Acute subdural hematoma associated with nontraumatic aneurysm rupture.

Five cases of subdural hematoma due to aneurysm rupture and their surgical treatment are reviewed. One patient recovered completely, one was severely disabled and 3 patients died. The literature regarding subdural hematomas associated with nontraumatic aneurysm rupture is reviewed and our therapeutical strategy is discussed. We conclude that severe neurological deficit and uncal herniation can be reversible if surgery is performed early.

[Three cases of non-traumatic acute subdural hematoma].

The authors present three cases of non-traumatic acute subdural hematoma showing interesting clinical features and operative findings. Case 1: A-50-year-old male was admitted because of sudden headache and epileptic seizure. Computed tomographic (CT) scan showed a right thin subdural hematoma, but cerebral angiography demonstrated no pathological findings, that might cause acute subdural hematoma on the follow-up CT scans. The hematoma changed to a chronic one within only 15 days, which was proved by the operation. Case 2: A 52-year-old male was hospitalized because of loss of consciousness. C

Spinal subdural hematoma, neurilemmoma, and acute transverse myelopathy.

A 74-year-old woman had acute cervical transverse myelopathy from a neurilemmoma. Although extremely rare, the possibility of tumor should be recognized in patients with apparently spontaneous spinal subdural hematoma. As in other cases of compressive myelopathy, early operation is essential for recovery of neurological deficit.

Rapid reduction of acute subdural hematoma and redistribution of hematoma: case report.

An 88-year-old woman presented with acute subdural hematoma (ASDH) which showed rapid resolution on computed tomography (CT) and magnetic resonance (MR) imaging. She was transferred to our hospital after falling out of bed. On admission, she was comatose with Japan Coma Scale score of 200 and Glasgow Coma Scale score of E1V1M2. Brain CT showed a thick left frontotemporal ASDH. Conservative treatment consisted of 200 ml of glycerol administered intravenously twice a day, and maintenance in the approximately 20 degree head-up position to reduce intracranial pressure. Three days later, her consci

A convexity meningioma presenting with an acute subdural hematoma.

BACKGROUND: Meningiomas presenting with acute subdural hematomas are extremely rare. To the best of our knowledge, only 45 cases have been reported to date. We report on a case of a meningioma mimicking an acute subdural hematoma as well as a thorough literature review. CASE DESCRIPTION: A 67-year-old man with no history of trauma was referred to our hospital with sudden onset of decreased level of consciousness and left hemiplegia. Computed tomography revealed an acute convexity subdural hematoma. Emergency surgery to remove the hematoma was performed. The hematoma was found to exist in the

[A case of small meningioma with acute subdural hematoma].

A 46-year-old female was transported to our clinic 4 hours after the onset of sudden headache and transient loss of consciousness. On admission, she was drowsy with slight left hemiparesis and anisocoria (left greater than right). The CT scan revealed a right subdural neurological deficit one month after the operation. the operation, we found that the hematoma was derived from a small meningioma attached to the dura at the middle temporal fossa. The meningioma was 1.5 cm in diameter. The patient was discharged without neurological deficit one month after the operation. Although glioblastoma an

Acute Surgery vs Conservative Treatment for Traumatic Acute Subdural Hematoma.

IMPORTANCE: It is unclear whether performing surgery for most patients with an acute subdural hematoma (ASDH) and traumatic brain injury (TBI) is superior to conservative treatment. OBJECTIVE: To compare the effectiveness of a strategy preferring acute surgical ASDH evacuation with one preferring initial conservative treatment. DESIGN, SETTING, AND PARTICIPANTS: This comparative effectiveness study used data from February 1, 2014, to July 31, 2018, from the prospective observational Transforming Research and Clinical Knowledge in Traumatic Brain Injury Study, conducted at 18 Level 1 trauma c

Effect of Antithrombotic Therapy on Development of Acute Subdural Hematoma After Burr Hole Drainage of Chronic Subdural Hematoma.

AIM: To evaluate the relationship between the time from cessation of anticoagulant/antiplatelet medication to surgery and risk of postoperative acute subdural hematoma (ASDH) after burr hole drainage of chronic subdural hematoma (CSDH). MATERIAL AND METHODS: A retrospective study of patients who underwent burr hole drainage of CSDH between December 2014 and December 2019 was performed. Demographic and clinical data regarding age, gender, medication (antithrombotic therapy), smoking, daily alcohol consumption, history of head trauma, presenting symptoms, and neurological examination were colle

Endoscopic Hematoma Evacuation for Acute Subdural Hematoma in the Elderly: A Preliminary Study.

AIM: Endoscopic surgery for acute subdural hematoma (ASDH) has recently been performed in elderly patients. Unlike other reported procedures, we used the endoscope's fixation device to ensure hemostasis because the surgeon can then use a surgical instrument with each hand. To date, there have no reports that prove lesser invasiveness or noninferiority of endoscopic surgery based on direct comparison with a large craniotomy. In this study, we investigated whether our endoscopic procedure was effective and safe. MATERIALS AND METHODS: We performed 6 cases of endoscopic surgical procedures in el

Coexistence of ipsilateral acute-on-chronic subdural hematoma and acute extradural hematoma: A case report.

Chronic subdural hematomas are typically observed in elderly patients receiving antithrombotic and/or anticoagulant therapy. In contrast, acute subdural and extradural hematomas are often observed in young people with traumatic brain injury. The coexistence of ipsilateral chronic subdural and extradural hematomas is rare. Depending on the Glasgow Coma Scale and neuroimaging findings, early surgical intervention is mandatory, as seen in our patient. Early surgical evacuation of a traumatic extradural and chronic subdural hematoma should be done. Also, antithrombotic drug use can lead to chronic

Contralateral acute epidural hematoma after decompressive surgery of acute subdural hematoma: clinical features and outcome.

BACKGROUND: Delayed contralateral epidural hematoma (EDH) after decompressive surgery for acute subdural hematoma (SDH) is uncommon. If unrecognized, this delayed hematoma can cause devastating consequences. We present our experience with this group of patients and discuss the diagnosis and management of this dangerous condition. METHODS: This study included 12 traumatic patients with acute SDH who developed delayed contralateral EDH after acute SDH evacuation. Clinical and radiographic information was obtained through a retrospective review of the medical records and the radiographs. RESULT

Anterior STEMI After Acute Subdural Hematoma: A Management Dilemma.

Management of acute coronary syndrome in patients with a high bleeding risk requires careful clinical judgment to weigh the benefit of percutaneous intervention and subsequent antiplatelet pharmacotherapy with the risks of bleeding. A 70-year-old man developed anterior ST-segment elevation myocardial infarction 4 days after undergoing neurosurgical evacuation of subdural hematoma. After successful percutaneous intervention with plain-old balloon angioplasty, cangrelor was started owing to bleeding risk. The subdural hematoma remained stable during dual antiplatelet therapy (DAPT) therapy. In a

Acute-Onset Hemiparkinsonism Secondary to Subacute-Chronic Subdural Hematoma.

Subdural hematomas constitute rare causes of secondary Parkinsonism in elderly. Subacute or chronic subdural hematomas occur in the elderly following minor head trauma or even without a remarkable history of trauma. A 69-year-old woman admitted with a rapidly progressive acute-onset hemiparkinsonism on the left side of her body. She denied any precipitating event before the onset of her symptoms, and her medical history was unremarkable. The anti-Parkinsonian therapy showed no benefit, but gradually worsening of the symptoms was observed. Her brain magnetic resonance imaging revealed a large s

Nonsurgical acute traumatic subdural hematoma: what is the risk?

OBJECT: The Brain Trauma Foundation has published guidelines on the surgical management of traumatic subdural hematoma (SDH). However, no data exist on the proportion of patients with SDH that can be selected for conservative management and what is the outcome of these patients. The goals of this study were as follows: 1) to establish what proportion of patients are initially treated conservatively; 2) to determine what proportion of patients will deteriorate and require surgical evacuation; and 3) to identify risk factors associated with deterioration and delayed surgery. METHODS: All cases

Spontaneous rapid resolution of acute subdural hematoma in children.

BACKGROUND: Spontaneous rapid resolution of acute subdural hematoma developing secondary to trauma has been reported in the literature, yet it is very rare in pediatric population. The aim of the present review is to analyze mechanisms, characteristics, and outcomes of pediatric acute subdural hematoma cases with spontaneous rapid resolution in conjunction with an exemplary case of ours. CASE DESCRIPTION: A 3-year-old boy was admitted to our emergency department following mild head trauma secondary to a fall from 2 m. He was alert and has a GCS of 15. He had no motor or sensorial deficit on n

Infected acute subdural hematoma associated with invasive pneumococcal disease.

A previously healthy 68-year-old woman presented with a rare case of subdural empyema which developed at the site of preceding acute subdural hematoma (SDH). She was first admitted for treatment of an acute SDH after a fall. Since she was neurologically intact and the SDH volume decreased with conservative management, she was discharged 9 days after admission for follow up as an outpatient. Three days after discharge, she unexpectedly returned with worsening headache and altered mental status. Brain computed tomography (CT) showed increased SDH volume. Her condition deteriorated rapidly after

Spontaneous acute subdural hematoma contralateral to an arachnoid cyst.

Arachnoid cysts (AC) are extra-cerebral cerebrospinal fluid collections of unknown origin. They correspond to 1% of all intracranial nontraumatic space-occupying lesions and appear more frequently in the middle fossa (50%). More than 25% of these cysts are incidental findings and the majority of patients are asymptomatic. Seizures, intracranial hypertension signs, neurological deficits, macrocrania, developmental delay and bulging of the skull are the main signs and symptoms of the lesion. AC rupture and bleeding are rare, usually occurring in young adults and associated with trauma. The risk

Falx meningioma presenting as acute subdural hematoma: case report.

BACKGROUND: Acute subdural hematomas caused by meningiomas have been rarely encountered. Pathophysiologic mechanisms and clinical considerations in these patients have not been sufficiently explored. We addressed the possible mechanism of spontaneous hemorrhage in our case and briefly discuss the optimal treatment. CASE DESCRIPTION: This case of falx meningioma presenting as an acute subdural hematoma in a 78-year-old woman is described. On initial computed tomography (CT), an enhancing tumor of the falx appeared to be the cause of hemorrhage. Only faint contrast staining in the periphery of
