intradialytic morbid events (IME), defined as hypotension, cramping, or lightheadedness that led to dialysis staff intervention. We monitored H continuously using a noninvasive optical technique in 93 hemodialysis sessions in 1
