OBJECTIVE: To assess possible long-term excess mortality and causes of death of patients with chronic subdural hematoma (CSDH).
METHODS: A retrospective study (1990-2015) of adult patients (n = 1133, median age = 76 years old, men = 65%) with CSDH identified by ICD-codes and verified by medical records. All patients were followed until death or the end of 2017. Cumulative relative survival ratios and relative excess risks of death (RER) were estimated by comparing patients' mortality with that in the entire regional matched population. The causes of death were compared with a separate referen

Chronic subdural hematoma (CSDH) is a common pathology in the elderly but very rare in young adults. When CSDH occurs in this age group, severe head injury or some promotive factors are usually present. This article reports the case of a 29-year-old female presented at our Emergency Department with a few days' history of progressive frontal headache. Computed tomography scan of the head showed a right frontal CSDH. Only a decreased level of consciousness without focal deficits was present at clinical examination and her medical history was negative for trauma or promotive factors. Blood count

OBJECTIVES: Chronic subdural hematoma (cSDH) frequently presents with headache, but the relationship between hematoma size and headache is not well defined. This study aims to determine the relationship between total hematoma size and headache presentation in cSDH patients while identifying a critical size threshold for headache development.
METHODS: We retrospectively reviewed adult cSDH patients diagnosed at our institution from January 1st, 2023, to June 4th, 2025. Patients with a Glasgow Coma Score (GCS) below 14 or a history of ventricular shunt were excluded. Data collected included dem

Chronic subdural hematoma (cSDH) is an increasingly prevalent condition, particularly among elderly population and is associated with elevated morbidity. The pathophysiology of cSDH involves proliferation of dural edge cells, fibroblasts, and release of vascular angiogenic factors leading to angiogenesis of a vascularized neomembrane perfused by the middle meningeal artery (MMA). MMA embolization is a technique that can be employed as treatment in conjunction with surgery or as a standalone therapy. Our aim is to describe our experience with treating selective cSDH patients with embolization o

Despite its benign nature, chronic subdural hematoma (SDH) can be fatal if surgical intervention is delayed. Here, we report on bilateral chronic SDH in an 84-year-old man who died of duret hemorrhage in the brain stem and ischemia in the occipital and temporal lobes. We discuss the necessity for urgent surgical intervention to treat bilateral chronic SDH, and provide a review of the relevant literature.

Chronic subdural hematoma (cSDH), previously considered fairly benign and easy to treat, is now viewed a possible sign of incipient clinical decline. We investigated case-fatality, excess fatality and need for reoperations following operated cSDH in a nationwide setting focusing on patient-related characteristics. Finnish nationwide databases were searched for all admissions with operated cSDH as well as later deaths in adults (≥ 16 years) during 2004-2017. There were 8539 patients with an evacuated cSDH (68% men) with a mean age of 73.0 (± 12.8) years. During the follow-up, 3805 (45%) patient

Chronic subdural hematoma (cSDH) is a common disease process associated with significant morbidity that occurs most often in elderly patients. Asymptomatic patients are typically treated conservatively, with surgical intervention reserved for patients with symptomatic and/or large hematomas that cause brain compression. However, conservatively managed cSDH cases frequently progress, and surgical evacuation of cSDH is associated with high rates of complication and recurrence. Recently, successful treatment of cSDH via middle meningeal artery (MMA) embolization has been reported in small case se

OBJECTIVE: Some studies demonstrated the role of steroids in medical management of chronic subdural hematoma (CSDH). Aim of our study is to evaluate the role of steroids in medical treatment of CSDH and to evaluate the subgroup of patients who respond to steroids.
METHODS: This prospective case study was done for 26 months from April 2013 to May 2015. On admission, the parenteral steroid dexamethasone (4 mg) was given every 8 hours for 72 hours. If the patient improved, the oral tapering doses of steroids were continued for 1 month. Neurological assessment and computed tomography scan done af

In most cases, subdural hematoma (SDH) is regarded as a complication of head injury and nontraumatic causes are rare. Moreover, spontaneous chronic SDH in child or adolescent is very unusual. Here, we present the case of a healthy 14-year-old girl who was diagnosed as a spontaneous chronic SDH. The patient presented with severe headache following blurring of vision two weeks ago without any history of trivial head injury. Computed tomography and magnetic resonance imaging depicted a chronic SDH. The cause of the hematoma was not established. After performing burr hole drainage of the hematoma,

OBJECT: Endoscopic III ventriculostomy (ETV) is an effective and a rather safe treatment for noncommunicating hydrocephalus secondary to aqueductal stenosis and other obstructive pathologies. Though not devoid of risk, ETV is increasingly replacing shunt operations, and it prevents related complications, including overdrainage.
METHODS: We report a rare case of a large chronic subdural hematoma (ChSDH) after ETV in a patient with aqueductal stenosis. Three weeks after he was shunted elsewhere, he presented to us with clinical symptoms of intracranial hypotension and overdrainage. ETV was perf

Histologic investigation was performed in 38 out of 224 chronic subdural hematomas treated at our institution from 1978 to 1988. Nucleated red blood cells were found in 33% of the chronic subdural membranes examined. Additional sections were examined in 14 of these 38 patients, revealing erythroblasts in 57%. The clinical course and radiographic findings in the patients whose membranes contained erythroblasts were compared with those whose specimens did not. Neither history, presentation, size of hematoma, nor postoperative recovery differed from one group to the other. By comparing histologic

A newborn infant demonstrated prenatal, bilateral chronic subdural hematomas with recent hemorrhage, internal hydrocephalus, and macrocephaly at birth. Serial cranial ultrasound and computed tomography evaluations revealed progressive hydrocephalus and subdural hematomas. Following surgical evacuation of subdural hematomas the child developed tension pneumocephalus with clinical deterioration. The pathogenesis of congenital subdural hematoma and rare postoperative complication of tension pneumocephalus are discussed.

In the literature, there has been no report of spontaneously growing calcified chronic subdural hematoma (C-C-SDH). The authors report a five-year-old boy with spontaneously growing C-C-SDH. The patient was admitted to author's department on July 24, 1979, because of progressive right hemiparesis, seizure and mental as well as physical retardation. He had a birth injury caused by forceps delivery resulting in skull fracture and subarachnoid hemorrhage. Three months after birth, gradual enlargement of his head circumference was noticed. A ventriculoperitoneal shunt was performed under the diagn

Two cases of relapsing juvenile chronic subdural hematoma with late relapse in adult life are presented and the literature reviewed. Both patients contracted subdural hematoma early in life; its persistence resulted in characteristic skull deformitites. The patients led an asymptomatic life until a second head trauma caused rebleeding into the old hematoma sac with recurring symptoms and signs. The source of rebleeding is the outer subdural membrane. Radiographic features vary, depending on the location and size of the subdural hematoma. The significance of localized thickening of the cranium

Chronic subdural hematomas (cSDHs) have risen in incidence to ≈17 to 19 cases per 100 000 persons per year over the recent decade, especially in the elderly population, with an estimated incidence of up to 355 cases per 100 000 per year in those ≥85 years. cSDHs are projected to be the most common neurosurgical condition by 2030. Recurrence rates with current treatment approaches, such as surgical evacuation with burr hole craniostomy or craniotomy, are high. These treatments often do not address the underlying inflammatory process, which is a key driver in cSDH development and progression. cS

Chronic subdural hematoma (cSDH) recurrence after burr-hole drainage remains incompletely understood, and the role of adjunctive middle meningeal artery embolization (MMAE) in reducing recurrence is debated. We retrospectively analysed 261 consecutive patients who underwent single burr-hole drainage of a unilateral cSDH between 2017 and 2024 at a single academic centre (71.26 % male; mean age 71.50 ± 13.31 years), of whom 97 received MMAE and 164 did not. Recurrence was defined as reoperation for a recurrent hematoma within 90 days. With only 14 recurrences observed, the multivariable model

Radiologic imaging has become integral in not only the detection and diagnosis of subdural hematoma (SDH) but also in guiding potential treatment options. This is especially true for chronic SDH, which has conventionally been managed via surgical drainage, but can now be treated with embolization of the middle meningeal artery (MMA). We review the imaging manifestations of SDH as a function of chronicity and standardized methods of measurement and identify the MMA and its clinically significant variant anatomy as it pertains to embolization planning. Equipped with a more comprehensive approach
